Item Acesso aberto (Open Access) Abandono do tratamento de tuberculose em co-infectados TB/HIV(2010-06) RODRIGUES, Ivaneide Leal Ataíde; MONTEIRO, Larissa Lima; PACHECO, Régia Hevelline Barros; SILVA, Sílvio Éder Dias daThis study aimed at analyzing the reasons that patients co-infected with tuberculosis and HIV leave the treatment of tuberculosis and to know the conduct of the health team toward that abandonment. The study, using a qualitative approach, performed semi-structured interviews on 45 professionals working at a referral health center in Pará state. Two units emerged based on the thematic analysis: patient-associated factors that make TB treatment adherence difficult; and service-associated factors that contribute to treatment abandonment. It was found that, in terms of the patients, that their low socioeconomic condition was the most common factor that led to abandonment. Other factors that led to this outcome included the adverse drug effects, the use of illegal drugs, and poor personal motivation. Regarding the service, issues related to the physical structure, working process organization and accessibility were also relevant to their non-adherence. Results show there is a need to change the practices performed at the health care services.Item Acesso aberto (Open Access) Ação e raciocínio pedagógico de professoras de enfermagem: expressões em diferentes contextos educacionais(Universidade Federal do Pará, 2018-08-27) MENEGAZ, Jouhanna do Carmo; BACKES, Vânia Marli Schubert; MOYA, José Luis MedinaObjective: to report sources of knowledge base for teaching, categories of knowledge base for teaching and phases of pedagogical reasoning and action of professors of higher education in Nursing in public and private universities in the South region of Brazil. Method: t collective study of instrumental cases with a qualitative approach. The cases involve the pedagogical reasoning and action of two professors of Nursing. Data collection incorporated documents, interviews and observation analyzed using the constant comparative method. The analysis gave rise to two metacategories, termed public case and private case, and their transversal axes: relationships between sources and knowledge base for teaching, relationships between categories of knowledge base for teaching, and relationships between knowledge base for teaching and the Model of Pedagogical Reasoning and Action. Results: there are distinct relationships between sources, knowledge base for teaching and phases of the Model in the cases’ pedagogical reasoning and action. Interviews were conducted with 40 adolescents from a support insti tuti on in the state of Pará. Thematic content analysis resulted in the category: attitudes and behavior related to drinking alcohol. Alcohol consumpti on is associated with fun, but it is also a way to avoid thinking about problems. The adolescents used other types of drugs at times, but the use of alcohol was unanimous. In conclusion, alcohol promotes socialization and enjoyment for adolescents, and it may lead to abuse and the contact with illicit drugs, such as marijuana, cocaine and thinner.Item Acesso aberto (Open Access) Aumento das cargas de trabalho em técnicos de enfermagem na atenção primária à saúde no Brasil(Universidade Federal do Pará, 2016-10) SCHERER, Magda Duarte dos Anjos; PIRES, Denise Elvira Pires de; TRINDADE, Letícia de Lima; GONÇALVES, Ana Sofia Resque; VIEIRA, Monica; OLIVEIRA, Neura Angélica deIn this article we analyze the main aspects that contribute to increase the workloads of nurse technicians working in primary health care in Brazil, we identify the type of loads and rank the most prevalent aspects of this occupational group. It is based on a multicenter study with 24 professionals from the South, Center-west and North regions of Brazil, from 2011 to 2013, using data from semi-structured interviews. The data were systematized in Atlas.ti software and submitted to thematic analysis. The aspects that contribute to increasing workloads are low monthly remuneration, lack of material resources, work overload, wear and illness, relationship with the user, and lack of human resources, prevailing psychological and physiological loads. Adequacy of the conditions and organization of work are shown paramount to ease the workload on the primary health care setting, and to promote the health, wellbeing and quality of life in the nurse technicians working environment.Item Acesso aberto (Open Access) Caracterização da estratégia saúde da familía no estado do Amazonas, Brasil: análise da implantação e impacto(Universidade Federal do Pará, 2011-01) OLIVEIRA, Verdelândia Milon de; GOLÇALVES, Maria Jacimara Ferreira; PIRES, Rodrigo Otavio MorettiThis article focuses on the implementation of the Family Health Program (FHP) in municipalities (counties) in the State of Amazonas, Brazil, analyzing potential population coverage, evidence of change in the health care model, and impact (2004 and 2008). Implementation was classified as fair versus unsatisfactory. The findings showed an increase in the number of municipalities with the FHP implemented, as well as in the program's coverage (which nevertheless was less than 50% of the population). Concerning change in the health care model, no municipalities were observed with a high rate of change. As for impact, there was a decrease in low impact and an increase in high impact. The final classification did not show a statistically significant change from one year to the next, and unsatisfactory implementation prevailed in the municipalities of the State of Amazonas. Evaluation of the FHP is not simple, since there are various factors involved in the issue. Further studies could reveal the factors that explain the program's suboptimal implementation in the State of Amazonas.Item Acesso aberto (Open Access) Clinical aspects in patients with pulmonary infection caused by mycobacteria of the Mycobacterium abscessus complex, in the Brazilian Amazon(Sociedade Brasileira de Pneumologia e Tisiologia, 2018-04) MONTEIRO, José Tadeu Colares; LIMA, Karla Valéria Batista; BARRETTO, Adriana Rodrigues; FURLANETO, Ismari Perini; GONÇALVES, Glenda Moraes; COSTA, Ana Roberta Fusco da; LOPES, Maria Luiza; DALCOLMO, Margareth PrettiObjective: To describe the clinical manifestations of patients with pulmonary infection caused by mycobacteria of the Mycobacterium abscessus complex (MABSC), and to compare these manifestations with those of patients infected with other nontuberculous mycobacteria (NTM). Methods: This was a retrospective cohort study involving 43 patients divided into two groups: the MABSC group, consisting of patients with pulmonary infection caused by MABSC (n = 17); and the NTM group, consisting of patients with pulmonary infection caused by NTM other than MABSC (n = 26). Patients were previously treated with a regimen of rifampin, isoniazid, pyrazinamide, and ethambutol before the diagnosis of NTM was confirmed by two culture-positive sputum samples. The nucleotide sequences of the hsp65, 16S rRNA, and/or rpoB genes were analyzed to identify the mycobacteria. Data were collected on demographic, clinical, and radiological characteristics, as well as on treatment responses and outcomes. Results: Loss of appetite was the only clinical manifestation that was significantly more common in the MABSC group than in the NTM group (p = 0.0306). The chance of having to use a second treatment regimen was almost 12 times higher in the MABSC group than in the NTM group. Treatment success was significantly higher in the NTM group than in the MABSC group (83.2% vs. 17.6%; p < 0.0001). The chance of recurrence was approximately 37 times higher in the MABSC group than in the NTM group. Conclusions: In the study sample, treatment response of pulmonary disease caused by MABSC was less favorable than that of pulmonary disease caused by other NTM.Item Acesso aberto (Open Access) Confiabilidade e validade das declarações de óbitos por câncer de útero no município de Belém, Pará, Brasil(2004-10) CARVALHO, Jacira Nunes; KOIFMAN, Rosalina Jorge; MATTOS, Inês Echenique; MONTEIRO, Gina Torres RegoBelém, Pará State, Brazil, presents high mortality rates for uterine cervical cancer, thus justifying an analysis of the reliability and validity of data on this underlying cause of death. Death certificates for Belém residents who died in 1998-1999 from neoplasms of the uterine cervix, uterine body, or unspecified uterine site, or with mention of such a neoplasm on any line in the death certificate, were selected, amounting to 188 death certificates (DCs). All DCs were submitted to new coding, and reliability analysis was performed by simple agreement and Cohen's kappa. The underlying cause of death, established after review of medical records and/or histopathological findings, was considered the gold standard for analysis of criteria validity, based on the positive predictive value. We observed a simple agreement of 94.0% and kappa of 0.87, suggesting high reliability of the official system's coding of uterine cancer as the underlying cause of death. In the validity analysis, 120 of the 127 deaths originally considered as caused by cancer of the uterine cervix and 3 of the 4 coded as cancer of the uterine body were confirmed. An 11.2% increase in uterine cervical tumors and a reduction of 62.5% in unspecified uterine tumors were observed.Item Acesso aberto (Open Access) Constituição cidadã e representações sociais: uma reflexão sobre modelos de assistência à saúde(2010-12) SILVA, Sílvio Éder Dias da; RAMOS, Flávia Regina Souza; MARTINS, Cleusa Rios; PADILHA, Maria Itayra; VASCONCELOS, Esleane VilelaThis article presents a reflection on the meaning of the terms citizenship and health, addressing the Theory of Social Representations as a strategy for implementing and evaluating health care models in Brazil. First, a brief history about the concept of citizenship is presented; then the article addresses the principles of freedom and equality according to Kant; the third section of the article shows that health is as a right of the citizen and a duty of the state. METHODS: A qualitative study in health linked to the methodological framework of Freire, which includes obtaining and analysis of data in dialogical circles, comprising three dialectical moments, intertwined in an interdisciplinary manner: thematic research, coding and decoding, and critical unveiling. This occurred in the period between April 2009 and February 2010, with an irregular number of participants, in the auditorium of the President Vargas Specialized Reference Unit, in the city of Belém / PA (Brazil). RESULTS: Inadequate training of health professionals was one of the most relevant topics generated, and unveiled the necessity of implementing a program of continuing education on the care of people with stoma. CONCLUSION: The proposed unveiling constituted a life experience so that these people with stomas were enabled to "read the world" and thus know and walk towards the transformation of their reality.Item Acesso aberto (Open Access) Dinâmica da família no contexto dos cuidados a adultos na quarta idade(2013-04) POLARO, Sandra Helena Isse; GONÇALVES, Lúcia Hisako Takase; NASSAR, Silvia Modesto; LOPES, Márcia Maria Bragança; FERREIRA, Viviane Ferraz; MONTEIRO, Hellen KarinnaThis study aimed to determine the pattern of family functioning on everyday care relationships of adults in the fourth age. This is a study of diagnostic-evaluative nature of adults with 80 or more years old who depend on care, and of their relatives as caregivers. The participants were selected among the registered patients of a Family Health Unit in a district in the suburbs of Belém-PA, Brazil. They were evaluated according to the dynamics of their family, and quality of life related health lifestyle. Most of the elderly rated their families with good functionality. However, data on the elderly and caregivers' quality of life and caregivers' life style only reached the median level, showing some difficulty in the family functioning system. It was concluded that the multiple results obtained through the assessments indicate some practical implications of care to the family unity and confirm the need for multidimensional assessment about the family intervention.Item Acesso aberto (Open Access) Enfermeiras desafiando a violência no âmbito de atuação da estratégia de saúde da família(Programa de Pós Graduação em Enfermagem, Universidade Federal de Santa Catarina, 2013-12) POLARO, Sandra Helena Isse; GONÇALVES, Lucia Hisako Takase; ALVAREZ, Angela MariaThis study aimed to describe and analyze how violence affects the working process of nurses in the Family Health Program. It consists of an exploratory-descriptive study developed in a district in the suburb of Belém, Pará, Brazil. Data were collected between August 2009 and February 2010, by means of interviews with 14 nurses, and then submitted to the method of content analysis, generating one of the themes of this study: 'Challenging violence', with derivative sub-topics such as territorial violence, institutional violence and intrafamilial violence. The result showed the impact of the violence phenomenon on the working process of the nurses of the Family Health Program units. This violence induces them to feel fear and professional frustration due to embarrassment and limitation of their roles, although they still challenge the obstacles encountered in their daily work.Item Acesso aberto (Open Access) Esse tal Nicolau: representações sociais de mulheres sobre o exame preventivo do câncer cérvico-uterino(2010-09) SILVA, Sílvio Éder Dias da; VASCONCELOS, Esleane Vilela; SANTANA, Mary Elizabeth de; RODRIGUES, Ivaneide Leal Ataíde; MAR, Dayse Farias; CARVALHO, Francilene da Luz CarvalhoThis research aims to identify women's social representations of women regarding cervical cancer and describe the relationship of these social representations with preventive care. The study used a qualitative-exploratory approach, with the theory of social representations as the theoreticalconceptual support. Data collection was performed using two techniques: the free association of words and the semi-directed interview with open questions. The thematic analysis technique was used for data interpretation. The research resulted in two thematic units: cervical cancer: an early treatable wound and the screening test: taking because of fearing. We studied 414 mother-baby binomials which babies were born in 2016. Socioeconomic and obstetrical questionnaires applied the Self-Efficacy Scale in Breastfeeding to mothers at their hospital discharge. The breastfeeding score mean was 61 points; 32.45% of the mothers were between 19 and 23 years old; 36.56% of them had a high school diploma; 43.34% received up to a minimum wage; 62.95% lived an informal union with their partners; and 17.92% were employed. The factors related to exclusive breastfeeding involved the mother age and schooling, the fact that the mother is employed, her marital status, the partner incentive to breastfeeding, and the self-efficacy scale score.Item Acesso aberto (Open Access) Healthy aging profile in octogenarians in Brazil(Universidade Federal do Pará, 2016-08-29) CAMPOS, Ana Cristina Viana; FERREIRA, Efigênia Ferreira e; VARGAS, Andréa Maria Duarte; GONÇALVES, Lucia Hisako TakaseObjective: to identify the healthy aging profile in octogenarians in Brazil. Method: this population-based epidemiological study was conducted using household interviews of 335 octogenarians in a Brazilian municipality. The decision-tree model was used to assess the healthy aging profile in relation to the socioeconomic characteristics evaluated at baseline. All of the tests used a p-value < 0.05. Results: the majority of the 335 participating older adults were women (62.1%), were aged between 80 and 84 years (50.4%), were widowed (53.4%), were illiterate (59.1%), had a monthly income of less than one minimum wage (59.1%), were retired (85.7%), lived with their spouse (63.8%), did not have a caregiver (60.3%), had two or more children (82.7%), and had two or more grandchildren (78.8%). The results indicate three age groups with a healthier aging profile: older adults aged 80 to 84 years (55.6%), older adults aged 85 years and older who are married (64.9%), and older adults aged 85 and older who do not have a partner or a caregiver (54.2%). Conclusion: the healthy aging profile of octogenarians can be explained by age group, marital status, and the presence of a caregiver.Item Acesso aberto (Open Access) Idosos residentes em instituições de longa permanência para idosos da região metropolitana de Belém-PA(Universidade Federal do Pará, 2012-12) POLARO, Sandra Helena Isse; FIDERALINO, Joana Cleia Trindade; NUNES, Priscila Araújo de Oliveira; FEITOSA, Elisa da Silva; GONÇALVES, Lucia Hisako TakaseDescriptive-exploratory study conducted in four long-term institutions for the elderly (ILPI), being two philanthropic and two governmental, in the metropolitan region of Belém city, state of Pará, Brazil. Objective: To describe social characteristics of low income institutionalized elderly in ILPI’s of the metropolitan region of Belém, and also identify the living conditions, health and dependence level, in basic activities of daily living. Methodolog y: the study included 165 institutionalized elderly who were limited to describe data obtained by the Elderly Handbook of the Health Ministry / SUS, evaluating independence in activities of daily living, according to Katz Index. Results: The prevalence of 62.5% unmarried elderly residents, without familiy, reinforces the need and usefulness of the ILPI as an alternative geronto-geriatric program for society. For the age group of 80 years and more (49%), among residents, care strategies for elderly focusing on vulnerability and frailty must be reviewed. Physical inactivity within the ILPI (87%) and dependence in activities of daily life (58%) point to the need for increasing daily institutional practices focusing on active ageing. The high prevalence of comorbidities (90%) and polypharmacy (71%) among residents call for specialized care in diseases treatment, control and rehabilitation, maintenance of the possible balance of living and health of those elderly. Conclusion: The long-term institution for the elderly, although being of social nature, considering the findings of this study, requires living and health care actions.Item Acesso aberto (Open Access) Implementação da estratégia de ensino-aprendizagem à família de paciente crônico(2002-02) FREITAS, Maria Célia de; SANTANA, Mary Elizabeth deThis study aims at implementing the teaching-learning strategy for families of patients in chronic conditions. It considers the importance of evaluating the needs and abilities of family members in order to prepare them to manage that situation appropriately, as well as to be able to provide adequate home care. A diagnostic evaluation of a family member/caretaker was conducted during the hospitalization of a patient with congestive cardiac insufficiency. During the diagnosis "knowledge deficit" was identified. Thus, a teaching plan was elaborated for such caretaker with the purpose of preparing him/her for home care. During implementation, formative evaluations were conducted with the purpose of learning about the efficacy of the strategies employed and the summative in the patient's home. The importance of nurses' resorting to the teaching-learning strategy in order to guide the family member/caretaker in the process of caring for people with chronic conditions was certified. The aim is promoting life, in spite of the disease, since the quality of such care will prevent aggravating recurrence and consequent re-hospitalizations.
