This dissertation presents an assessment of the implementation of a predictive maintenance system of electrical machines (SIMME) on Balbina hydroelectric power plant in Amazonas, Brazil. The system was elaborated to predict defects, failures or bad operational states of machines of the hydroelectric plant and suggest maintenance intervention and operational emergency actions. The results show that the SIMME improved the economic performance of the hydropower plant.Item Acesso aberto (Open Access) Desigualdade de renda, desemprego e pobreza nas regiões metropolitanas do Brasil: uma perspectiva heterodoxa(Associação Keynesiana Brasileira, 2019) CARVALHO, André Cutrim; CARVALHO, David Ferreira; RODRIGUES, Phelipe da SilvaThe present paper aims to deal with the theoretical and empirical view of the evolution of income inequality, unemployment and poverty in the Brazilian metropolitan regions from 2001 to 2011. For this purpose, it is necessary to recover the theoretical framework from the theories of John Maynard Keynes and even Hyman Philip Minsky. The main conclusion is that the social diagnosis, during this period, showed an impressive reduction in income inequality and poverty. In this way, it can be seen that the reduction of income inequalities are extremely important parameters for the expansion of the national domestic market. On the other hand, family income transfer programs, such as “Bolsa Família”, have contributed to the reduction of absolute poverty. This was a cross-sectional, observational study including clinical and biochemical data from 72 patients (60 families) from Brazil (South, n = 13; Southeast, n = 37; Northeast, n = 8; North, n = 1; and Midwest, n = 1). Parental consanguinity was reported in 42% of families. Ocular manifestations were the earliest detected symptom (53% of cases), the main reason for diagnostic suspicion (63% of cases), and the most prevalent manifestation at diagnosis (67% of cases). Pyridoxine responsiveness was observed in 14% of patients. Only 22% of nonresponsive patients on treatment had total homocysteine levels <100 mmol/L. Most commonly used treatment strategies were pyridoxine (93% of patients), folic acid (90%), betaine (74%), vitamin B12 (27%), and low-methionine diet + metabolic formula (17%). Most patients diagnosed with HCU in Brazil are late diagnosed, express a severe phenotype, and poor metabolic control. In this context, this work is a contribution to the study of the development of a methodology for diagnosis of defects to the machine number three hydro Hydropower Plant Coaracy Nunes, located in Amapa State. In many situations the methods of vibration analysis is used to detect the presence of faults in this type of machine, this work will also be used to analyze the current signals to provide similar indications. This work aims to present a methodology for fault diagnosis in electrical machines using the vibration signals and correlation with the analysis of the stator current. Throughout this work presents a review of techniques for monitoring and diagnosis of the condition of electrical machinery, through vibration tests correlated with the characteristics of the stator current. The result of the correlation of vibration measurement with the current measurement is based on a methodology implemented by a system of acquisition and data processing platform developed in LabView. The experimental results were obtained from mechanical defects (unbalance and mechanical defects on the slopes, and internal bearings) induced in an experimental device designed with the intention to represent a generation system. Finally, the vibration and current signals were analyzed and compared to verify that the defects that were detected by the conventional vibration altered the behavior of current signals. These tests can be influenced by age, gender, nutrition, breed, species and environmental conditions. Thus, data from one region should not be extrapolated to animals raised in other regions. The objective of this study was to determine the hematological and biochemical values of different ages and genders of Santa Inês sheep, raised in the eastern Amazon. There were examined 91 sheep that were assigned to three groups: G1 (three to six months old, n = 31); G2 (seven to 24 months old, n = 30) and G3 (above 24 months old, n = 30). The blood cell count, white blood count and biochemical determinations were made with an automatic counter and a semi-automatic analyzer, respectively. Mean values were compared using the Tukey test. The number of erythrocytes, the red blood indices, the thrombogram, eosinophils, total protein, urea and creatinine concentrations were influenced by the age of the animals. The erythrocyte coefficient of variation and the creatinine concentration were influenced by gender, and were greater in males. The neutrophil:lymphocyte (N:L) ratio was greater than one for all age groups. In Pará, there is still a high number of cases of gestational syphilis detected in childbirth and puerperium. The factors that relate to the maintenance of this context are unknown. Objective: To determine the predictors of gestational syphilis in a state in the Brazilian Amazon region. Methods: Unpaired case-control study, carried out in a public reference maternity hospital in Pará. Data were collected from November 2020 to July 2021, from the application of the form, analysis of the pregnant woman's card and medical records. Women diagnosed with syphilis during pregnancy, childbirth or puerperium were considered as cases. Controls were women without a diagnosis of syphilis in the pregnancy puerperal cycle. The response variable was the diagnosis or not of gestational syphilis, the event being considered the diagnosis of gestational syphilis in the prenatal, delivery or postpartum period. Independent variables were grouped into: a) sociodemographic; b) gynecological and obstetric by two (02) categorical variables and five (05) discrete variables; c) sexual and obstetric practices by eight (08) categorical variables; d) knowledge is composed of fifteen (15) categorical variables and e) Attitudes is composed of six (06) categorical variables. The sample size, calculated using the Kelsey method, was 204 participants, 68 cases and 136 controls. To test the main hypothesis of the study, multiple logistic regression analysis was performed. In the Bioestat 5.3® program, the chi-square test of independence, tendency and G test were applied. In the Minitab 20® program, descriptive analyzes and logistic regression were performed. Odds ratio, 95% confidence interval was calculated. P values <0.05 were considered statistically significant. Results: Among the 68 cases and 137 controls, age ranged from 15 to 41 years, mean age was 26.1 years (SD: 6.2). In the multiple logistic regression, the cases are independently associated with the lowest number of prenatal consultations (OR: 0.82; p = 0.02), history of previous STI (OR: 24.8; p = 0.00 ), spontaneous abortion (OR: 2.86; p = 0.025) and difficulty in talking with the partner about the use of sexual condoms in relationships (OR: 4.43; p = 0.00). Among the factors of knowledge and attitudes, cases were associated with knowledge about vertical transmission (OD: 0.24; p= 0.019), treatment with penicillin (OD: 0.47; p= 0.023) and on the availability of treatment in basic health units (OD: 0.14; p= 0.014). However, they are associated with the lack of knowledge about the lack of vaccine (OD: 1.94; p= 0.04). The analysis of variables related to attitudes showed an association of cases (OD: 2.40; p= 0.016) with inadequate attitude about the beginning of prenatal care. There is an increase in reactive cases from the 1st trimester to the puerperium with p < 0.0001, with the highest significant proportions of reactive cases during delivery and postpartum. Conclusion: gynecological-obstetric factors, sexual and reproductive practices and knowledge are associated with gestational syphilis. In a population with precarious socioeconomic conditions, no association of these factors was demonstrated. Like diabetes, it is an important cardiovascular risk factor and microvascular complications, such as nephropathy and retinopathy, and has higher health costs than diabetes itself. The treatment of hypertension in diabetes has been widely debated and there is still a need to reach a consensus on treatment goals and strategies. The objective of the present study was to develop a clinical protocol on the diagnosis and treatment of hypertension in diabetes mellitus whose main foundation was its applicability in the Brazilian public health system, focusing on medical adherence to recommendations, the impact of applying the evidence in primary and secondary care, and the evolution of health quality in these scenarios. To this end, an integrative literature review was carried out, selecting health articles based on the quality of evidence located in the PUBMED database and published in the last 20 years (2004-2024). Five researchers were responsible for selecting those with the best quality of evidence. National and international guidelines and works published by the UFPA diabetes clinical research group were also used. In the end, 106 articles were selected as theoretical references for preparing the protocol. The results were synthesized in the form of a clinical protocol containing concise texts and teaching resources (27 tables, 3 flowcharts and 1 figure) that will guarantee safety in clinical decision-making in primary and secondary health care settings, configuring a practical guide on the topic in question.Item Acesso aberto (Open Access) Protocolo de diagnóstico e tratamento da Neuropatia Diabética(Universidade Federal do Pará, 2024-02-09) VIEIRA, Jocyelle Nascimento; FELÍCIO, João Soares; http://lattes.cnpq.br/8482132737976863Diabetic neuropathy (DN) is currently the most frequently observed chronic complication of diabetes mellitus (DM) and, at the same time, the most underdiagnosed, affecting up to half of these patients, depending on the diagnostic methodology used. It generally affects individuals with a long duration of the disease (>10 years), however, it may be present at diagnosis in patients with DM2 or manifest early, within the first five years of the disease, in patients with DM1. Additionally, once diagnosed, DN is usually irreversible, but its appropriate management in patients with early diagnosis increases the possibility of regression and stabilization of the disease, considerably delaying and preventing the onset of complications. The objective of this work was to create a protocol for diagnosing and treating diabetic neuropathy in an objective and didactic manner, with a focus on helping general practitioners and specialists, both in primary and tertiary care. The protocol was prepared using the integrative literature review method, which was divided into 6 phases. The first phase consisted of preparing the guiding question to define the studies to be included and then an integrative review was carried out in the PUBMED and SciElo databases with the terms “NEUROPATHY”, “TREATMENT”, “DIAGNOSIS”, “NEUROPATHY DIABETIC” and its counterparts in Portuguese. Guidelines from the main national and international diabetes scientific societies, one book and relevant information from relevant websites were also included. After data collection and analysis, 106 materials were used as theoretical references for the preparation of this protocol. This resulted in the construction of didactic and objective tables, charts and flowcharts. The clinical applicability of the study consists of providing a tool, not only for the specialist, but for the general practitioner at different levels of health care, with the aim of enabling effective care for individuals with DN. Thus, the main contributions of the protocol were the development of a total of 20 didactic and visual tools aimed at the prevention, tracking, diagnosis and treatment of DN. 5 flowcharts, 4 charts and 2 tables are authored by the author.Item Acesso aberto (Open Access) Protocolo de rastreio e diagnóstico do Diabetes Mellitus Gestacional(Universidade Federal do Pará, 2022-08-31) SOUZA, Larissa Mayane Reis Barros de; SOUZA, Ana Carolina Contente Braga de; http://lattes.cnpq.br/0609863332556837Gestational diabetes mellitus (GDM) is associated with severe adverse perinatal outcomes and unfavorable short- and long-term health consequences for both mother and child. Due to the increasing prevalence of GDM worldwide, the impact and importance of this medical condition in prenatal care is growing. There is no consensus on the best forms of diagnosis for GDM, different criteria are adopted worldwide which currently often leads to a late diagnosis. The aim of this study is to develop an objective and didactic clinical protocol to help general practitioners and specialists on the diagnosis of GDM, in both primary and tertiary care. The study was carried out using the method of rapid literature review, which occurred through searches in the PUBMED and Scielo databases, with the terms "GESTATIONAL DIABETES MELLITUS CLASSIFICATION", "GESTATIONAL DIABETES DIAGNOSIS", "GESTATIONAL DIABETES RISK ALGORITHM", “ALGORITHM OF COMPLICATIONS OF GESTATIONAL DIABETES MELLITUS” and its correlates in english. Repeated studies, case reports, letters to the editor, comments and studies not available in English or Portuguese were excluded. Observational, descriptive studies, literature reviews, meta-analyses and systematic reviews were included. 107 publications were carried out for this protocol, which resulted in 11 flowcharts, 9 figures and 4 tables, in addition to explanatory texts, presented in 8 topics: categorization of hyperglycemia in pregnancy (pre-gestational diabetes, overt diabetes and GDM), maternal physiology, risk factors in GDM, maternal and fetal complications, screening and diagnosis of GDM, investigation of diabetes mellitus in the puerperium and practical examples for diagnosis of GDM. This project resulted in the creation of a practical instrument, which allows medical professionals carry out the diagnosis between the different forms of hyperglycemia in pregnancy, better understanding of the pathophysiology, risk factors and maternal-fetal complications of GDM, and therefore, assist professionals in the screening and and diagnosis, in situations of total/ partial financial viability and/or technical availability, regarding both GDM and diabetes mellitus in the puerperium, in order to promote early therapeutic intervention when applicable.Item Acesso aberto (Open Access) Quadruplex PCR assay for identification of Corynebacterium pseudotuberculosis differentiating biovar Ovis and Equi(BioMed Central Ltd, 2017) ALMEIDA, Sintia Silva de; DORNELES, Elaine Maria Seles; DINIZ, Carlos Augusto Almeida; ABREU, Vinicius Augusto Carvalho de; SOUSA, Cassiana Severiano de; ALVES, Jorianne Thyeska Castro; CARNEIRO, Adriana Ribeiro; VILAS BOAS, Priscilla Carolinne Bagano; SPIER, Sharon; BARH, Debmalya; LAGE, Andrey Pereira; FIGUEIREDO, Henrique Cesar Pereira; AZEVEDO, Vasco Ariston de CarvalhoBackground: Corynebacterium pseudotuberculosis is classified into two biovars, nitrate-negative biovar Ovis which is the etiologic agent of caseous lymphadenitis in small ruminants and nitrate-positive biovar Equi, which causes abscesses and ulcerative lymphangitis in equines. The aim of this study was to develop a quadruplex PCR assay that would allow simultaneous detection and biovar-typing of C. pseudotuberculosis. Methods: In the present study, genomes of C. pseudotuberculosis strains were used to identify the genes involved in the nitrate reduction pathway to improve a species identification three-primer multiplex PCR assay. The nitrate reductase gene (narG) was included in the PCR assay along with the 16S, rpoB and pld genes to enhance the diagnosis of the multiplex PCR at biovar level. Results: A novel quadruplex PCR assay for C. pseudotuberculosis species and biovar identification was developed. The results of the quadruplex PCR of 348 strains, 346 previously well-characterized clinical isolates of C. pseudotuberculosis from different hosts (goats, sheep, horse, cattle, buffalo, llamas and humans), the vaccine strain 1002 and the type strain ATCC 19410T , were compared to the results of nitrate reductase identification by biochemical test. The McNemar’s Chi-squared test used to compare the two methods used for C. pseudotuberculosis biovar identification showed no significant difference (P = 0.75) [95% CI for odds ratio (0.16–6.14)] between the quadruplex PCR and the nitrate biochemical test. Concordant results were observed for 97.13% (338 / 348) of the tested strains and the kappa value was 0.94 [95% CI (0.90–0.98)]. Conclusions: The ability of the quadruplex assay to discriminate between C. pseudotuberculosis biovar Ovis and Equi strains enhances its usefulness in the clinical microbiology laboratory.
