| dc.description.abstract | Although the act of swallowing can be considered a voluntary action of transferring food from the oral cavity to the stomach, this function of the human body can be initiated long before the presentation of food and requires extensive brain activity that allows the swallowing process to occur a coordinated, safe, effective, pleasurable way and enables the individual to behave at a social level, ensuring interaction with the environment and maintenance of his own body. However, the neurological mechanisms of swallowing are still not entirely clear and require investigation for a better definition of its normality pattern and understanding of its alterations through disorders that may affect the Central Nervous System (CNS). So, this work aims to analyze the cortical and subcortical areas activated during the swallowing function. This is a quantitative and qualitative, observational, cross-sectional and descriptive study, with a convenience sample (nonprobabilistic) of the continuous type. A total of 9 healthy men, with a minimum of 3 hours of fasting, underwent 1 Tesla Functional Magnetic Resonance Imaging (FMR) with axial T2 Flair sequences, 3D Bravo and BOLD echoplanar sequences for functional evaluation of swallowing. Each participant was asked to perform multiple swallows of saliva with a gustatory stimulus in the mouth (mint candy) for a period of 1 minute with an interval of 1 minute (ratio 1:1). During the analysis of the exams, a total of 13 brain areas activated during swallowing were identified, being the Thalamus, Cerebellum, Primary Motor Cortex, Somatosensory Cortex, Anterior Cingulate Cortex, Inferior Occiput Parietal Cortex, Inferior Frontal Cortex, Superior Temporal Cortex, Frontal Cortex Medial Cortex, Superior Parietal Cortex, Nucleus Ambiguus, Inferior Salivary Nucleus and Insular Cortex. The area of the Primary Motor Cortex, where the Motor Homunculus is characterized, was the only common area in activation among all research volunteers (100%), followed by the left Frontal Medial Cortex (77.78%), left Inferior Parietal Occipito Cortex (77.78%) and Cerebellum on the left side (77.78%). The lowest frequency of areas that were not activated in the sample of this research were the Medial Frontal Cortex (11.11%) and Lower Frontal Cortex (11.11%). Although common areas of activation in the brain are observed in the sample of this study, it is not possible to say that the central nervous system behaves in the same way for the same task of swallowing. Thus, the distributed nature of this swallowing neurological network helps to explain why so many neurological conditions result in dysphagia, and these findings are therefore of great clinical relevance, because by understanding the cortical areas for a given function, it is possible to better understand how the nature of distribution of the swallowing neurological network the synaptogenic effects that patients with neurological injuries develop in the swallowing rehabilitation process when this difficulty is impaired. Swallowing involves a very wide distribution network of neural areas, still little understood and possible to be larger than supposed. | |
| dc.description.resumo | Embora o ato de deglutir possa ser considerado uma ação voluntária de transferência do alimento da cavidade oral para o estômago, essa função do corpo humano pode ser iniciada muito antes da apresentação do alimento e requer extensa atividade do cérebro que permite que o processo de deglutição ocorra de maneira coordenada, segura, eficaz, prazerosa e possibilita o indivíduo se comportar em nível social garantindo interação com o meio e manutenção do próprio corpo. Entretanto, os mecanismos neurológicos da deglutição ainda não são totalmente claros e requerem investigação para melhor definição de seu padrão de normalidade e entendimento de suas alterações mediante distúrbios que possam acometer o Sistema Nervoso Central (SNC). Então, este trabalho tem por objetivo analisar as áreas corticais e subcorticais ativadas durante a função de deglutição. Trata-se de um estudo quanti-qualitativo, de caráter observacional, transversal e descritivo, com amostra por conveniência (não-probabilística) do tipo contínua. Fizeram parte deste estudo o total de 9 homens hígidos, com mínimo de 3 horas de jejum, submetidos a Ressonância Magnética Funcional (RMF) de 1 Tesla com sequencias axiais de T2 Flair, 3D Bravo e sequências ecoplanares BOLD para avaliação funcional de deglutição. Cada participante era solicitado a realizar múltiplas deglutições de saliva com um estímulo gustativa na boca (bala de menta) durante o tempo de 1 minuto com intervalo de 1 minuto (relação 1:1). Durante análise dos exames foram identificadas o total de 13 áreas cerebrais ativadas durante a deglutição, sendo o Tálamo, Cerebelo, Córtex Motor Primário, Córtex Somato Sensorial, Córtex Cingulado Anterior, Córtex Occipito Parietal Inferior, Córtex Frontal Inferior, Córtex Temporal Superior, Córtex Frontal Medial, Córtex Parietal Superior, Núcleo Ambíguo, Núcleo Salivar Inferior e Córtex Insular. A área do Córtex Motor Primário, onde é caracterizado o Homúnculo Motor, foi a única área comum em ativação entre todos os voluntários da pesquisa (100%), seguido do Córtex Frontal Medial esquerdo (77,78%), Córtex Occipito Parietal Inferior esquerdo (77,78%) e Cerebelo do lado esquerdo (77,78%). Já a menor frequência de áreas que não foram ativadas na amostra desta pesquisa foram o Córtex Frontal Medial (11,11%) e Córtex Frontal Inferior (11,11%). Embora seja observado áreas comuns de ativação no cérebro na amostra deste estudo, não é possível dizer que o sistema nervoso central se comporta da mesma maneira para a mesma tarefa de deglutir. Assim, a natureza distribuída dessa rede neurológica da deglutição ajuda a explicar por que tantas condições neurológicas repercutem em disfagia, sendo, então, esses achados de grande relevância clínica, pois ao entender as áreas corticais para determinada função é possível compreender melhor como a natureza de distribuição da rede neurológica da deglutição os efeitos sinaptogênicos que pacientes com lesões neurológicas desenvolvem no processo de reabilitação de deglutição quando essa dificuldade se encontra prejudicada. A deglutição envolve uma rede de distribuição de áreas neural muito ampla, ainda pouco compreendia e possível de ser maior do que se supõe. | |
