    Análise molecular dos genes VP4, VP7 e NSP4 de rotavírus do tipo G1 circulantes em Belém e Marituba, Pará, Brasil, de 1982 a 2008

    Rotaviruses are major viral agents of acute gastroenteritis and responsible for 36% of hospitalization for diarrhea among children less than five years of age, resulting in 453.000 deaths annually, mostly in developing countries. Rotavirus is a member of Reoviridae family, and its genome consists of 11 double-stranded RNA (dsRNA) which encode 12 proteins. G1 rotavirus is commonly detected in epidemiological investigations, occurring under different prevalence rates. The aim of this study was to analyze the VP4, VP7 and NSP4 diversity genetic of G1 rotavirus circulating in Belém and Marituba, Pará, Brazil, from 1982 to 2008. We selected 83 samples previously characterized as G1 type and submitted to RT-PCR. The samples were from seven studies conducted in IEC. It was possible amplification for 63 (75.9%) specimens. Lineages 1 (8/63, 12.7%), 2 (29/63, 46.0%), 3 (18/63, 28.6%) and 9 (8/63, 12.7%) of VP7 gene were detected. The sublineages 2E and 3A were co-predominant detected in 57.1% (36/63) of samples. Three amino acid substitutions (97 [D→E], 147 [S→N] and 218 [I→V]) were observed in VP7 antigenic regions (A, B and C) in samples of 1, 2 and 9 lineages. All samples showed P[8] specificity for VP4 gene and lineages 2 (21/63, 33.3%) and 3 (42/63, 66.7%) were detected. Two substitutions (35 [I→V] and 38 [S→G]) occurred in antigenic region of VP4 of samples analyzed. For NSP4 gene, all samples belonged to E1 type. Phylogenetic analysis of NSP4 gene revealed that occurred changes in nucleotide positions 47 (C→T) and 101 (T→C), resulting in amino acid substitutions at positions 16 (S→P) and 34 (L → P) in all samples and 9 specimens displayed amino acid substitution in NSP4 toxicity residue (aa 131). This study allowed us to broaden our understanding about genetic diversity and circulation of G1 variants and represents the first molecular epidemiology analyze of this genotype in Brazil corroborating the high heterogeneity of this genotype.

    This research is about a case study in the Agricultural Cooperative Santo Antônio (COOPSANT) in Marituba City, state of Pará. The cooperative is a national and regional reference as a collective entrepreneurshipment in the area of production of hydroponic lettuce, ornamental plants and organic vegetables. The goal of the research is to study the selfmanagement process in the cooperative. For this were used qualitative and quantitative approaches based on non-formals and semi-structured interviews (application of a questionnaire) with the cooperated, as well as in loco observation. The work focuses mainly on the French School of Sociology of Organizations of Crozier and Friedberg that deals with issues such as power, organization and self management. Even facing problems such as conflicts, difficulties in enlisting the cooperation, among others, the shareholders contribute to the management of the organization, but it is obvious the predominant role of the president in coordination of actions and in the creation and modification of the operational rules of the cooperative. The power can be considered a force structuring the organization. The research identifies, in addition to the president, others in the cooperative that are strategic to it works. In conclusion we can say that the COOPSANT has a balanced self-management with an active participation of its members, but it is still essential the figure of the president driving the cooperative directions and supporting the decisions process in general assembly. The family participates actively in the form of individual and collective work, strengthening, buy this way, the familiar character of the associative entrepreneurshipment. So the cooperative members, even those who do not have family involved, say that the cooperative is a family business. The cooperative members are identified, both as a farmer, and as cooperating. The identity of the members creates a link between them and the organization to which they belong.

    Efeitos da comunicação alternativa na interação professor-aluno com paralisia cerebral não-falante

    Speech has been regarded as a social communicative modality required for building relationships in different contexts and cultures. Nevertheless, this is not always a readily available resource, especially for those who have developmental disorders. In this sense, Augmentative and Alternative Communication resources and strategies have been used by nonspeaking students with disabilities as useful tools to achieve greater social participation in different environments. Therefore, the present study examined the interaction between teachers and a student with cerebral palsy before and after the use of Alternative Communication strategies in the classroom. The study had a quantitative and qualitative nature and was designed as an intervention study. There were four main stages: 1) filming before the introduction of the resources in the classroom, 2) training the school teachers 3) providing technical assistance regarding the use of alternative communication to the child's teacher 4) filming teacher-student interactions using these technological resources. The interactions were transcribed and grouped into links. In this research, a link means the initiator's behavior directed to the exchange partner and the response in sequence from the interlocutor back to the initiator. It was found that the established interactions presented a maximum of four links, with greater frequency for episodes of 1st and 2nd links. After the intervention, there were different contexts with the presence of interactions up to the 6th link, often greater frequency for episodes of 1st, 2nd and 3rd links. In addition, the teacher showed a tendency to use symbols only as a tool for evaluating and teaching concepts. This perception changed as the teacher became familiarized with the Augmentative and Alternative Communication technology resources.

    Efetividade da atividade exercício sobre o componente de desempenho força muscular em pacientes hansênicos com incapacidade decorrente de dano neural nas mãos

    Leprosy is a highly incapacitating infectious disease. In the absence of specialized treatment, it disables, alters sensitivity and weakens hands’ muscles. The main objective was to investigate the effectiveness of an exercise protocol had on muscle strength performance. A therapeutic intervention study was conducted, and it was classified as open, self-control and involving leprosy patients coming from a reference specialized place (URE - Unidade de Referência Especializada) Dr. Marcelo Cândia, Marituba - Pará. 56 patients with hands’ nerves damage disability were evaluated, of which 36 were male and 20 female, aged from 19 to 60 years, of poor education, mostly unemployed, which 17 completed the minimum of 10 exercises sessions. The clinical characteristics present in the majority of patients were: operating clinical multibacillary form, frequent complaint about hand’s muscle weakness - which held polychemotherapy and presented greater impairment on the ulnar nerve. The patients had their palmar grip and a variety of pinches’ strength measured on Jamar® and Preston PinchGauge® dynamometers before and after the exercise protocol intervention. A specific protocol was applied through the use of therapeutic resources that offer varying strength results. At the end of these interventions, the tested strength on those 17 patients was significantly different (p < 0,05) among all categories on both hands, before and after the exercise intervention. The conclusion is that the patients who have hands muscle strength deficit caused by peripheral nerve damage, subject to exercise protocol, obtained beneficial results for muscle strength performance component enabling significant autonomy, independence and quality of life improvements.

    The thesis presented here deals with the question of mystique as an anthropological phenomenon, considering the phenomenon of mystique in culture as in language, also noting the mystical healing, in other words, the understanding of a mystical experience process in the narrative of illness or healing in practitioners of Kabbalah and Reiki in Belem and Marituba. Find a comprehension of the relationship between mystique and life, in the sense of an anthropological analysis of narratives and poems contained in the way of telling the mystical experience of the different mysticals. There is also a discussion to understand how ethnography made by the anthropologist may be affected by the narrative experience, that is, the native ethnography in the form of mystical narrative. It begins the discussion on the injunctions between ethnography and literature. Also discusses how mystical experience is treated in literary writers as poets and essayists Fernando Pessoa and Jorge Luis Borges, meaning that the mystical experience of these writers is transformed into his literary and poetic works. The mystique as metalanguage enables us to think of ethnography as metalanguage like the literature, showing the ethnographic notion as metalanguage consistent with literature. The thesis shows that the anthropological field of mystique can not be seen as a derived manifestation of religion field, or an epiphenomenon of religion. Although it has relations with religion, mystique, observed in Kabbalah and Reiki practiced in Belem and Marituba moves away from the religion field and maintains close relationships with the art field, it means to say that the mystical experience command occurs in transfiguration of life of the mysticals in a masterpiece, in a work lived in language and orality. Mystics observed both in Kabbalah as in Reiki are narrators of their lives, literary works inscribed in language, speech, bricolage of multiple references, syncretism between mystique, life and world. In some cases it observed the distance and a contrast of mystical experience against religious experience because even those mystics who claim to be religious are actually opposed to religion in order to show in their narratives that there should be an individual’s return to Self. An understanding of the Sacred is not tied to the religious institution, but is referred to within the search experience of the individual in that sense religiosity turns to be mystic, which differs from one religious identity. The mystic in his narrative takes different skins, different people, mystical narrative presents a mystical ethos that does not fit the vision of a religious ethos, because the ethos of the mystique is flexible and performative.

    Gerenciamento dos resíduos sólidos dos serviços de saúde: aspectos do manejo interno no município de Marituba, Pará, Brasil

    Medical wastes offer a potential risk to public health and the environment before an inadequate management. This study aims to verify aspects of internal handling of medical wastes in the city of Marituba, Pará State. By means of questionnaires and field visits, a descriptive and observational study was performed in 13 health establishments in the city. The total volume of generated medical wastes was about 13,000kg/week. There were deficiencies in many stages of the internal handling, for example the internal treatment that was performed in only one of the establishments, external storage made in 4 establishments and in precarious ways, among many others. Also, there were conformities as packing in adequate bags and containers as well as common waste separation. In general way, the federal norms were not accomplished and management of medical wastes in health establishments needs adequacy in every stage of the handling in order to control and reduce risks, decreasing the quantity of residues.

    Os limites da ação coletiva nos Conselhos Municipais de Saúde

    This article addresses the collective actions of participation in the municipal health councils. The underlying research sought to establish the performance of the councilors in the municipal health councils in the Metropolitan Region of Belém in the State of Pará. A survey of documents for the 2005-2010 period, structured interviews and observations with counselors in sessions of meetings of the boards of three counties were analyzed. An indicator of three levels of participation centered on the legal functions of the health councils and based on the theory of social groups as its analytical principle was constructed. The results indicate that the activities of the councils are marked by the co-option of councilors and the definition of deliberative agendas on the part of representatives of management, in which individual interests predominate over the collective interests of the councilors in the deliberations. The conclusion is that such behavior is contrary to the principles of the Unified Health System - SUS and weakens the social image of the councils, in addition to opening up a new analytical perspective.

    This research is a study in dance with ederly leprosy patients living in a shelter in the city of Marituba-PA. The investigation has its starting point in the facts recordered in the memory of lived bodily experiences in artistic practices, specifically the carnival in the period in which they resided at the Leprosy Colony. The methodological procedures used here are the etnopesquisa, because the everyday interactions through direct observation and participation, as well as the use of laboratory reserach in movement, as estimulation to recollect and express the artistic experiences lived during the compulsory segregation period. I restrict myself to propose clues for further research in dance, so that the experiments produced in this study may become a scenic product.

    Perfil clínico-epidemiológico de saúde bucal em pacientes de hanseníase

    The aim of this study was to evaluate the clinical-epidemiological profile of oral health in leprosy patients at multidrug terapy (MDT) through the oral mucosa clinic assessment, the VPI (visible plaque index), GBI (gum bleeding index), DMFTI (decayed, missing and filled teeth index) and salivary flow rate measure by SSI (salivary secretion index) as epidemiological indicators, well over buffeing capacity by salivary pH. The sample was 80 patients, aged from 8 to 73 years submited to the analysis of the oral conditions and stimulated saliva collection, divided in study group (SG) constituted of 40 leprosy patients in MDT carried in the CRTDS Dr. Marcello Candia Marituba-Pa, Brazil and control group (CG) of 40 no leprosy patients. The results pointed 62,5% unspecific oral lesions, among the most frequent was melanin pigmentation of the attached gingiva, mordiscatum line of the buccal mucosa, gingival inflammation, tongue varicose veins and trauma for use of prosthesis, without statistical significant difference (p=0,14) with the CG. The average of the VPI was 50,4 % of the dental surfaces with accumulation of bacterial plaque, without statistical significant difference (p=0,40) with the CG. The VPI was bigger in the individuals without access to the odontologic service and while it increased the degree of incapacity of the patients, juvenile was the access to the service. The average of the GBI was 29,7% of the dental surfaces with gingival bleeding, without statistical significant difference (p=0,35) with the CG. The middle DMFTI was of 11,6 varying from 4,0 to 24, which increases with the advancement of the age. The biggest accumulation of plaque, the gingival bleeding and the consumption of foods cariogenic out of the time-table of the meal also contributed to the increase of the DMFTI. Another factor made a list to the increase of I DMFTI was no access it to the odontologic service for 70 % of the leprosy patients, having statistical significant difference is (p=0,0005) with the CG and the lack of education of oral hygiene in 60 %, having statistical significant difference (p=0,01) with the CG. The middle SSI was of 0,9ml/min and did not present association with the MDT and not even with the use of prednisolone. The SG patients, 25 % presented hiposalivation, but there was no increase of DMFTI and none of the groups revealed alteration of salivary pH , varying from 5,85 to 7,34, with buffeing capacity inside the standard of normality. Conclusion that the clinical-epidemiological profile of leprosy patients is similar to the control group, do not have find specific oral lesions for leprosy, which does not cancel the possibility of the buccal cavity to be a fountain of infection for leprosy, needing histopathological confirmation and / or PCR for detection of viable M.leprae. Moreover, the data showed most of the leprosy patients do not have access to the odontologic service and not even education of oral hygiene, results absence of public politics of oral health for leprosy.

    This work entitled "Health Policy and the Prison Population: A Study in State Prison Metro I - PEM I - Para-Marituba" aims to analyze the process of higher health care to prisoners in State Prison Metro I - PEM I - Marituba / PA. For this it was necessary to use the following specific objectives: to investigate the situation of the Brazilian prison system before the picture of social inequality and criminalization; identify how the right to health is structured prison in Brazil, and analyze how the right care health is being materialized as a social right in PEM I. From the perspective of uncovering the object of study methodology was based on the critical approach, using application forms with two (02) inmates who passed health care within the PEM I. Thus, it was established limits and contradictions in health care as a social right of internal PEM I, reflected the precariousness of the right to health in prison.

    Qualidade das águas subterrâneas rasas do bairro Centro do município de Marituba - PA e sua relação com as principais fontes potenciais de poluição

    The water crisis is one of the most important social problems of the current decade. Surface water sources are increasingly polluted and groundwater is assuming an important role in human supply. The objective of this work is to analyze the shallow groundwater quality and the geometry of the flows of these waters in the central area of the Marituba district to show their relation with the potential sources of pollution. For this purpose, nineteen groundwater samples were collected from shallow wells in the central district of the municipality of Marituba. Thirty physico-chemical parameters including fifteen heavy metals were analyzed. From the results, tables, graphs and maps of isovalores distributed in the studied area were elaborated. The analyzed waters showed physical and chemical qualities that are not recommended for human consumption. Two parameters are particularly important in this context. The pH values show that, in the rainy season, about 95% of the samples showed pH values outside the drinking water range of Ordinance 2914/11 / MS. While, for the less rainy period, 79% of the samples showed pH values outside the limits of the Ordinance. Fieldwork has shown a great lack of basic sanitation in the area and the discharge of open sewage that can interact through surface runoff with underground springs. Acidic pH values can lead to corrosion in pipes and gastric diseases in humans and are therefore not suitable for human consumption. The other parameter with content above the recommendations of Ordinance 2914/11 / MS is nitrate, with 63.15% of the analyzed samples showing values above 10mg/L. This characterizes these waters as unfit for human consumption and is interpreted as evidence of contamination by liquid effluents (mainly domestic sewage) in the surface waters that interact with the groundwater, added to the presence of septic tank and lack of basic sanitation. All this is added to a shallow static level in these areas, often less than 5 meters and sometimes sub-outcropping. The Piper diagram was constructed and it showed that these waters are characterized as calcium bicarbonated. The study of groundwater flows allowed to identify the influence of potential sources of pollution of the area, with emphasis on a chemical industry existing in the southwest sector of the area. For public supply it is suggested to construct deeper wells that exploit water from confined aquifer units. For this, an alternative water supply proposal for the population of Marituba is presented at the final cost of m³ or 1000 liters of groundwater of R $ 0.124.

    Soroprevalência de testes rápidos (ML Flow) em casos de hanseníase e contatos intradomiciliares em municípios endêmicos do Pará

    Leprosy, a millennial, still poses a major challenge to public health, including in relation to early diagnosis and health surveillance. In order to determine the seroprevalence of antiPGL-1 through ML-Flow rapid test in leprosy cases and their household contacts of endemic municipalities in Pará, we performed a cross-sectional study including 73 new leprosy cases and 135 household contacts, selected from April 2011 to January 2012, the reference units for treatment of leprosy in the municipalities of Belém, Marituba, Igarapé-Açú and Santarém. The results showed a prevalence of 14.8/10.000 inhabitants of leprosy cases among contacts investigated. The ML Flow seropositivity in index cases was 53.42% in multibacillary and 13.33% in household contacts. There was a direct association of positive ML Flow in household contact with the bacterial index of the index case. There was no association with time of coexistence and consanguinity of the index case in relation to Ml Flow test. There was an association between positive ML Flow test and realization of BCG between the contacts. These results indicate that the introduction of the ML Flow test could serve as an aid in the monitoring of cases and their contacts, becoming very important in epidemiological surveillance of leprosy.

    Tinea Capitis: aspectos clínico-epidemiológicos e tratamento com terbinafina em crianças atendidas na localidade de Marituba-Pará-Brasil

    Tinea capitis is an infectious disease that affects specially children and has as the main clinical features pruritus, scaling, breaking hairs and alopecia. Conventional treatment is based on the use of griseofulvine, a drug that needs to be used for a mean period of 45 days and have important adverse effects. In this prospective work, clinical and epidemiological aspects of tinea capitis in children living in Marituba, Pará, were analyzed. Furthermore, the efficacy of a 2-week regimen of terbinafine was analyzed. Among the main findings are: 1) the identification of the genera Trichophyton as the most prevalent in the cases of tinea capitis in this region, represented by the species Trichophyton tonsurans and Trichophyton rubrum; 2) the characterization of the clinical picture with pruritus, scalling, breaking hairs and alopecia, what is consistent with the literature; 3) the absence of a specific relation between the clinical picture and the etiologic agent found, what does not permit the diagnosis of the species only by the clinical picture; 4) the identification of antropophilic fungi as the etiologic agents, what indicates the possibility of interhumans transmission instead of animal-human transmission, besides the high levels of contact found between children and animais; and 5) the efficacy and safety of terbinafine, that is well tolerated and had a high healing rate of 94.11%, specially considering the fact that the predominant genera in the region of Marituba, Pará, is the Trichophyton. Therefore, the use of terbinafine in the patients diagnosed with tinea capitis in this region is an alternative to the use of griseofulvine.
