Tese Acesso aberto (Open Access) Efetividade do uso de protocolos de reabilitação sensorial e motora em pacientes hansenianos com dano neural nos pés(Universidade Federal do Pará, 2014) SILVA, Rodrigo Luis Ferreira da; XAVIER, Marília Brasil; http://lattes.cnpq.br/0548879430701901This research aimed to evaluate effectiveness superficial sensory stimulation and deep sensory stimulation in the recovery of leprosy patients with neural damage in the feet. Thirty six patients categorized into three groups of equal size underwent 30 physical therapy interventions. The control group (CG) conducted activities recommended by the Brazilian Department of Health, while other two groups received experimental treatment. The Sensory Stimulation Surface Group (SSSG) was characterized by application of different tactile and thermal stimulus in the plantar region. The Deep Sensory Stimulation Group (DSSG) held proprioceptive exercises. The groups were subject three evaluations (initial and every 15 treatment sessions). In this evaluations were recorded the plantar sensitivity, the plantar arch index and the strength of some foot movements (dorsiflexion, eversion, plantar flexion and toes flexion) measured by charge cell. Concerning to plantar sensitivity the three protocols had significantly higher levels than in the initial time, despite the unfavorable performance in the early 15 sessions for the SSSG. As to plantar arch, the DSSG showed noticeable trend to elevation, from the early 15 sessions, surpassing the other both groups at all step of the research, although this superiority has not obtained statistical significance. As for the evolution of muscle strength, the dorsiflexion showed a significant evolution after 30 sessions done, just to CG. About eversion movement, there was a significant strength gain after 30 physical therapy interventions, just to GC over again. The DSSG showed significant improvement of the middle strength between the first and the second evaluation, just. At the end of the 30 interventions neither group demonstrated significant superiority over other. The toes flexion showed better results for the DSSG, which was the only group to present strength evolution significant after 30 sessions of care, despite the small decrease in the second phase. At the end of this investigation it was found that the SSSG showed to be less effective for recovery plantar sensitivity, during the first 15 interventions, offsetting this bad result after persistence of its application for a further 15 sessions, when then presented the best results to plantar sensitivity evolution. On the other hand, the DSSG showed to have influence upon plantar sensitivity gain, as well as about increasing the plantar arch. Methods: this is an assessment of health programs, focusing on leprosy, targeting managers of the leprosy program and managers of health facilities of primary care. Two different questionnaires, developed specifically for this purpose, were used. Conclusion: the municipality in question has a structure classified between poor and regular, showing several weaknesses in the assessed program for the care of leprosy.
