    The interest of this study is, in general, to identify how the privatized model influenced the actions of public health policy in Brazil, also how it gave the impacts of macroeconomic policy in this regard. One of the key points to be checked revolves around the inequality of the population access to health services, the non-occurrence of universality, generating a process called "excluding universality". This process consists of the migration of the SUS users for operators of private health insurance that contributes to the change of the rationality of health as a right for the rationality of efficiency, bourgeois rationality. The Brazilian Sanitary Reform was a landmark of the struggle of social movements for democratization in the country and a starting point the recognition of health as a right and duty of the State; because of that, we sought a historical survey of this movement. It also has reference to the assumption of the minimization of state action in dealing with social policies and direct interference of international financial institutions on how the State makes health policy, such as the World Bank. This study consists of a qualitative research, with theoretical nature, with the goal of providing subsidies to the discussion of health policy in Brazil, as well as promote and expand the theoretical debate about the role the State plays in thinking and executes this policy.
