Tese Acesso aberto (Open Access) Adaptação da Escala de Ansiedade de Beck para avaliação de surdos e cegos(Universidade Federal do Pará, 2013-09-05) SANCHEZ, Cintia Nazare Madeira; GOUVEIA JUNIOR, Amauri; http://lattes.cnpq.br/1417327467050274This study met three articles concerning the adaptation of psychological instruments to evaluate the population of deaf and blind. Considering that these instruments are not adapted to evaluate people with special educational needs, complicating diagnosis and prognosis. Therefore the need to adapt these instruments for this population is indisputable. This situation also occurs in the area of deafness and blindness, in which there is a shortage of jobs in Brazil. In the first article was conducted a literature review of instruments adapted for this population. It is concluded that the area of hearing assessment scales are adapted to various factors as psychometric for measuring depression, anxiety and intelligence, but the area of blindness instruments are suited for the assessment of cognitive functioning. The aim of the second study was to adapt the Beck Anxiety Scale (BAI) to sign language and alphabet digital generating a scale to assess anxiety in deaf users of Brazilian Sign Language (LIBRAS). The sample consisted of 25 deaf users LIBRAS (experimental group) and 25 listeners (control group), aged between 18 and 25 years of age of both sexes, matched for age and sex. The application was made in a group. Following the guidelines, the subjects completed the scale, the scale pattern control group and the experimental group the adapted scale. The results of the study showed that the BAI adapted not statistically significant compared to the standard scale and total anxiety subscales: subjective, neurophysiological, autonomic, and panic. Therefore BAI adapted showed validity equivalent to BAI standard to assess anxiety in deaf, the items appear to have adapted the modified factor structure of the instrument, thus allowing its use in the assessment of anxiety in deaf users of LIBRAS. The third study was conducted with visually impaired, this deficiency is most prevalent in the population reaching 35.8 million people with difficulty seeing even with corrective lenses, and 506,3 thousand are blind. Despite the significant number of blind in the literature there are few studies of adapting assessment tools for the blind, as they are evaluated on the parameters of the seers. Given this reality, the objective of this study was to adapt the Beck Anxiety Scale (BAI) for Braille generating a scale to assess anxiety in blind Braille users. The sample consisted of 25 blind Braille users (experimental group) and 25 seers (control group), aged between 18 and 25 years of age of both sexes, matched for age and sex. The application was made in a group. Following the guidelines, the subjects completed the scale, the scale pattern control group and the experimental group the adapted scale. The results of the study showed that the BAI adapted not statistically significant compared to the standard scale and total anxiety subscales: subjective, neurophysiological, autonomic. Subscale panic in this difference was statistically significant at the limit. Among the diseases that cause dementia, AD is the most frequent nature of the vascular a ratio of 4:1, respectively. In addition to the pharmacological therapies, diagnostic methods assist in the early identification of the disease by helping the pretreatment, thus reduced disease progression. Currently cytogenetic studies have demonstrated chromosomal abnormalities in individuals with AD and may aid in the diagnosis of disease. The aim of this study was to investigate the potential of karyotype analysis of peripheral blood lymphocytes as a diagnostic biomarker of Alzheimer's disease. For this work, we used two groups of women aged 65 or more, one group (10) suffering from AD and other normal group (10). Each subject was submitted to the socioeconomic survey, a cognitive screening test (MMSE) and the Venous blood lymphocyte culture and chromosome analysis. Our results demonstrate that the group of women with AD showed high rate of monosomy and trisomy compared to normal women. Through the study of history via questionnaire, we found the lifestyle of both groups. Compared the relationship of chromosomal abnormalities with the cognitive level of the AD group, we evidenced an inverse trend between the number of monosomy / trisomy and cognitive performance. Another aspect of our analysis was the role of each chromosome linked to AD. Chromosomes 1, 14 and 21 showed no trisomy and verify the frequency of monosomy, each chromosome has frequency below 3 % of aneuploidy, i.e., the chromosomes studied did not have a great importance in chromosomal alterations found in the study.Artigo de Periódico Acesso aberto (Open Access) Análise crítica dos achados hematológicos e sorológicos de pacientes com suspeita de Dengue(2008-10) BARROS, Lilian Patrícia Souza; IGAWA, Sônia E. S.; JOCUNDO, Susana Y.; BRITO JUNIOR, Lacy Cardoso deThe increase in the number of more serious forms of dengue fever among cases in the city of Belém, Brazil has astounded local authorities. The objective of this study was to conduct a critical analysis of hematologic and serologic findings of patients with clinical suspicion of dengue seen at a clinical laboratory in Belém in Pará State. This retrospective study involved 210 patients who were referred to the Laboratory of Clinical Pathology Dr. Paulo C. Azevedo in Belém, in the period of February and March of 2007, with requests for a complete blood count and serological test for IgM to confirm the diagnosis of dengue. Of the cases studied, 51/210 (24.3%) presented with thrombocytopenia and 53/210 (25.2%) leukopenia. The serologic test for IgM was positive in 47.1% (99/210) of cases. A statistically significant association (p<0.05) was observed only among patients who presented with thrombocytopenia (33/99) and positive serology for dengue, suggesting that the hematological alterations of leukopenia and thrombocytopenia, frequently associated with this illness, may not be present at the beginning of the infection. This dissertation presents an assessment of the implementation of a predictive maintenance system of electrical machines (SIMME) on Balbina hydroelectric power plant in Amazonas, Brazil. The system was elaborated to predict defects, failures or bad operational states of machines of the hydroelectric plant and suggest maintenance intervention and operational emergency actions. Due to the absence of laboratory diagnosis of leprosy, new tools that contribute for identification of cases are necessary for enable patient treatment before progression to physical disabilities. In this sense, the present study aimed evaluate serological biomarkers contribution for early diagnosis of leprosy. Was perfomed an active case finding study in Mosqueiro district, Belém – Pará. All individuals were clinically examined by experient leprologists doctors and than 5mL of peripheral blood were colleted for future titration of anti-ND-O-BSA, anti-LID-1 e anti-NDO-LID by ELISA. The action of active finding in Mosqueiro district diagnosed 104 new cases of leprosy between 895 subjects examined (11.6%), indicating a high hidden endemy that agree with the high seroprevalence between schoolchildren. Were observed a significant difference among patients with late or early diagnoses, mainly in multibacillary forms. All biomarkers tested showed promising results in detection of late cases, such as related in literature, however, for early cases those molecules identified correctly only 50% of patients. None of biomarker tested presented sufficient sensitivity to detect all leprosy patients, early or lately diagnosed. Besides, LID-1 molecule had evidenced a lower sensitivity for early cases, their high especificity and accuracy suggest their use as a potential tool for serological screening to identify assintomatic subjects with high risk of illness. Thus, we concludes that besides no biomarker had reveled utility as a serological diagnostic tool, the detection of anti-LID-1 presented a possible aplicability as a screening marker of subjects with increased risk to develop leprosy, contributing indirectly for leprosy diagnosis.Dissertação Acesso aberto (Open Access) Avaliação do nível de concordância do teste imunocromatográfico OptiMAL-IT® e a gota espessa no diagnóstico da malária, no município de Mazagão-AP, Brasil(Universidade Federal do Pará, 2007) FADUL, Danielle Scerne; COUTO, Álvaro Augusto Ribeiro D'Almeida; http://lattes.cnpq.br/3800209721205388The precocious diagnosis and the opportune treatment of the cases of malaria is one of the main strategies for the control of the disease. Several alternatives for the traditional microscopic diagnosis were proposed in the last years, the Immunochromatographic tests that capture white antigens of the parasites of the malaria they are being proposed, as the test OptiMAL-IT® that captures the lactic desidrogenase of the Plasmodium sp.. The study had as objective the evaluation of the level of agreement between the Immunochromatographic test (OptiMAL-IT®) and the thick drop for the diagnosis of the malaria in the City of Mazagão – Amapá, Brazil. 413 individuals were analyzed with malaria sintomatology that had looked for the service of the unit of health service of the city, with age among 01-68 years. The results of the OptiMAL-IT® test were compared with the obtained results, of the same samples, through the thick drop red-faced by the Giemsa. Of the 413 patients suspicious to present malaria, 317(76.8%) were positive through GE and 311 (75.3%) were positive for TDR OptiMAL-IT®. Of the positive blades of GE, had been found 27.4% of P. falciparum and 72.6% of P. vivax . The OptiMAL-IT® test detected 27.7% of P. falciparum and 72.3% of P. vivax. The sensibility obtained with TDR for P. falciparum was of 97.7% and for P. vivax was of 98.2%, the global sensibility of TDR was of 98.1% and the global specificity for both the species was of 100%. They were found preditivos values positive and negative of 100% and 94.1%, respectively. The OptiMAL-IT® test had a high agreement with thick drop, it is specific and efficient. Thus, research into new ways of early diagnosis of neurocognitive disorders associated with diseases such as Alzheimer's and Parkinson's, as well as the improvement of the understanding of currently available clinical neurological diagnostic methods, has been promoted throughout the world. In this sense, this work proposes to investigate the existence of possible correlations between the results of exams used in clinical neurological investigation of patients diagnosed with neurocognitive disorder associated with these important neurodegenerative diseases (Alzheimer's and Parkinson's diseases). The results show that the measurement of the thickness of the nerve fiber layer of the perimacular retina, performed by optical coherence tomography, is a parameter that may not differ significantly between groups of patients and healthy subjects. On the other hand, the measurement of the amplitude of the spectroscopic signals generated by encephalic metabolites, performed by magnetic resonance proton spectroscopy, reveals encephalic changes that vary from region to region. In addition, the neuropsychological measure of cognitive functions, performed by the automated CANTAB battery, reveals that several aspects of these functions are impaired in these patients. Finally, Principal Component Analysis shows that, considering the set of variables obtained by tomographic and neuropsychological measurements, it is possible to observe a correlation between several of these variables. Among the effects of this very common practice today, is the exponential growth of prescription drugs, such as amphetamines to children, causing severe problems in their lives, which should be the subject of an ethical, political, and social problematization. Specifically, the more drug administered in this situation has been methylphenidate and in Brazil, it has been widely sold and used in cases assessed as inattention and restlessness. For this purpose, it is necessary to recover the theoretical framework from the theories of John Maynard Keynes and even Hyman Philip Minsky. The main conclusion is that the social diagnosis, during this period, showed an impressive reduction in income inequality and poverty. In this way, it can be seen that the reduction of income inequalities are extremely important parameters for the expansion of the national domestic market. On the other hand, family income transfer programs, such as “Bolsa Família”, have contributed to the reduction of absolute poverty. Creating mosaics in different ecosystems aims to promote integrated management Foundation and participatory from different categories, being designed as a tool for environmental management, according to the own SNUC. In the Amazon, are recognized by the following: mosaics of MMA Apuí-AM (2010), the lower Rio Negro-AM (2010) and the mosaic of Tucuruí (PA) (2002)-this being the oldest and the focus of this study. The mosaic of Tucuruí is inserted in a specific context of creation, have arisen after popular mobilization of those affected by the construction of the dam of Tucuruí hydro-power complex. However, it is worth mentioning that the recognition of the mosaic (2002) was conducted years after the inauguration of dam (1984), a fact that, without a doubt, generated the escalation of issues related to the use and occupation of the surroundings of the Lake. In addition, the creation of the mosaic comes as part of the public policy of environmental compensation, derived from the construction of the dam. By this context, need a diagnosis about the effectiveness of the creation of the mosaic of Tucuruí, which also falls within the context of environmental compensation. Analyze the effectiveness of the mosaic of Tucuruí implies the assessment of public policy who predicted the consolidation of specific objectives for the territory in question. In this way, the monitoring and evaluation processes are as essential items for viewing of the reach of State policies. Such range materializes in the verification of the quality of management, the efficiency of public spending and the effectiveness of the action of the State as a regulator of public policy. To this end, this study used as reference parameter the "Protocol of evaluation of effectiveness of management of mosaics of protected areas in Brazil", developed by Gidsicki (2013) are defined areas, principles, criteria and indicators of effectiveness of management that make up an array analysis for hierarchical and mosaics that allows making a preliminary diagnosis of the current situation the mosaic with its advances, obstacles and suggestions for effective management.Artigo de Periódico Acesso aberto (Open Access) Diagnóstico de imunofenótipos de síndromes linfoproliferativas crônicas por citometria de fluxo na Fundação HEMOPA(2011-12) BRITO JUNIOR, Lacy Cardoso de; FEIO, Danielle Cristinne Azevedo; BARBOSA, Suane Reis; BENTES, Alessandra Quinto; FRANCÊS, Larissa Tatiane MartinsINTRODUCTION: Lymphoproliferative syndromes comprise a heterogeneous group of malignant neoplasias with different clinical behaviors, pathological factors and epidemiological characteristics, whose diagnosis may be based on lymphoid cell morphology observed in peripheral blood. OBJECTIVE: To test the diagnostic feasibility of immunophenotyping by flow cytometry for lymphoproliferative syndromes through the definition of minimal antibody panel. MATERIAL AND METHODS: During the period of July 2008 to July 2010, 47 patients from HEMOPA blood center participated in this study for differential diagnosis of lymphoproliferative syndromes subtypes by flow cytometry. RESULTS: The mean age was 68 years old. There was no statistical difference between genders, and the most frequent subtype of lymphoproliferative syndromes was chronic lymphoid leukemia/small B-cell lymphocytic lymphoma. CONCLUSION: Based on the antibody panel recommended in this investigation, the immunophenotyping method by flow cytometry associated with morphological characterization of peripheral blood samples is a reliable, rapid, feasible, and non-invasive procedure for the diagnosis of chronic lymphoproliferative syndromes.Tese Acesso aberto (Open Access) Um dizer que fala de vida e morte: revelações e silêncios na comunicação sobre o diagnóstico do câncer(Universidade Federal do Pará, 2008) SALGADO, Ana Cristina Soeiro; MAUÉS, Raymundo Heraldo; http://lattes.cnpq.br/0915136632611666One of the problems faced by physicians in their practice with cancer patients refers to the dilemmas related to communication about the illness, especially because of the associations still present in the social imagination about death and suffering, which gives an important symbolic dimension to diagnosis. Interpreted as a social construct, the theme of cancer diagnosis is analyzed from the perspective of Brazilian physicians, at Ophir Loyola Hospital, a state public cancer hospital in Pará, north of Brazil, including a total of 20 informants, who agreed to take part in interviews focused on diagnosis communication, including disclosure of diagnosis and other information related to the diagnosis. Through a socio-anthropological approach, different variables are analyzed, including those related to the physician, the patient, the family and the illness, which enables comprehension and integration of the discourse in its social context, including some differences between public and private practice. Data suggest the physician-patient interaction is influenced by class condition, with its socio-historical and cultural determinant. This makes communicating diagnosis a complex phenomenon, aggravated by the limited access to health services, which contributes towards the great amount of patients who arrive at the hospital with no chances of cure. The results indicate that influence of the family on medical decision-making is determinant to the limits of information related to cancer diagnosis, including disclosure of diagnosis and prognosis. Hopelessness is frequently mentioned as a reason for non-disclosure, especially in advanced stages of cancer, with a progressive silence as the illness worsens.Artigo de Periódico Acesso aberto (Open Access) Electrocardiogram Performance in the Diagnosis of Left Ventricular Hypertrophy in Hypertensive Patients With Left Bundle Branch Block(Universidade Federal do Pará, 2016-12) BURGOS, Paula Freitas Martins; LUNA FILHO, Bráulio; COSTA, Francisco de Assis; BOMBIG, Maria Teresa Nogueira; SOUZA, Dilma do Socorro Moraes de; BIANCO, Henrique Tria; OLIVEIRA FILHO, Japy Angelini; IZAR, Maria Cristina de Oliveira; FONSECA, Francisco Antonio Helfenstein; PÓVOA, Rui Manuel dos SantosBackground: Left ventricular hypertrophy (LVH) is an important risk factor for cardiovascular events, and its detection usually begins with an electrocardiogram (ECG). Objective: To evaluate the impact of complete left bundle branch block (CLBBB) in hypertensive patients in the diagnostic performance of LVH by ECG. Methods: A total of 2,240 hypertensive patients were studied. All of them were submitted to an ECG and an echocardiogram (ECHO). We evaluated the most frequently used electrocardiographic criteria for LVH diagnosis: Cornell voltage, Cornell voltage product, Sokolow-Lyon voltage, Sokolow-Lyon product, RaVL, RaVL+SV3, RV6 /RV5 ratio, strain pattern, left atrial enlargement, and QT interval. LVH identification pattern was the left ventricular mass index (LVMI) obtained by ECHO in all participants. Results: Mean age was 11.3 years ± 58.7 years, 684 (30.5%) were male and 1,556 (69.5%) were female. In patients without CLBBB, ECG sensitivity to the presence of LVH varied between 7.6 and 40.9%, and specificity varied between 70.2% and 99.2%. In participants with CLBBB, sensitivity to LVH varied between 11.9 and 95.2%, and specificity between 6.6 and 96.6%. Among the criteria with the best performance for LVH with CLBBB, Sokolow-Lyon, for a voltage of≥ 3,0mV, stood out with a sensitivity of 22.2% (CI 95% 15.8 – 30.8) and specificity of 88.3% (CI 95% 77.8 – 94.2). Conclusion: In hypertensive patients with CLBBB, the most often used criteria for the detection of LVH with ECG showed significant decrease in performance with regards to sensitivity and specificity. In this scenario, Sokolow-Lyon criteria with voltage ≥3,0mV presented the best performance. It is genetically characterized by a somatic mutation in the PIG-A gene (phosphatidylinositol glycan anchor biosynthesis, class A), in which the best known antigens are DAF (decay accelerating factor or CD55) and MIRL (membrane inhibitor of reactive lysis or CD59). OBJECTIVE: To determine the frequency of paroxysmal nocturnal hemoglobinuria in patients attended at the HEMOPA foundation from November 2008 to July 2009. METHOD: Thirty patients, with ages ranging from two to 79 years old and suspected of having paroxysmal nocturnal hemoglobinuria were examined. All patients were immunophenotyped by flow cytometry for the CD5, CD59, CD16 and CD45 antigens. RESULTS: Paroxysmal nocturnal hemoglobinuria was identified in nine of the thirty patients investigated. Another 3 cases had inconclusive results with CD59-negative labeling only for neutrophils. The highest frequency of paroxysmal nocturnal hemoglobinuria patients (7/9) and inconclusive cases (2/3) were between 19 years old and 48 years old, with a median of 28 years. Means the environmental management in the city a determining factor in the search for sustainable alternatives to stimulate the economy, since one of the main income-generating activities revolves around an illegal activity in the case, logging of Alto Rio Guamá indigenous land. In this context, this work is a contribution to the study of the development of a methodology for diagnosis of defects to the machine number three hydro Hydropower Plant Coaracy Nunes, located in Amapa State. In many situations the methods of vibration analysis is used to detect the presence of faults in this type of machine, this work will also be used to analyze the current signals to provide similar indications. This work aims to present a methodology for fault diagnosis in electrical machines using the vibration signals and correlation with the analysis of the stator current. Throughout this work presents a review of techniques for monitoring and diagnosis of the condition of electrical machinery, through vibration tests correlated with the characteristics of the stator current. The result of the correlation of vibration measurement with the current measurement is based on a methodology implemented by a system of acquisition and data processing platform developed in LabView. The experimental results were obtained from mechanical defects (unbalance and mechanical defects on the slopes, and internal bearings) induced in an experimental device designed with the intention to represent a generation system. Finally, the vibration and current signals were analyzed and compared to verify that the defects that were detected by the conventional vibration altered the behavior of current signals. Therefore, the Núcleo Brasileiro para Estudo do Helicobacter pylori e Microbiota (NBEHPM), association linked to Brazilian Federation of Gastroenterology (FBG) held its fourth meeting again in Bento Gonçalves, RS, Brazil, on August 25-27, 2017. Twenty-six delegates, including gastroenterologists, endoscopists, and pathologists from the five regions of Brazil as well as one international guest from the United States, participated in the meeting. The participants were invited based on their knowledge and contribution to the study of H. pylori infection. The meeting sought to review different aspects of treatment for infection; establish a correlation between infection, dyspepsia, intestinal microbiota changes, and other disorders with a special emphasis on gastric cancer; and reassess the epidemiological and diagnostic aspects of H. pylori infection. Participants were allocated into four groups as follows: 1) Epidemiology and Diagnosis, 2) Dyspepsia, intestinal microbiota and other afections, 3) Gastric Cancer, and, 4) Treatment. Before the consensus meeting, participants received a topic to be discussed and prepared a document containing a recent literature review and statements that should be discussed and eventually modified during the face-to-face meeting. All statements were evaluated in two rounds of voting. Initially, each participant discussed the document and statements with his group for possible modifications and voting. Subsequently, during a second voting in a plenary session in the presence of all participants, the statements were voted upon and eventually modified. The participants could vote using five alternatives: 1) strongly agree; 2) partially agree; 3) undecided; 4) disagree; and 5) strongly disagree. The adopted consensus index was that 80% of the participants responded that they strongly or partially agreed with each statement.
