    Câncer gastrointestinal: dificuldades para o acesso ao diagnóstico e tratamento

    Cancer of the gastrointestinal tract has its importance in mortality profile of Brazil, being among the ten most incidents in the country. Early detection ensures better life quality for cancer patients, but often these arrive at treatment centers in advanced stage of the disease. The study investigates the difficulties of access to diagnosis and treatment for patients with gastrointestinal cancer treated by the Unified Health System. To this end, we performed a descriptive, in the form of a questionnaire survey observational database of patients undergoing treatment were collected in two public hospitals in Belém, in the period from March to June 2013.. Fulfilled the inclusion criteria 122 patients were grouped in different trajectories. In addition, were also obtained information registered in the records of these patients. The analysis of the data obtained showed that the diagnosis of the disease in 68.1% was held by the general practitioner; the greatest difficulty at that stage, was access to diagnostic tests, because spending generating the majority of patients (68.9%) did not carry out specialized examinations through the Unified Health System, but with its own resources. In the centers/units of references in Oncology, the difficulties reported by 56 patients begin with the appointment of medical consultation, schedule delay occurring by the institution for 94.6% of these patients. The lack of bed for hospitalization was cited as the biggest obstacle (54.4%) to start surgical therapy, particularly for gastric cancer and colon and rectum. The analysis of the trajectories followed by patients, since the beginning of the symptoms until the attendance in the references reveals that the diagnosis of the disease in 50% of patients occurred only 10 months after the start of symptoms, and the treatment began only after 90 days of diagnosis. The time that patients remain symptomatic without a diagnosis impacts negatively on the prognosis. In this research, the cases of gastric cancer and colon and rectum were diagnosed late (stage IV and IIIB) and therefore the treatment did not occur within desirable.

    The Waste of Health Services, especially hospital waste, are configured in important object of analysis because they involve complex environmental issues that dialogue with the challenging relationship consumption and environmentally proper disposal and, above all, minimizing the potential risks to environmental health, occupational and collective. Considering the normative instrument which guides its actions for sustainability in the management of this waste, this thesis aimed to identify the prospects and challenges the Commission's public three hospitals Waste Management from Pará State in attending to what is established legally. With technical, methodological tools for data analysis and field research and documentary, interviews and participant observation, evidence of political, social, economic and technical fragility between the members responsible for minimization, reuse, recycling and disposal of waste generated environmentally by their respective hospitals. From the analytical contributions of theoretical and methodological framework used, it was found that the economic interest together with the rejection of the changes demanded by the waste management outnumber the interests focused on the prevention and reduction of risks they may pose to global health. The identification of watering "beyond gaming" also allowed strip that waste management visibility and effectiveness is only achieved when there is commitment to the interest to go along with the social and environmental perspective that the subject requires.

    The nosocomial infection is a serious public health problem worldwide, mainly in patients admitted to the Intensive Care Unit, which are subject to greater risk due to the severity of clinical symptoms, constant use of broad spectrum antibiotics and frequency of use of invasive procedures. Staphylococcus aureus is a major pathogen that colonizes healthy individuals and is also responsible for infections in hospitalized patients. This study aimed to identify the resistance profile, main sites affected by infection and possible risk factors associated with infection or colonization by S. aureus isolated from patients and healthcare professionals from the Intensive Care Unit of Hospital Emergency and Emergency, Rio Branco (HUERB) – Acre. We developed a cross-sectional study, conducted between January to August 2009. To search for carriers, biological samples were collected from microbiota of patients and professionals and professionals hand washing. For a survey of cases of patients with nosocomial infection were collected biological samples from sites suspected of being affected, 72 hours from the date of admission until discharge, transfer or death. Of the 62 patients enrolled in the study, 19.3% were carriers and 6.4% developed nosocomial infections by S. aureus, and 35 professionals, 28.6% were carriers of S. aureus. It was the second most bacterial species isolated from patients, and was the fifth most isolated from cases of nosocomial infection. There was no statistical evidence for the variable state of coma, use of invasive procedures and state of carrier patient are considered risk factors for acquiring nosocomial S. aureus in this study. The anatomical sites affected by IH by S. aureus were the respiratory tract ( n=2), followed by blood (n=1). The sample catheter tip was responsible for one insulated. One (1.6%) patient developed IH by MRSA, and 5 (8,1%) patients and 2 ( 5.7%) professionals were MRSA carriers, low occurrence as it relates to the results of the rest of Brazil and world. We also emphasize the incidence of MRSA over MSSA and low antomicrobial resistance of MRSA demonstrating that the UTI-HUERB, the IH S. aureus does not constitute a public health problem. There were no isolates of S. aureus resistant to vancomicina, wich can be considered a therapeutic option for cases of IH by MRSA. It is worth emphasizing the importance of this study in Acre State, for being the first of its kind in the UTI, involving S. aureus and MRSA.

    A logística de materiais como elemento de análise na estrutura organizacional de um hospital universitário

    This study analysis the materials‟ logistic of one public hospital organization, evaluating how the logistic process interferes in the organization goals. The relevance of this evaluation is justified by the nature of services of a hospital, knowing that the materials logistics is crucial to provide good services. In this perspective, one important question is the legislation that concerns the process of buying and contracting services within the public administration. Another is the legal requirements that imply in fulfilling the norms and deadlines. As the processes of contracting and acquiring services are different in public and private sectors, deadlines assume an extra source of concerning. To conclude, the study overlooks to the materials management as fundamental to the logistics efficiency as its management supposes planning, coordination, evaluation and monitoring, to eliminate the non-functionality logistics process in the health services. Results show that indeed, the studied organization has failures in the materials‟ logistic process in such a way that interferes in the organization‟ goal regarding the adopted structure of this organization.
