Item Acesso aberto (Open Access) Análise conformacional da enzima protease do HIV-1 relacionada à resistência ao inibidor Nelfinavir(Universidade Federal do Pará, 2017) HOLANDA, Luiz Henrique Campos; SILVA, Jerônimo Lameira; http://lattes.cnpq.br/7711489635465954; SOUSA, Maisa Silva de; http://lattes.cnpq.br/1775363180781218The Human Immunodeficiency Virus (HIV), which causes acquired immunodeficiency syndrome (AIDS), is a retrovirus that has highly virulent glycoproteins that invade the CD4 + T lymphocyte through its CCR4 and CXCR5 receptors. The biological cycle of HIV is mediated by the protease, transcriptase and integrase enzymes. HIV-1 protease is an enzyme that is present in the final phase of the biological cycle, where virus maturation occurs, and is an important pharmacological target. The main objective of this project is to verify the effects of the D30N, I84A and M46I mutations on the HIV-1 protease enzyme and the complex formation with the nelfinavir inhibitor through molecular dynamics and bioinformatics techniques. The results based on the structural analyzes showed structural differences between the studied systems. The 1OHR system presented a closed conformation, the systems D30N and D30N_I84A_M46I presented semi-open conformation and the D30N_I84A system presented open conformation, in which the latter presented lower free energy value and greater instability in the RMSD analyzes, however the greater flotation of residues Of amino acids. The theoretical analyzes showed the importance in the resistance of the double mutation D30N_I84A and the conformational restructuring capacity of the M46I mutation and catalytic capacity.Item Acesso aberto (Open Access) Detecção laboratorial de Chlamydia trachomatis em escolares da rede pública do estado do Pará com diagnóstico clínico de tracoma(Universidade Federal do Pará, 2012) CARVALHO, Raimunda Marques de; LIMA, Karla Valéria Batista; http://lattes.cnpq.br/9795461154139260; SOUSA, Maisa Silva de; http://lattes.cnpq.br/1775363180781218Trachoma as a leading cause of preventable blindness in the world is a neglected disease related to low socioeconomic conditions and locations without basic sanitation. Present mainly in developing countries brings great harm to public coffers in lost productivity and visual impairment. With the creation of the Alliance for the Global Elimination of Trachoma in 1997 (GET2020) the State of Pará, with support from the Ministry of Health of Brazil, held in the 2006 epidemiological survey of trachoma in school 1st to 4th grades of the official network of education in municipalities with the human development index of less than national average, to discover the prevalence of the disease. Data from the survey proved that the disease was not eradicated, revealing 35 priority municipalities in Pará and prevalence above 5%. A sub-sample of the conjunctiva of clinically positive students was collected to confirm the diagnosis by direct immunofluorescence (DIF). The present study used 52 cryopreserved samples obtained during the epidemiological survey to be analyzed by the methods of DIF and molecular biology to laboratorial identification of Chlamydia trachomatis. We found frequencies of 26.92% (14/52) and 49% (24/49) of positive results by DIF techniques and polymerase chain reaction (nested-PCR), respectively. Considering the 49 samples analyzed by two methods, the sensitivities for detection of the etiologic agent, by FAT and PCR were 28.57% (14/49) and 48.98% (24/49), respectively (p = 0,0127). The two techniques together confirmed 57.14% (n = 28) of samples with infection, where 50% (n = 14) were positive only by PCR, 35.72% (n = 10) for both technical and 14, 28% (n = 4) only by the IFD. The nucleotide sequence analysis of seven isolates showed homology to C. trachomatis genotype L1. These studies is a pioneer in Brazil, as well as confirm the presence of C. trachomatis in samples from ocular school clinically positive for trachoma, validated protocol for obtaining DNA from cryopreserved sheets DIF, showed the highest sensitivity of the molecular method front DIF and identified the genotype L1 in the sample of the conjunctiva of clinically positive students.Item Acesso aberto (Open Access) Diversidade clínica e peculiaridades da transmissão do vírus Linfotrópico-T humano em famílias da Região Metropolitana de Belém-Pará(Universidade Federal do Pará, 2016-06-23) COSTA, Carlos Araújo da; SOUSA, Maisa Silva de; http://lattes.cnpq.br/1775363180781218The human T-lymphotropic virus type 1 (HTLV-1) and type 2 (HTLV-2), were the first retrovirus discovered in humans and named as such because they inhabit the T-lymphocytes. Both mainly transmitted by being transferred from mother to child due infected lymphocytes in breastfeeding or from a man to a woman by semen. HTLV-1 is the causative agent of an incapacitating myelopathy (HAM/TSP) and Adult T-cell leukemia / lymphoma (ATLL), among other diseases. The high endemicity of such viruses in households of the metropolitan area of Belém-Pará requires investigations to better characterize this viral spread and morbidity. This study was developed in order to identify and characterize the clinical diversity and peculiarities the HTLV transmission in families of the metropolitan region of Belém-Pará. Between 2007 and 2015, we investigated 178 family groups that are confirmed carriers of the virus (index cases). From these families, 140 have HTLV-1 and 38 have HTLV-2, of which 433 members were spontaneously subjected to serologic testing of anti-HTLV-1/2 antibodies. Such test consists on the ELISA method and the detection test of proviral DNA in the blood (PCR). Routes of infection and clinical aspects were evaluated in all individuals and families. From the 611 surveyed 64.6% are female and 37.9% male with, mean age of 41 years. The transmission of HTLV occurred in 92 families (51.8%) with occurrence of three to four infected per family. The infection was more prevalent in females (p< 0.0001) and sexual contact was greater than the vertical (p = 0.0002). 44.3% (62/140) from the index cases of HTLV-1 and 9.9% of their family contacts (11/111) were recognized as symptomatic. The most common diagnostic modalities were neurological (21.4%) and dermatological (19.3%), with a prevalence of neuromuscular osseous symptoms, dysautonomia and skin changes. The main diseases caused by the HTLV-1, out of the 251 infected cases, were: HAM/TSP 7.2% (18); Lymphoproliferative Diseases 2.6 % (6); Strongyloides hyper infection 1.6% (4); Dermatitis Infective in 1.2% (3) and uveitis in 0.4% (1). The parenteral route was the most common form of transmission of HTLV-1 in the causation of neurological diseases (HAM/TSP) whereas the vertical route was the most common form in lymphoproliferative diseases (including ATLL). In some families, HTLV-1 was inlaid spread for generations in a "sui generis" form, mimicking a genetic factor, differentiating itself from other pathogens. Adenoviruses (HAdV) are icosahedral non-enveloped viruses, possess 240 proteins "hexon" specific and a double-stranded DNA. They belong to the Adenoviridae family, Mastadenovirus genus, and are assigned to 7 species (A to G) and 57 serotypes. Epidemiological studies found that HAdVs account for 2-14% of cases of acute childhood diarrhea in hospitals and outpatient units. The aim of this study was to determine the prevalence and to assess the epidemiological features of of HAdV infection, as well as the occurrence of types in 842 children under three years of age, hospitalized for acute gastroenteritis in Belém, Brazil. All of these children had been vaccinated against rotavirus.These children were selected from a previous rotavirus vaccine case-control study conducted at Instituto Evandro Chagas, Belém, Pará, from May 2009 to April 2011. ELISA and immunochromatography were used for HAdV screening in stool samples and PCR and oligonucleotide sequencing for typing and molecular characterisation. HAdVs were found in 7.2% (61/842) of the tested samples, with the enteric adenoviruses (EAD) accounting for 50.8% (31/61) of the HAdV positive cases. The distribution of HAdV according to the gender showed that 7.7% (28/362) of whom were female and 6.8% (33/480) were male. The highest prevalence rates of HAdV infection were seen among patients older than 24 months of age, corresponding to 8.9% (16/178) of all positive cases. In regards to the temporal distribution, the highest prevalence rate was observed in June, representing 11.4% (8/70) of cases overall. The oligonucleotide sequencing showed that HAdV species F was predominant in our region, accounting for 64.5% (29/45) of the sequenced samples. These samples included HAdV types 41 and 40, detected at rates of 69% (20/29) cases and 31% (9/29), respectively. The results of this study indicate that HAdV accounts for a significant proportion of gastroenteritis among hospitalized children aged less than 3 years, during a post-rotavirus vaccine introduction period in Belém, Pará state, Brazil.Item Acesso aberto (Open Access) A implicação do papillomavírus humano no câncer de laringe: revisão sistemática(Universidade Federal do Pará, 2013-03-08) BARROS, João Guilherme Pereira; ANDRIOLO, Regis Bruni; http://lattes.cnpq.br/1205071530087205; SOUSA, Maisa Silva de; http://lattes.cnpq.br/1775363180781218Background: Cancer of the larynx is one of the most common in men after age 50, reaches over the head and neck region, representing 25% of malignant tumors that affect this area and 2% of all malignancies. Approximately two thirds of these tumors arise in the true vocal cord and 1/3 is located above the vocal cords. The relationship between HPV and the diseases of the upper airways has been known for almost a century, but only in the last three decades has its potential oncogenic activity as recognized in the literature. HPV types similar to those found in the cervix were also observed in cancer of the larynx, tongue and oropharynx. Objectives: To assess the frequency of HPV in laryngeal cancer samples, identify the genotypes of HPV in laryngeal cancer sample; establish the relationship between laryngeal cancer and HPV as a risk factor. Methods: Systematic review of clinical trials in which descriptors and synonyms for Laryngeal Neoplasms and Papillomavirus Infections were used in the following electronic databases by March 2012: CENTRAL, MEDLINE (PUBMED), LILACS and SciELO. Three reviewers selected, assessed methodological quality and extracted data from relevant studies. Results: Individual Estimates combined in a meta-analysis resulted in a statistically significant difference of HPV among cases compared to controls, most likely among cases (OR 4.26, 95% CI of 2.05 to 8:87, P = 0.004). Statistical analysis suggests substantial heterogeneity (I2) between studies (I2> 50%, P <0.1), which can be explained by the type of control used. The viral type more frequent among cases was between 16 and HPV controls viral types were HPV-6 and HPV-16 and -18. Conclusions: The results of this meta-analysis support the hypothesis of the involvement of HPV in laryngeal cancer, suggesting that HPV is a risk factor of differentiation depends on the other factors and the method of identification of viral DNA.Item Acesso aberto (Open Access) Perfil das mulheres ribeirinhas com infecção pelo Papilomavírus humano, com ênfase nos tipos 16 e 18, em populações da Amazônia oriental(Universidade Federal do Pará, 2015) DUARTE, Daniel Valim; SOUSA, Maisa Silva de; http://lattes.cnpq.br/1775363180781218The human papilomavírus (HPV) is the cause of the most common sexually transmitted infection in the world and is recognized as the main causative agent of cervical cancer, but little is known about the prevalence of this virus in isolated communities in the Amazon. We aimed in this study to identify among women in riverside communities of several municipalities in the state of Pará - Brazil, the general and specific prevalence of HPV infection and HPV types 16 and 18, building an epidemiological profile of women positive for the virus and these types. Between February and December 2008, cervical samples riverside women were collected on active serach for the Pap smear testing. In these samples, molecular analysis of HPV were performed by conventional polymerase chain reaction (PCR) followed by real-time PCR to diagnose types 16 and 18. Of the 353 samples tested, 58 (16.4%) were positive for HPV , of these 8 (2,3%) were identified positive for type 16 and 5 (1,4 %) were positive for type 18 with one case of infection by both types investigated . Women under 16 years had 34.6 % prevalence had rate and women between 52 and 66 years rate of 29.8 % (p=0.003), women who reported having only one fixed sexual partner had lower prevalence of viral infection (11,8%) (p<0,001). There was a relative increase in viral presence in accordance with the severity cytological shown (p = 0.026). There was no statistical association between viral types 16 and 18 with the variables investigated. The presence of HPV and its most associated with cervical cancer types highlights the importance of specific actions aimed at the prevention, transmission and screening of lesions related to this virus in the river communities of the target municipalities.Item Acesso aberto (Open Access) Prevalência da infecção pelo vírus linfotrópico de células T humanas em mulheres de comunidades remanescentes de quilombos de Santarém, Pará(Universidade Federal do Pará, 2013) CARDOSO, Fernanda Jacqueline Teixeira; SOUSA, Maisa Silva de; http://lattes.cnpq.br/1775363180781218The Lymphotropic Virus Human T-Cell is a oncoretrovirus associated with hematological, neurological, ophthalmological and dermatological pathologies. Currently, other clinical manifestations are described that are possibly associated with direct and indirect effects, immune-mediated, the action of the virus on the body.The main pathologies are described Leukemia / Lymphoma, Adult T Cells (ATLL), Tropical Spastic paraparesis / HTLV Associated Myelopathy (HAM / TSP), infective dermatitis (ID) and Uveitis Associated with HTLV. The transmission of the virus can occur by vertical form, by utero, at birth, or, in most cases through breastfeeding. It can also be transmitted parenterally through, blood transfusion or sharing of contaminated needles and syringes and by sexual contact. In Brazil and the world, the literature has shown that the virus infects mainly women and African descent over the age of 50 years. The aim of this study was to investigate HTLV infection in women remaining quilombos from Santarém, western Pará State. The research was conducted in representative cross demand of women in family groups of ten “quilombola” communities, from October 19 the December 17, 2012. Blood samples were tested for anti-HTLV antibodies using the enzyme immunoassay - ELISA and Polymerase Chain Reaction - PCR was used to identify the viral genotype. genotype. The seroprevalence of HTLV infection was 0,46% (01/216) HTLV-1 was confirmed in a woman of 52 years who has thirteen years dermatological manifestations ith histological diagnosis of erythema multiforme or polymorph. The university students are part of a differentiated demand of the population for their high level of education. Objective was to assess the prevalence and variability of the genotypes of C. trachomatis in cervical infection of public university students in the state of Pará, Brazil, and to evaluate the association of the respective socio-behavioral characteristics and gynecological complaints. They included 438 college students from September 2012 to October 2014 and cervical samples were obtained during gynecological examination. It was performed phenol-chloroform technique for extracting total DNA from the sample cervical secretion, and for the detection of C. trachomatis used the Polymerase Chain Reaction (PCR) PCR-seminested the type for amplification 224pb gene omp1. For the identification of genotypes, we carried out a nested PCR for the amplification of the gene omp1 990pb, which was purified and subjected to ABI3130 sequencer, then the nucleotide sequences were compared with GenBank. The prevalence of cervical infection by C. trachomatis was 12.5% (CI: 95% ± 5.89) and genotypes were identified J genotype (36.3%), followed by D genotypes (18.2%) E (18.2%), F (18.2%) and la (9.1%). There was no significant association for age, first sexual intercourse, number of partners, and condoms are used, the presence of gynecological complaints and genotypes found in the study population. Although the prevalence found present itself high among university students, the lack of statistical significance may be due to small sample size and/or result in socially acceptable answers. The vertical transmission has fundamental role on HTLV silent dispersion and the implementation of policies needs epidemiological indicators to develop prevention campaigns of infection transmission. Objective: To investigate the infection by the human T lymphotropic virus (HTLV-1 and HTLV-2) with parturients attended on public maternity of Belém, Pará, Brazil. Materials and Methods: A representative sample of parturients was investigated on period of March to August 2013, to detection of antibodies anti-HTLV by imunoenzimatic method (ELISA) and for research proviral genome by polymerase chain reaction. Results: The seroprevalence of 0.2% (1/452) identified one infected parturient who reported to be intravenous drug user and to have multiple sexual partners, without genotype identification. The postpartum carrier of HTLV did not breastfeed her newborn son, who showed no anti-HTLV antibodies one year after birth, like her mother and her older daughters, excluding the vertical familial transmission. Conclusion: This study identified a low but present prevalence of HTLV in pregnant women attended at a public maternity, as has been identified in other units of primary and secondary health care throughout Brazil, demonstrating the need for control measures of this infection in all levels of public health attention.Item Acesso aberto (Open Access) Soroprevalência da infecção pelo vírus linfotrópico de células T humanas em portadores de doenças linfoproliferativas atendidos no Hospital Ophir Loyola, Belém, Pará(Universidade Federal do Pará, 2012-05-25) BARBOSA, Sheyla Fernanda da Costa; SOUSA, Maisa Silva de; http://lattes.cnpq.br/1775363180781218About 20% of the worldwide incidence of cancer in humans is attributed to infection by viruses called oncovirus, among them stands out Lymphotropic Virus Human T type 1 (HTLV-1). This virus is associated with several pathologies, among the most studied are paraparesis Tropical Spastic Associated Myelopathy HTLV (HAM / TSP) and the Leukemia Lymphoma, Adult T Cells (ATLL). The HTLV-1 is endemic in many regions of the world, with the highest concentration of cases in southern Japan, while HTLV-2 has been found in greater frequency among intravenous drug users in the United States and Europe, and among native populations of Americas. In Brazil, HTLV 1 is distributed throughout the national territory. The objective of this research was to determine seroprevalence for T-Lymphotropic Virus Human Lymphoproliferative Disease in patients treated at Ophir Loyola Hospital, Belém, Pará The study population consisted of 364 patients diagnosed with lymphoproliferative disease (Hodgkin's lymphoma, Hodgkin lymphoma and non Leukemias Acute and Chronic Lymphoid), treated between January 2005 and December 2011. We used the ELISA test for serological examination and a case was confirmed by PCR. The seroprevalence of HTLV infection in this population was 3.17%, the molecular study of a case showed that it was of HTLV type 1. All serum-reactive individuals were diagnosed as non-Hodgkin lymphoma T cells (p = 0.0021). Among these, the initial manifestation of the disease occurred after the age of 30, the average age was 43.75 years and the majority were women. It is concluded that the prevalence of HTLV in the group studied is consistent with studies of HTLV between leukemias and lymphomas.Item Acesso aberto (Open Access) Transmissão intrafamiliar do Vírus linfotrópico de Células T Humanas tipo 1 em Belém, Pará, Brasil(Universidade Federal do Pará, 2012) ALMEIDA, Danilo de Souza; SOUSA, Maisa Silva de; http://lattes.cnpq.br/1775363180781218Retroviruses belong to a group of infectious agents that have received more attention from the scientific community since the early 19706, when cellular oncogenes have been reported related to them. From these preliminary studies it was described the first human retrovirus, virus Human T lymphotropic 1 (HTLV-1)20. Besides the leukemia/lymphoma adult T cell, HTLV-1 is also causing a degenerative neurological disease associated myelopathy tropical spastic HTLV-1/paraparesia (HAM / TSP) 7, 21. Blood samples from patients and their families living with the virus, seen in the NMT/UFPA were analyzed by molecular method, aiming to investigate the intrafamilial transmission of HTLV-1. Intrafamilial transmission was observed with 100% similarity between the nucleotide bases in nine (75%) of the 12 families studied, of which two families showed vertical transmission, sexual transmission had three families and three families who had both types of transmission. It was observed that three families change occurred in more than one nucleotide base pair, and in two families diverged by 0.2% and 0.4%, ie, had high similarity between their nucleotides (> 99.4 %), suggesting that in this case there intrafamilial transmission of HTLV-1. In one family there was a difference of 1.83%, indicating no intrafamilial transmission. The nucleotide sequence of the 5 'LTR segment and phylogenetic analysis showed high similarity entres the strains isolated in this study and confirmed the occurrence of the Cosmopolitan subtype Transcontinental subgroup. This study provides molecular evidence of intrafamilial transmission of HTLV-1 in patients treated at the Tropical Medicine Center UFPA. In this study the routes of transmission as breastfeeding and sex, was more prevalent for this spread.
