    Análise da sarcopenia e a sua associação com indicadores clínicos, funcionais e de qualidade de vida em pessoas idosas atendidas no ambulatório do hospital Universitário João de Barros Barreto

    Introduction: Introduction: Ageing can be accompanied by a progressive decline in muscle mass, strength and function. This resulting health condition is known as sarcopenia, a muscle disease that develops progressively and chronically. Age-related sarcopenia has several factors that accelerate this process and need to be identified and controlled in order to promote a good health prognosis and quality of life for the elderly population. Objective: To assess the association between sarcopenia and functional and quality of life indicators in elderly people treated at the geriatrics outpatient clinic of the João de Barros Barreto University Hospital (HUJBB). Methods: This is an observational, analytical, cross-sectional study carried out at the HUJBB geriatrics outpatient clinic. Sociodemographic and clinical assessments were carried out: sarcopenia assessment (SARC-Calf, handgrip strength assessment, Tetrapolar Electrical Bioimpedance (BIA) and Short Physical Performance Battery (SPPB)), functional indicators assessment (Barthel, quadriceps strength assessment, physical activity level assessment (IPAQ) and presence of falls in 60 days) and quality of life assessment (SF-12). The algorithm of the European Working Group on sarcopenia in the elderly (EWGSOP2) was used. Results: 129 participants were assessed (73% women, p = 0.001), with a mean age of 75.4 years and from the capital (80.6%). 57% were found to be at risk of sarcopenia, with a prevalence of sarcopenia of 27.1%. Handgrip strength (HGS) 18kg/f, skeletal muscle mass 18.3kg; SPPB 9 points; Barthel score 58.8% of independent participants; quadriceps strength 14kg/f; IPAQ 38.8% with a low level of physical activity and SF-12 37.7 points for the physical component and 48.2 points for the mental component. There was an association between appendicular skeletal muscle mass (ASMR) and calf circumference (WC), age, SARC-Calf, FPM and quality of life (physical component) for men (adjusted R2 0.42 and p<0.05), as well as sarcopenia, classified by ASMR, with BIA variables: body resistance, total body water in lean mass, lean mass and basal metabolic rate for men (adjusted R2 0.49 and p<0.05), and for women, intracellular water and total body water in body weight (adjusted R2 0.60 and p<0.05). Conclusion: This study concludes that WC, age, sarcopenia risk screening, HGS and quality of life were associated with MMEA measured by BIA. For the diagnosis of sarcopenia, there were different associations between the BIA markers when adjusted for gender.

    Análise temporal e distribuição espacial das internações hospitalares por queda de idosos no Brasil, entre 2009 e 2018

    Objective: To analyze the temporal trend and spatial distribution of hospital admissions due to falls in the elderly in Brazil. Method: Cross-­sectional population-based study based on data from the Informatics Department of the Unified Health System (DATASUS) and the Hospital Information System of the Unified Health System (SIH / SUS). Therefore, the data were collected in a secondary database. Data were extracted from the SUS Information System - DATASUS, and correspond to information referring to Hospital Admissions Authorizations - AIH, of elderly hospitalized for falls in the period from 2009 to 2018 in Brazil. Data analysis used resources from descriptive and inferential statistics. Terra View® programs were used for spatial analysis and Join point® for trend analysis. Results: Over the ten years studied, the trend of hospitalizations for the elderly as a result of falls has grown significantly by 1.2% per year in Brazil. Among the regions, this behavior was variable, with periods of growth and stationary, with significant hospitalization among all age groups of elderly people above 75 years old, and among women, also observing different patterns between each state. The spatial analysis showed a Moran Global index of 0.39 (p = 0.02) in the period from 2009 to 2013, showing the existence of spatial autocorrelation, with a grouping of three states with low-­low pattern, Pará, Amazonas and Amapá. Conclusion: Brazil is heading towards a new population configuration with an increase in the elderly population. The need to improve the structure of health care networks is urgent in all regions of the country in order to provide quick and effective care to the elderly victims of falls.

    Direito à saúde do idoso portador de doença renal crônica: uma análise no município de Castanhal-Pará

    This research is about health law as a basic human right of all human beings, a right that can’t be excluded from the elderly, who are even in more need of this due to their biological condition, which will imply more investments and expenses made by the government with the goal to provide sanity for everyone but especially forthe old aged population. The essay exposes the liberalism of principles as theoretical models that are capable of substantiate the government’s duty of providing this human right (health law) to every citizen, based on John’s Rawls theory of distributive justice. By the same token, bioethical principles were stated as vectors to settle the emblematic cases, giving the chance of an equal health service especially for the elderly. It’s an exploratory descriptive research, with qualitative approach, for the old aged population that suffer chronic kidney disease and that need health hedge from the “Sistema Único de Saúde” for hemodialysis treatments. Field researches were made with social information and real diagnostics of those who suffer chronic kidney disease; biographical research through scientific perusal, with the aim to understand the concepts explained in this research; also, official government documents were analyzed; legal processes; and the applicable legislation. Through the methodological process, we can see that Brazil’s government, even though it has a constitutional regulation that cares about the distributive justice, when it’s needed; there are limitations for the defrayal in the public sanity, leaving uncovered the majority of the social welfare, for example the long waiting lists for those who need hemodialysis treatment, which is bigger everyday.

    Enfermagem de família: o enfrentamento de famílias cuidadoras de idoso com doença de Alzheimer

    Alzheimer's is a neurodegenerative disease that causes the decline of cognitive functions, reducing physical and social abilities, interfering in behavior and personality. With the evolution of the picture, Alzheimer's causes a great impact on the daily life of the person becoming more and more dependent on the help of family members. This study had as objective To know the ways of functioning of caregiver families who experience the coexistence of the care of the elderly person affected by Alzheimer's disease, in the domestic sphere. METHODOLOGICAL DESCRIPTION: A descriptive study with a qualitative approach, used as a theoretical and methodological reference point, the Calgary Family Assessment Model. It was developed in the city of Belém-PA, in a reference hospital in geriatrics. The study included 08 families of elderly people diagnosed with Alzheimer's who were being treated at the outpatient clinic of the João de Barros Barreto University Hospital. The semistructured interview technique was used, whose sessions were written in full and drawn (genogram), avoiding the loss or misrepresentation of the information. The analysis of thematic content was carried out in the light of the Calgary Family Assessment Model in three categories: structural, developmental and functional evaluation, presented through the Genogram and Ecomapa through the statements of each family interviewed. RESULTS: From the experiences of caring for the elderly with Alzheimer's, we found that families experienced significant changes and problems of various magnitudes that compromised some relationships in the internal and external structure of families. The Calgary Family Assessment Model allowed us to observe a scientific and systematic structure, through its family assessment categories, because it helped us gather information, which subsidized us in the construction of the Genogram and Ecomapa in a way that allowed us a more structure of the family structure. Conclusion: We concluded that the objective of the research was achieved, since the elderly and their family members require a qualified and systematized assistance in order to enable paths that favor feelings of hope, safety before treatment and, consequently, aiming a satisfactory prognosis. we emphasize the need for new studies related to the family of the elderly with Alzheimer, especially the applicability of the Calgary Family Intervention Model (MCIF), in order to deepen and enable new dimensions, providing to family nursing new collaborations of great scientific relevance.

    Funcionalidade familiar de idosos brasileiros residentes em comunidade

    Objective: To evaluate the family functioning of Brazilian elderly people and test how determining factors influence it. Methods: Cross-sectional study with 2,052 elderly people based on data collected in the baseline of the study Aging, gender and quality of life (AGEQOL), with participants answering questionnaires about family dynamics, basic and instrumental activities of daily living (ADL and IADL), cognitive state and sociodemographic characteristics. Multivariate ordinal regression models and multiple correspondence analysis identified factors associated with good family functioning. Results: Most elderly people had good family functioning (76.3%), were married and lived with their spouse (55.5%), had more than six children and grandchildren (85.4% and 76.7%, respectively) and were independent to perform IADL (71.5%). Correspondence analysis resulted in three groups: good, moderate and poor family functioning, and a profile of elderly people with different socioeconomic conditions. Conclusion: It was possible to infer implications for practices and policies of family care with elderly members to meet their specific routine and life and health conditions.

    A intervenção em dupla-tarefa protege do declínio associado à idade nas atividades em dupla-tarefa

    Introduction: Older adult’s functionality is related to the ability to divide attention into daily life multitasking activities. The ability to coordinate attention in motor and cognitive activities performed simultaneously decreases with aging, compromising functional ability, and therefore older adults' participation and healthy aging. Objectives: The main objective of the present study is to evaluate the effects of a multimodal physical exercise intervention protocol, at moderate intensity, simultaneously with cognitive stimulation (dual-task) on the dual task cost in healthy community-dwelling older adults. Methods: 70 older adults, with no cognitive disfunction participated in the study. Participants were grouped into a Dual Task Exercise group (DTEx, n=40) who performed the intervention protocol of 24 sessions, twice a week, for 75 minutes, and a control group (CG, n=30) who received information on health education and did not perform physical exercises. Two-way mixed ANOVA was used for dual-task cost analysis and Bonferroni tests were used as post-hoc for within-group and between-group comparisons. The project was registered in the Brazilian Registry of Clinical Trials (UTN code: U1111-1233 6349) and approved by the Research Ethics Committee of the Institute of Health Sciences of the Federal University of Pará (CAAE no. 03427318.3.0000.0018). Results: There was Group x Time interaction (F (1.68) = 7.207 p ≤ 0.009, η2 p = 0.096) observed for the performance of the motor component of the dual task cost. The DTEx group showed maintenance of the gait speed as the motor component of the dual task cost (Assessment = -11.4% ± 3.0; Reassessment = 10.2% ± -2.6, p = 0.665) while the CG showed an increased cost of approximately 49.76% (Assessment: -10.4% ± 3.4; Reassessment: -20.9% ± 3.0 p ≤ 0.002). Significant differences were found on the performance of the motor component of the cost of the dual task between the groups in the post-intervention condition (GC Reassessment: -20.9 ± 3.0. Reassessment DTEx = -10.2% ± -2.6, p ≤ 0.011). No main effects were observed in the assessment of the cost of the cognitive component. Conclusions: The results suggest that moderate-intensity multimodal physical exercise associated with dual-task cognitive stimulation attenuated the decline in the dual-task cost in the older adults. The cost of dual task is an important clinical measure to assess the functional and cognitive ability to perform tasks of daily living in aging.

    The concept of Development Niche consists of three subsystems: the physical and social environment, the psychology of caregivers and care practices. The present study aimed to apply the concept of Niche for the study of development in old age focusing on the care provided in Long Term Care Institution for Elderly (LTC), through environmental research, beliefs and knowledge of caregivers and care practices. The study was conducted in a Philanthropic LTC and was attended by five formal caregivers. The results indicate that the bathroom was the space that received more negative evaluations; In addition, we identified little interaction among older and no activities that encourage this practice; all caregivers are female, mean age 48.2 years and with low education; beliefs were neutral in general, especially in the domain that agency received more negative evaluations; the level of knowledge about old age was low and care practices, mostly involving childish treatment, devaluation of understanding of the elderly, threat of aggression etc. It follows that the subsystems of the concept of Development Niche can also be used to evaluate the development and care in LTC, since the presence of a poor environment stimulation, with caregivers who have negative beliefs about the independence of older and with few knowledge in gerontology may favor the emergence of inadequate care practices, making it difficult to maintain the functional capacity of the elderly.

    O/A idoso/a na política de assistência social: um estudo nos espaços da proteção social básica no município de Belém-PA

    This study aims to analyze the relationship between the legal and the real with regard to the Social Assistance Policy regarding the offer of actions in the Cohabitation Service for the elderly in the municipality of Belém-PA. The research had as method of investigation the historical and dialectical materialism, having as technical-operative procedures: bibliographical, documentary and field survey. The bibliographical survey prioritized beyond the statistics on longevity the debate of classic and contemporary authors problematizing human aging in the context of the production of capitalist inequalities, understanding it as a socially constructed category. The documentary survey consisted of readings of national and international documents, as well as documents from federal agencies, in order to identify the demographic aspects of aging at the national and local world level, as well as, if a historical overview of the policies of social protection to the elderly in Brazil, having as initial mark the guidelines implemented by international organizations. The field work consisted of the systematic observation of the twelve CRASs and the Zoé Gueiros Third Age Co-existence Center, as well as the consultation of technical documents produced to guide the actions carried out with users such as reports, projects and programs, in addition to, of the medical records of the elderly. In addition, a semi-structured interview with five professionals from four teams was also carried out to analyze how they evaluate the limits and possibilities for the materialization of the precepts of the Policy at the local level. The results indicate the contradictions between what the Assistance Policy advocates with the reality observed in the Coexistence Service's execution spaces, thus, it was identified the precariousness in the physical structure and in the material and human resources used in the CRAS, as well as the unequal treatment that the Municipal Assistance Policy offers to the Zoé Gueiros Center, which is the only space that meets the structural and accessibility requirements set by the Policy. Also, it was found that women in the age group of 60 to 69 are the main claimants of the Service, mainly because of the isolation condition in which they live which leads them to be regularly served by the Service, resisting the dismissal after two years of admission, recommended by the Policy, this situation leads to the technicians' effort to make feasible strategies that legitimize the permanence of these elderly women in the Service, demonstrating the contradiction existing in the Politics characterized by the discourse of access universalization and the real difficulty in its materialization. It is concluded, therefore, that in spite of advances in the scope of the Assistance Policy, in particular with regard to the Coexistence Service for the elderly, the materiality of the actions, today, are still permeated by weaknesses. In this sense, it is considered that there are structural and management limits that directly affect the materialization of actions, because what is observed in reality is that the social assistance spaces attend minimally to what predisposes the Policy, which leads to both effort and improvisation of professionals in trying to attend to what the Policy advocates. Therefore, it is evident that the real and the legal regarding the offer and execution of social assistance services dialogue minimally, in the spaces of execution of the Policy in the municipality of Belém-Pa, especially in the Coexistence Service.

    Principais queixas vocais de pacientes idosos pós-tratamento para hanseníase

    Leprosy is an infectious disease, with vocal involvement varying between hoarseness and difficult breathing. Aim: compare the main vocal complaints among elderly patients after treatment for leprosy and a control group. Study design: descriptive prospective. Materials and methods: We included 50 patients aged over 60 years, 32 had been treated for leprosy, and the others formed the control group. We used our own questionnaire to analyze the vocal symptoms presented by the two groups, as well as gender, age, life style and comorbidities. Results: among the treated group, the most frequent symptoms were hawk (34.4%) and hoarseness (28.1%), while in the control group the most prevalent symptoms were hoarseness (77.8%) and a foreign body sensation (55.6%). Conclusion: the most prevalent voice complaints in patients treated for leprosy are hawking and hoarseness, and that its development is influenced by life style and associated diseases.

    O protagonismo da prevenção de quedas por idosos na perspectiva de promoção da saúde de Nola Pender

    INTRODUCTION: Falling in the elderly represents a public health problem, since it manifests itself in high prevalence, presents multi­causality and is one of the geriatric syndromes. It is important to emphasize the importance of nursing to develop safe practices directed to the factors that influence the health behaviors of the elderly. OBJECTIVES: To apply the Pender Health Promotion Model in the Nursing process to the elderly with risk factors for falls; Evaluate health behavior safety promoters against falls; Explore self­efficacious practices of safe behaviors in the face of the event falls. METHODOLOGY: The study was characterized by a descriptive research with a qualitative approach, such as Research Convergent Care. This study was carried out with 7 elderly residents of the Guamá ­ Belém / PA district who are users of public services. The data were obtained through the application of the nursing process, having for analysis, the sources: (a) Nursing History; (b) The application of the Photovoice technique; (c) International Classification for Nursing Practice ­ CIPE; d) Planning, debates and synthesis of each of the four focal group meetings. RESULTS: Health behaviors according to MPS are summarized in three categories and their subcategories: 1 Characteristics, Individual experience: Risk behavior for falls, Consequence of falls and Learning with falls; 2 Specific Behavior: Influential self­perception for falls, Interpersonal influences that affect the behavior of others, Wishes for changes that will contribute to the prevention of falls; 3 Behavioral Result: Self­efficacious fall prevention and positive action / suggestion assessment. The self efficacious practices exercised during the application of the nursing process resulted in initial changes in behavior regarding life habits such as: paying more attention to where to walk, wandering slowly and looking at the floor, do not be ashamed to ask for help. CONCLUSION: The demonstration of the health behavior of the elderly has influence coming from the experiences of fall. Although being users of public services, no influences from educational programs were perceived. Regarding the observed behavioral changes of self­efficacious practices, some positive life habits were observed, however, socioeconomic and family barriers make it difficult to prevent falls in the elderly, especially in the context studied.

    The rate of HIV infection has increased among the elderly. Likewise, the improvement of social life and the growth of the pharmaceutical industry have stimulated the prolongation of sexual activity in old age. However, the conception of the elderly as an asexual person, including among health professionals, has reduced the space for the elderly to express their sexuality or to discuss the subject in a way that provides information, resulting in dangerous sexual practices and remoteness of their sexual health. The objective of this study was to analyze how the knowledge about HIV / AIDS reflects the sexuality experience of the elderly users of a Health Center in the city of Belém-PA. For this purpose, it was opted for the evaluation research approach, adopting the triangulation method. In compliance with the first objective that was assessing the knowledge of the elderly about HIV / AIDS, applying the Questionnaire about HIV to Third-Age (QHIV3I). Already to reach the second objective, which was to explore how the elderly think and experience their sexuality, it was adopted the focus group technique, applying for two separate groups of men and women. The data obtained through the questionnaire were treated and analyzed by simple descriptive statistics, resulting in the fact that the elderly people who participate of this research have some knowledge about AIDS, mainly in relation to the areas "concept", "transmission" and "prevention ". However, it was observed the existence of knowledge gaps especially about the asymptomatic phase of the disease and ways in which virus transmission does not occur. In addition, 93% of them do not use condoms in their sexual practice and only 40% have already had the HIV test. The data obtained through the study group were treated by the Thematic Content Analysis, thus emerging four categories: sexuality of the elderly among the challenges of aging and prejudice; elderly widows – the influence of marital status on sexuality; the negotiation of condom use among couples of the elderly – between trust and suspicion of infidelity; Nursing between the old and new: the conversation about IST and HIV / AIDS with the elderly. It was concluded that knowing does not always reflect positive attitudes toward HIV / AIDS prevention and other STIs. The elderly do not use the condom because of the following reasons: trust in the partner, relationship of power of the man to the woman, biological changes and lack of habit.

    INTRODUCTION: Population aging is considered a worldwide demographic phenomenon. In Brazil we specifically observed a rapid and consistent demographic expansion in the elderly population, the country counts with about 21 million people aged 60 years or more, representing a significant portion of the total population, approximately 11.3%. As a consequence of this change in the age pyramid, we have the increased prevalence of chronic non-communicable diseases, which in turn can lead to functional disability. In addition to chronic diseases, mental health deserves special attention, as they often affect the elderly, with dementia and depression being the most prevalent. Understanding the factors associated with the incidence of depression, as well as the functional and cognitive impairments resulting from this condition, allow us to assist in the prevention and early identification of these symptoms, promoting specific intervention strategies in order to avoid or minimize damages to the quality of life of the elderly. OBJECTIVES: To identify the presence of depressive symptoms, cognitive decline and functional impairment in the elderly enrolled in the program for the control of hypertension and diabetes mellitus of the Municipal Health Unit of Guamá, in the city of Belém, Pará. METHOD: Cross-sectional study with a sample of 100 The elderly. Data were collected through individual interviews using the Mini Mental State Examination (MMSE), the Geriatric Depression Scale (GDS-15), the Katz Scale (ABVD) and the Lawton and Brody Scale (AIVD) for the evaluation of Cognitive ability, presence of depressive symptoms and functional capacity, respectively. Pearson's Chi-square test for trend / adhesion and association between nominal variables and Pearson's Linear Correlation test were used for numerical variables. RESULTS: The profile of the studied population is predominantly female, with low educational level, low income and age range between 60-69 years. The prevalence of depression was estimated in 22% of the sample and was associated with female and sedentary elderly individuals. Regarding mental status, cognitive deficit was identified in 27% of the interviewed elderly, and the age of the elderly presented a moderate negative correlation with the MMSE performance. Regarding the functionality of the elderly interviewed, most of them were considered independent to perform basic activities of daily living, and 46% of the sample had dependence on the instrumental activities of daily living. There was an association between depressive symptoms and functional dependence. CONCLUSION: It was verified that sedentary women and elderly people are more susceptible to the occurrence of depression and that age is a factor strongly associated with a decrease in cognitive performance. It was also observed that the level of dependence of the elderly in the AIVD is strongly associated with the presence of depressive symptoms.
