The objective of this work was to study the incidence of accidents involving centipedes at "Centro de Informações Toxicológicas de Belém" (CIT-Belém) over a two-year period. Seventy-six patients were studied from March 30, 1998 until March 30, 2000. Centipede accidents occurred in 16.8% of all accidents by venomous animals at Centro de Informações Toxicológicas de Belém; compared to snake accidents, 44.4 % and scorpion accidents, 20.5%. The majority of cases occurred in the residence (86.8%). The most important age group was 20-49 years old (64.4%). The part of human body mostly affected was the superior members (47.4 %). Local pain and edema were found in 95.8% and 52.1% of the patients, respectively. Treatment was symptomatic. Healing occurred in 94.7%, although the outcome of 5.3% of cases was unknown. They also become brackish waters in the other period what makes possible to find stingrays of the Potamotrygonidae family and other seawater species. These stingrays have retrossered and sharp stings that once got into the victims they injure the skin which recovers the glandurar cells with venon, exposing the tissues to the toxin, leading them to clinical manifestations. It has been used an observational and descriptive model of prevalence in order to estimate the clinical and epidemiological aspects of 116 stingrays accidents occurend in the Mosqueiro and Outeiro beaches, around Belem, Para, from July 1999 to August 2001. These accidents happened specially in the Mosqueiro beaches (80,2%) with more frequency in the male sex (63,8%) while in the teenager and adult groups it occurs (38,8% and 50,0%) respectively, and during the month of July (37,9%). In all tides there were victims but with less frequency in the high tide (4,3%). The most accidents ocurred in the bottom of a river with sand (49,1 %) and during the afternoon period (62,1 %). The major part of the stingrays caught in the Mosqueiro beaches belongs to the Potamotrygonidae family. The victims helped in the first hour were 79,3% of the cases being the lowest members the part the most affected in 84,5%specially the feet in 75,0%. All the patients appeared with local clinical manifestations, the pain with 99,1% and the cianosis with 42,2% of the cases. The systemic symptoms were present in 14,6% being the dizziness (6,0%) and the sudoresis (5,2%) the most ones found. The blood pressure above the normal was present in 6,0% and the taquicardiac was in 9,5%. The debridment has been the cirurgic behaviour the most used (94,5). Only 18,1% came back to a revaluation whose 33,4% of them evolved to necrosis 28,6% with local infection. There has been no statistic signifecance in the correlationamong the clinical evolution, the treatments used and the time between the accidents and the beginning of the medical service. We concluded that the stingray accidents are an important health injury for the population who despite the local complications and difficulties, the victims don't display a serious healthy state. The brackish waters period seems to have a great influence to increase the number of these accidents.Artigo de Periódico Acesso aberto (Open Access) Aspectos epidemiológicos e clínicos do escorpionismo na região de Santarém, Estado do Pará, Brasil(2003-06) PARDAL, Pedro Pereira de Oliveira; CASTRO, Lívia Correa; JENNINGS, Erik; PARDAL, Joseana Silva de Oliveira; MONTEIRO, Maria Rita de Cássia CostaThis is a descriptive and prospective study on epidemiological and clinical aspects of 72 scorpion accidents admitted to Santarém Municipal Hospital, state of Pará, Brazil, from February 2000 to February 2001. Only 8.3% brought the animal with them, identified as T. cambridgei. The majority of victims were male (83.3%). The mean age and the time of the medical help were respectively 33.6±18.3 years and 4.6±3.2 hours. The parts of the body most affected were the superior members (51.5%). Local symptoms occurred in 91.7% cases and systemic manifestations in 98.6% of the accidents. The local symptoms included: paresthesia in 79.2% cases, pain in 52.8% and edema in 26.4%. Among the systemic manifestations neurological disorders predominated in 97.2%, and the symptom of "electric shock" occurred in 88.9% patients. The most common neurological signs were: myoclonia (93%), dysmetria (86.1%), dysarthria (80.6%), and ataxia (70.8%). The accidents were classified as moderate in 76.4% without any serious cases. The specific anti-venom serum was not administered in 32.7% of the moderate cases, due to non-availability of the anti-venom serum at the time of attendance. The victims of scorpion envenomation notified at Santarém, present a different clinical and regional behavior from previous reports in Brazil and Amazonia regions. The predominantly neurological picture has not previously been described in the Brazilian literature.Tese Acesso aberto (Open Access) Contribuição ao conhecimento do escorpionismo e do escorpião Tityus obscurus Gervais, 1843 (Scorpiones: Buthidae), de duas regiões distintas no Estado do Pará na Amazônia brasileira(Universidade Federal do Pará, 2014-03-21) PARDAL, Pedro Pereira de Oliveira; VIEIRA, José Luiz Fernandes; ISHIKAWA, Edna Aoba YassuiIntroduction: Tityus obscurus Gervais, 1843 (Scorpiones: Buthidae) is a senior synonymous of T. paraensis Kraepelin, 1896 and T. cambridgei Pocock, 1897. It is widely distributed in the Brazilian Amazon basin and is considered the most important species responsible for scorpion stings. There were several differences in the clinical manifestations of envenomation by this species. Objective: To contribute for the knowledge of scorpionism by T. obscurus in Brazilian Amazon. Methods: We describe and compare the clinical and epidemiological findings of envenomation by T. obscurus in eastern and west areas of the state of Para. The composition of the venom, morphometric measures and DNA analysis of specimens captured were also compared between study areas. Results: From Jan 2008 to Jul 2010 were included 48 patients with confirmed envenomation by T. obscurus. Most of them came from the eastern region (70.8%). Median age groups were similar in both areas. Most scorpion stings took place during the day and occurred significantly on the upper limbs. The time between the sting and admission to the health center was less than three hours in both areas. Most eastern patients had local manifestations while in the west, systemic manifestations predominated. Local symptoms were similar in both areas, but systemic signs and symptoms were more common in the west. Symptoms frequently observed at the sting site were local and radiating pain, paresthesia, edema, erythema, sweating, piloerection and burning. The systemic manifestations were higher in patients from the west. Furthermore, neurological symptoms, such as general paresthesia, ataxia, dysarthria, myoclonus, dysmetria, and electric shock-like sensations throughout the body were reported only by patients from the west. Mitochondrial gene 16S rRNA showed a polymorphism of 9.06% with divergence from 9.7 to 11% forming two distinct at Neijgbor-Joining and Máxima Parcimônia. There were 18 and 20 specimens captured in west and in eastern regions, measuring between 62.15 to 85.24mm. The largest specimens were captured in the west. Multivariate analysis showed a significant difference between specimens from both sites. The analysis of venom by mass spectrometry and high performance liquid chromatography showed significant differences in both the composition and concentrations of venom components. The venom of specimens captured in west showed a high concentration of compounds with affinity for K+ and Na+ channels. Conclusion: There are a regional diversity in several aspects of envenomation by T. obscurus Gervais, 1843, in the state of Para. The geographical separation promotes, probably, a speciation of specimens, forming two distinct lineages, with 16S rRNA divergences and different length, chemical composition of venom and clinical manifestations of envenomations. Em nove pacientes a variedade do agente etiológico foi estudada e identificada como Cryptococcus neoformans var. gattii. A média de idade destes pacients foi 7,8 anos (variação, 5-13 anos). Havia 5 meninas e 4 meninos (razão, 1,25:1). Cinco foram a óbito em três meses apesar do tratamento com anfotericina B (associada com fluconazol 3 ou fluocitosina 1). Here, we report a case of beetle-caused dermatitis that occurred in Pará State, the Brazilian Amazon. A female patient while putting on her shoes, felt a burning pain in the right hallux and noticed a beetle inside the shoe. During medical consultation, the physician observed that the injury site was brown and discolored. The patient complained of localized burning pain and blurred vision. The beetle responsible for the injury was identifi ed as Pheropsophus sp. Administered treatment included cleaning with saline, analgesics, and topical hydrocortisone acetate and the patient was discharged without complaints.Artigo de Periódico Acesso aberto (Open Access) Paracoccidioidomicose em crianças em Belém do Pará(1999-02) FONSECA, Eliane Regine de Souza da; PARDAL, Pedro Pereira de Oliveira; SEVERO, Luiz CarlosDuring the period from January/85 to July/96, 102 cases of paracoccidioidomycosis were observed in a hospital in Belém, PA. Thirteen of these cases were children between 3 and 13 years of age, with a predominance of females (8:5). All patients presented the disseminated subacute form of the mycosis. The high prevalence of the mycosis in children in endemic areas of the Amazon region is discussed.
