    Efeitos de instrução e de treino parental em cuidadores de crianças com câncer

    This research aimed to examine the effects of parental training and instruction on observed behaviors in caregivers and children diagnosed with cancer during the venipuncture procedure on an outpatient basis. Nine caretakers participated in three different conditions (Routine, Manual and Training). Family characteristics, parental style, and effects of manual and parental training were analyzed with direct observation of behavior during the venipuncture procedure of children. After parental training, the results showed an increase in the positive effect of the manual for short term behavior change, and an increase in the positive monitoring rates of the caregiver, with reports of generalizations of these behaviors for other contexts. The importance of parenting style as a protective factor for children with cancer was discussed.

    Obesity is a chronic illness defined as the accumulation of abnormal or excessive fat that can cause harm to people’s health. It has become necessary to fight it and intervene in favour of its prevention, especially among children. The literature in this field indicates that, in general, interventions only with children or caregivers, or both, show favorable results. This study assessed the effects of instructions, verbal report training (VRT) and self-monitoring training (ST), applied with and without the participation of the primary caregiver, on following nutritional rules in obese or overweight children. The participants were two children (9 and 11 years old) and their primary caregivers. The setting was an outpatient clinic in the Psychology department of a university hospital. The patients’ medical records, an initial interview form, the Parenting Style Inventory (PSI), a second interview form, dietary recall, an informative manual on obesity and healthy eating habits, a knowledge test, a nutritional counseling protocol for children, a self-monitoring protocol, a guide for analysis of the self-monitoring protocol, toys and games and a final interview form were used. The collection procedure occurred in 10 sessions distributed in approximately 15 weeks and consisted of medical records analysis; interview at the outpatient clinic with the caregiver and the child for signing the Free and Informed Consent Form, application of the initial interview form and the PSI; participants’ inclusion in one of two conditions (Condition 1, only the child present [P1]; Condition 2, both the child [P2] and his/her primary caregiver [C2] were present); application of the second interview form followed by the dietary recall use (Baseline 1 [BL1]); application of the informative manual on obesity and healthy eating habits and the knowledge test; VRT (Baseline 2 [BL2]); ST; follow-up interview and final interview. As for the effects of instructions, the results indicate that P1 maintained the same classification on all items, while P2 improved his/her performance in the items knowledge and nutritional orientation follow and C2 showed improvement in the knowledge about obesity at the end of the study. Adherence to diet scores obtained by P2 were higher than those obtained by P1 in all phases of the study. Comparing the mean obtained by both participants in BL1 and BL2, an increase of 39.77 % was observed indicating clinically significant change after intervention. The combination of variables in this study was favorable on the participants’ repertoire extension in relation to eating behavior, having the subject himself as reference throughout the study. The results suggest that there is higher effectiveness when children and caregivers are the target of intervention.

    Efeitos de videomodelação instrucional sobre o desempenho de cuidadores na aplicação de programas de ensino a crianças diagnosticadas com autismo

    Caregiver teaching has been a promising methodology to face the arising demand of individuals diagnosed with developmental delays, especially those with Autism Spectrum Disorders (ASD). This study aimed to develop and evaluate the effect of a package of instructional videomodeling on the performance accuracy of caregivers in implementing teaching programs to children diagnosed with ASD. The videos used had, as essential elements: exhibition of multiple exemplars, subtitles, narrations, and visual cues throughout the video. A multiple-baseline design across participants was used to measure the effect of the videomodeling procedure on the performance accuracy of the caregivers. Three parents of children diagnosed with TEA participated: Alice, Cassio, and Fernanda, who presented, respectively, 6%, 25%, and 45% average accuracy throughout baseline measures. After videomodeling procedure, performance accuracy reached 83%, 88%, and 80% average accuracy. Considering such results, we believe that the procedure of instructional videomodeling is promising to disseminate behavioranalytic intervention, in a less resource demanding way. Subsequent research should improve the intervention package here described, in order to obtain better results and provide a tool to disseminate behavior analytic intervention to wider portions of the affected population.

    Estresse parental de cuidadores de crianças com paralisia cerebral no estado do Pará

    Children with Cerebral Palsy (CP) require care that can be lifelong and, for this reason, it is of scientific and social interest to investigate aspects related to their primary caregivers. Thus the aim of this doctoral thesis has been to investigate indicators of parental stress among caregivers of children with CP living in urban centres of the meso-Regions of the State of Pará, and to verify if these are associated with caregivers’ sociodemographic profile and their perceptions of social support. The study covered a total of 132 caregivers of children aged one to 12 years old, who used free philanthropic facilities in the cities of Belém, Castanhal, Marabá and Santarém. Caregivers were interviewed individually and responded to the following four data-collection instruments: the Sociodemographic Inventory, the Medical Outcomes Study Social Support Survey (MOS-SSS), the Gross Motor Function Classification System (GMFCS) and the Parental Stress Index (PSI/SF). The data were analysed using descriptive statistics with particular emphasis being given to central tendency measures, and the Correspondence Analysis technique was also employed. The results showed that in all cities most caregivers were mothers living in a stable union, with low family income and little schooling. In addition, the data suggested that there existed physical barriers in the built environment in proximity to the caregivers’ homes. The mean values of the MOS-SSS score ranged from 72 and 79.3, indicating that the sample’s caregivers had a high level of perception of regarding the extent of social support. However, a significant proportion of caregivers (30%) said that they rarely or never benefited from the presence of someone able to help them relax. Regarding parental stress, almost half (46%) of the sample experienced high levels. The results of the correspondence analysis showed that caregiver profiles varied according to their levels of stress and the classification of the city in which they resided. Among the variables that were shown to be associated with parental stress in at least three different cities are: schooling, age and the income of the caregiver’s family, as well as the extent of material and emotional support. The knowledge acquired by the research reported on in this thesis concerning the interrelationships between parental stress, caregivers’ perception of social support, and the sociodemographic characteristics of the caregivers of children with CP in the State of Pará, contributes to a better understanding of the impacts of this clinical condition on caregivers who live in these specific urban context.

    Parental Stress and Social Support of Caregivers of Children With Cerebral Palsy

    Stress and social support are relevant variables for understanding the impact of disability on the care relationship. Thus, this study investigates the association between the parental stress index, social support indicators, and the sociodemographic variables of caregivers of children with cerebral palsy in a capital city of the Eastern Amazon. The following instruments were applied to 100 caregivers: the Sociodemographic Inventory, the Gross Motor Function Classification System, the Parenting Stress Index, and the Medical Outcomes Study Social Support Survey. For data analysis, descriptive statistics were used, in addition to techniques of multivariate analysis. It was found that most participants had high parental stress and a high perception of social support. Specific aspects of the perception of social support and sociodemographic indicators were associated with stress. This knowledge favors the design of more assertive interventions because it outlines the aspects of these variables that appear to have a more effective impact on parental stress.

    This study assessed the effects of instructions, verbal report training (VRT) and self-monitoring training (ST), with and without the presence of the primary caregiver, on following nutritional rules in obese or overweight children. Two children (P1: a nine-year-old boy; P2: a eleven-year-old girl) and their caregivers participated through interviews performed in an outpatient clinic. Ten interviews were performed. After instructions, P1 maintained and P2 improved the knowledge on nutritional orientation. Adherence to diet scores obtained by P2 were higher than those obtained by P1 in all phases. There was a clinically significant change after the introduction of the VRT in both participants. Efficiency of the intervention procedures used and the importance of the caregiver’s presence are discussed.
