    Saúde suplementar: um estudo da ampliação dos planos e seguros de saúde na Região Metropolitana de Belém

    This study is a discussion about the relationship between health and reproduction of the capitalist mode of production, focusing on supplemental health (health insurance plans).Using the Historical Materialism as theoretical and methodological framework, we analyzed the expansion of health insurance plans on the Metropolitan Region of Belém (MRB) – Pará State, Brazil - at the 2000s. To start this analysis, we present the characteristics of the new international Division of Labor since the 1970s crisis and its impact and developments in the Brazilian social-economic formation. This study analyzes the constant and chronic crisis of contemporary capitalism, and demonstrates how an understanding about the excess of capital (arising from the 1974 crisis) associated to changes in international division of labor in the Brazilian economy are key events to understanding the expansion and the new profile assumed by the supplemental sector of health services in Brazil and in MRB. For this eason, the study revives and introduces the concepts of State, Imperialism, capitalism crisis, overcapitalization, among others. It also exposes the development of health services in Brazil using a historical perspective, showing the changes in the provision of health services, public or private, always from the perspective of the relationship between the reproduction of capital in the Brazilian social-economic formation and the analysis of reproduction of capital in the health services sector, highlighting the emergence and expansion plans and health insurance. With these assumptions, we present the core features of the health insurance in the MRB from four dimensions: the users, the specific conformation of health insurance plans, the plan operators and the distribution of workforce in the sector. And also highlights the main determinations, both general and specific, of the expansion and some likely trends for development of this market in MRB.
