    Aspectos clínicos e epidemiológicos da infecção pelo Papilomavírus Humano (HPV) em mulheres da população urbana e ribeirinha no município de Santarém-PA

    The human papilloma virus (HPV) is a virus that infects the skin and mucosa and is associated with cancer of the cervix, by increasing the likelihood of this happening, but parallel to this, other behavioral risk factors should be investigated. Focusing research on the prevalence and the clinical epidemiology of genital infection by HPV in women of urban and riverside municipality of Santarém-Para State proposed a comparative study to be observational, descriptive and analytical , transversal, where there was a selection of 511 women, with 391 women (38.54 ± 11.4 years) and 120 for Urban sample comprised the sample riverfront (37.2 ± 11.7 years). For data collection an interview was conducted, containing information about the factors that may be associated with the incidence of HPV and cervical cancer, plus a collection of biological material that was analyzed by cytology, DNA isolation and PCR . The analysis of these data was performed using the tab in excel and subsequent treatment through statistical software Bioestat 5.0, with the results a low prevalence of HPV compared with surveys from other regions, and homogeneity of results between urban and riverside, differing only in the topics of education, smoking status, number of sexual partners during their lifetime, and oral contraceptive used during life. Concluding that despite geographical location, there is for the municipality of Santarém a big change in the profile of woman between data found in the urban area and the waterfront on the epidemiology of HPV.

    Aspectos clínicos, epidemiológicos e moleculares do papilomavírus genital em adolescentes gestantes

    Cervical cancer represents the third most common cancer in the world. Its etiologic agent is HPV. HPV is the most prevalent sexual transmissible disease in the world, affecting mostly young women. Adolescence is a time of behavioral, social and body changes, which reflects on the adolescent’s conscious awakening for sex, that can lead to contact with sexual transmissible diseases and early pregnancy. Being so, it is a time when the woman is more susceptible to infection, such as HPV. This project was dedicated to estimate the prevalence of HPV and to evaluate factors associated to this infection in pregnant adolescents in the Health Unique System, Pará State. It was a prospective transversal study including 257 pregnant women aging 12 to 19 attended at municipal (Curió and Tapanã) and state (Mother-child Reference Unit and Woman’s ambulatory of Holy House of Mercy of Pará Foundation Hospital) health units. The selected patients were submitted to a clinical-epidemiological questionnaire and to harvest of cervical-vaginal material for detection of DNA-HPV through polymerase chain reaction (PCR). The association of HPV infection and risk factors was evaluated through chi-square test (χ2) and/or Fisher’s exact test, with an alpha significance level of 0,05. HPV prevalence was 38.1% (98/257), affecting preferentially pregnant adolescents on the third gestational trimester (44.4%; p = 0.0312), between 11 and 14 years-old, with less than 6 years of schooling and those who did not have a mate. The risk factors associated to HPV infection in the population, also varied according to the gestational trimester, were “two or more sexual partners in life”, “first pregnancy” and “genital problems”. These findings show the adolescents susceptibility to HPV infection and, consequently, to the development of pre-neoplastic and malignant lesions. Therefore, there is the need of educational public measures in health promotion.

    Confiabilidade e validade das declarações de óbitos por câncer de útero no município de Belém, Pará, Brasil

    Belém, Pará State, Brazil, presents high mortality rates for uterine cervical cancer, thus justifying an analysis of the reliability and validity of data on this underlying cause of death. Death certificates for Belém residents who died in 1998-1999 from neoplasms of the uterine cervix, uterine body, or unspecified uterine site, or with mention of such a neoplasm on any line in the death certificate, were selected, amounting to 188 death certificates (DCs). All DCs were submitted to new coding, and reliability analysis was performed by simple agreement and Cohen's kappa. The underlying cause of death, established after review of medical records and/or histopathological findings, was considered the gold standard for analysis of criteria validity, based on the positive predictive value. We observed a simple agreement of 94.0% and kappa of 0.87, suggesting high reliability of the official system's coding of uterine cancer as the underlying cause of death. In the validity analysis, 120 of the 127 deaths originally considered as caused by cancer of the uterine cervix and 3 of the 4 coded as cancer of the uterine body were confirmed. An 11.2% increase in uterine cervical tumors and a reduction of 62.5% in unspecified uterine tumors were observed.

    Estudo da prevalência da infecção pelo Papiloma Vírus Humano (HPV) em mulheres no climatério em um hospital de referência de Belém

    HPV infects a large proportion of women worldwide. Approximately 291 million women worldwide are carriers of this virus. HPV is the cause of cervical cancer, that the incidence is higher in women after 40 years old, the same age of climacteric. Therefore, it is important to investigate the presence of HPV genital infection and risk factors in climacteric women to evaluate cervical cancer susceptibility. This study is to investigate HPV genital infection in climacteric women in Belém city. We evaluated 213 women between 35 and 60 years old, whose are assistance at Ambulatório da Mulher da Fundação Santa Casa do Pará. Clincal and epidemiologic data were collected using a questionnaire. Cervical cells were collected using cytological brush to detect HPV DNA. HPV DNA was detected using PCR with MY9/MY11 primers. To type HPV 6, 11, 16, 18, 31, 33, 35, 52 e 58, it was performed using real time PCR with specific probes. Among 213 women, HPV genital infection was 11,7%. Associated risk factors were irregular use of condom and sexual initiation before 14 years old. Subtypes HPV frequency were 4% to HPV 11, 16, 31, 33 and 8% to HPV 6 e 35.

    Cancer of the cervix is an important public health problem, is the second highest incidence in females. HPV is the main risk factor for developing the disease, though not enough to cause it, requiring other associated factors. This study aimed to delineate the epidemiological profile and assess the prevalence and subtypes of HPV in women with cervical cancer. To this was applied a form epidemiological and viral DNA detection and typing of subtypes 6, 11, 16, 18, 31, 33, 35, 52, and 58 for PCR. Ninety-seven percent of the samples about cancer of the cervix were positive for HPV. The prevalence of subtype HPV 16 was 48.6%, HPV 58 subtype was 10.8%, HPV 52 subtype was 5.4% and subtypes of HPV 18 and HPV 33 was 2.7%. Women in general were married had first sexual intercourse after the age of 15, had low education. Could not detect differences in variables among women aged 45 years and over 45 years, except the condom use was greater than 80% in women no more than 45 years, being statistically significant.

    Cervical cancer is one of the serious health public problems in the world and in Brazil. Papanicolau test and health education strategies are tactics to the success of the program for prevention of cervical cancer in public health services. A cross-sectional analytical study aiming to identify and discuss interfering factors in strategies to control cervical cancer , with emphasis on HPV infection , from the perspective of program clients. The study population was formed by 858 women who were attended in public health facilities in two Family Health strategies and three basic Health Units , one of them is the Health Center School, in the city of Belém / PA . Data were collected by means of interviews. Among the factors that affect negatively strategies effectiveness of the respective program are included: client’s profile in age, educational level and family income; service characteristics and program demand type; women knowledge about uterine cervical cancer, cervical intraepithelial neoplasia, HPV virus and anti - hpv vaccine and the use of educational practices by Health Units in this study. It is concluded that interventions in public health organization services can contribute to the effectiveness of these strategies, such as efficient supervision to improve women access in the age groups which most infections prevail by HPV virus and women who delay the annual Pap smear exam. As well as the establishment of educational practices as a permanent organizational resource to raise the level of women knowledge about cervical cancer and HPV virus, Moreover taking effective monitoring of women under annual checkup. After all, get organizational tools in important primary and secondary prevention perspectives for the control of cervical cancer and HPV virus.

    The human Papillomavirus infects basal cells of stratified epithelium, inducing the development of benign proliferative lesions in the skin or mucous membranes. The infections have universal distribution. Many studies have demonstrated the strong association of infection by species of high-risk cervical cancer. Therefore, this study sought to determine the prevalence of HPV infection in a group of women screened for cervical cancer. From january 2008 to december 2009 were collected cervical samples from 180 women seen at the Laboratory of Cytopathology of Universidade Federal do Pará (UFPA), with 162 eligible for the study. Each participant was collected two samples, one for the manufacture of cytological smears for subsequent analysis by the Papanicolaou method and the other for analysis of molecular biology, in order to investigate the presence of HPV, using the primers MY09 and MY11. The species of HPV have been identified by PCR sequencing of nucleotide bases of the L1 ORF of HPV. The analysis of the epidemiological profile of the study group showed that the average age accounted for 37.5 years. The majority (49.38%) were married, 37.65% had initiated sexual activity between 13 and 17 years old and 58.64% did not use condoms during sex. The overall prevalence of HPV was found to be 18.52%. Thirteen different types were identified, with the majority (66.67%) was a low-risk group, in which HPV-11 was more frequent. Among the high risk group (30%) HPV-31 was found in four cases. The species distribution of HPV according to the cytology showed that the prevalence of infection was higher in the group of women whose smear was free of abnormal cytology (43.33%). In the group of women with premalignant changes only observed infections by species of low risk (10%). HPV infection was significantly associated with sexual activity and the frequency in the completion of screening. The prevalence in this study corroborates other findings in the medical literature. The prevalence of infection in women with normal cytology reinforces the idea that the infection is mostly asymptomatic and the Papanicolaou method is less efficient in detecting the infection than the techniques of molecular biology. However, when dealing with disease detection premalignant or malignant, molecular biology is not applicable and therefore should not replace the PCCU.

    Prevalência do papilomavírus humano em mulheres portadoras do vírus da imunodeficiência humana atendidas no serviço de atendimento especializado de Imperatriz-MA

    This work aims to assess the prevalence of HPV and the risk factors associated to the co-infection HIV HPV. Seventy-eight cervical samples of HIV-positive women attended at the SAE of an STD/ AIDS Program of the city of Imperatriz, Maranhão, Brazil, were analyzed. Cytopathology exams and amplification by PCR were realized. A questionnaire was used as instrument for data collection. The DNA positivity of HPV was 74,36%. In our study, the cytology diagnosed alterations in 16 (20,51%) of the cases. DNA HPV in 71% of the patients with inflammatory cytology was detected, and 93% of cytologies altered. Amongst the alterations we feature ASCUS with 100%; ASCUH 100%; LIE low level 100%; LIE high level 66,6%. By analyzing the social-demographic risk factors of this population in relation to the prevalence of infection by HPV, it was noticed that women who assumed they never had alcohol showed higher prevalence (87,5%), and women who presently use tobacco (84,6%) were infected by HPV. No differences were noticed amongst the variables “marital status”, “education”, “number of partners”, “use of preservatives” and “use of contraceptives”, with similar profile occurring. This study was a pioneer in the city of Imperatriz and confirmed a high prevalence of co-infection. The fight against uterine cancer must be adopted as a priority of public health services since it is a disease with potential to prevention, whose tracking is effective.

    Representações sociais de mulheres amazônidas sobre o exame papanicolau: implicações para a saúde da mulher

    This research has as objective to describe the social representations of women amazônidas on the Papanicolau examination and to analyze the implications of this for the same care of itself. One is about a qualitative-exploratório study with the use of the Theory of the social representations. The collection of data was carried through in the period from January through December of 2007, with the use of two techniques: the interview half-structuralized with open questions and the free comment. For the interpretation of these data the technique of analysis of thematic content was used. The research had as a result, three thematic units, thus called: The Papanicolau examination - a care with the health of the woman; Taboos and beliefs on the Papanicolau examination; and practical One of same care of itself: the Papanicolau examination. In the study we observe that the women are afraid of having uterine neoplasm for this reason, the Papanicolau examination represents a practical care of itself.
