Assuming that Ferenczi's notion of confusion of languages could be useful for thinking about problems that arise from the coexistence of different patterns of social relationships in highly excluding societies, we emphasize different dimensions of psychic suffering, with special focus on the psycho-social dimension.Artigo de Periódico Acesso aberto (Open Access) Abandono do tratamento de tuberculose em co-infectados TB/HIV(2010-06) RODRIGUES, Ivaneide Leal Ataíde; MONTEIRO, Larissa Lima; PACHECO, Régia Hevelline Barros; SILVA, Sílvio Éder Dias daThis study aimed at analyzing the reasons that patients co-infected with tuberculosis and HIV leave the treatment of tuberculosis and to know the conduct of the health team toward that abandonment. The study, using a qualitative approach, performed semi-structured interviews on 45 professionals working at a referral health center in Pará state. Two units emerged based on the thematic analysis: patient-associated factors that make TB treatment adherence difficult; and service-associated factors that contribute to treatment abandonment. It was found that, in terms of the patients, that their low socioeconomic condition was the most common factor that led to abandonment. Other factors that led to this outcome included the adverse drug effects, the use of illegal drugs, and poor personal motivation. Regarding the service, issues related to the physical structure, working process organization and accessibility were also relevant to their non-adherence. Results show there is a need to change the practices performed at the health care services.Artigo de Periódico Acesso aberto (Open Access) Ação e raciocínio pedagógico de professoras de enfermagem: expressões em diferentes contextos educacionais(Universidade Federal do Pará, 2018-08-27) MENEGAZ, Jouhanna do Carmo; BACKES, Vânia Marli Schubert; MOYA, José Luis MedinaObjective: to report sources of knowledge base for teaching, categories of knowledge base for teaching and phases of pedagogical reasoning and action of professors of higher education in Nursing in public and private universities in the South region of Brazil. Method: t collective study of instrumental cases with a qualitative approach. The cases involve the pedagogical reasoning and action of two professors of Nursing. Data collection incorporated documents, interviews and observation analyzed using the constant comparative method. The analysis gave rise to two metacategories, termed public case and private case, and their transversal axes: relationships between sources and knowledge base for teaching, relationships between categories of knowledge base for teaching, and relationships between knowledge base for teaching and the Model of Pedagogical Reasoning and Action. Results: there are distinct relationships between sources, knowledge base for teaching and phases of the Model in the cases’ pedagogical reasoning and action. Conclusion: emphasis is placed on hierarchization and linking in the relationships between Shulman’s constructs and encouragement to reflection, and broadening of the sources of knowledge base for teaching as a source of lecturer training is suggested.Artigo de Periódico Acesso aberto (Open Access) Achados incidentais de aneurismas torácicos e abdominais(Universidade Federal do Pará, 2016-06) GÓES JUNIOR, Adenauer Marinho de Oliveira; FREIRE, Bárbara Íris Mascarenhas; RODRIGUES, Sofia Cunha; ANDRADE, Mariseth Carvalho de; FRANCO, Reinaldo Sérgio MonteiroBackground: Abdominal aortic aneurysms (AAAs) are the most common. The annual incidence of AAA rupture is eight cases per 100,000 inhabitants. Incidental detection can benefit patients if diameter is monitored and the correct treatment provided. Objectives: To estimate the prevalence of incidental diagnosis of thoracic aortic aneurysm (TAA) and AAA by computed tomography (CT); to determine prevalence by age and sex of patients and determine which arteries are involved and the morphological characteristics of the aneurysms; to determine the indications for CT most associated with incidental diagnosis of aneurysms. Methods: This was a descriptive, retrospective and randomized study. Inclusion criteria: patients over 50 years of age examined with CT of the thorax, abdomen or pelvis. Exclusion criteria: follow-up or diagnostic suspicion of aneurysms. Data collection instruments containing questions covering demographic and anatomic data were used. Results: A sample of 1,202 radiology reports was reviewed. A total of 27 aneurysms were detected (prevalence of 2.2%). Patients: 60% were male and 40% were female (p < 0.05). Sites: there were 13 cases (48.2%) in the ascending aorta (TAA); 7 (25.9%) in the infrarenal aorta (AAA); 2 (7.4%) at the thoracoabdominal transition of the aorta (TTA); 2 (7.4%) in the common iliac artery ; 1 (3.7%) in the internal iliac artery; 1 (3.7%) in the splenic artery; and 1 (3.7%) in the renal artery. Conclusions: The majority of patients were male (60%); TAA was the most common type (mean diameter: 4.1 cm), followed by AAA (mean diameter: 4.0 cm) and ATA (mean diameter: 3.9 cm). Scanning electron microscopy (SEM) images are presented to characterize the interface between adhesives and dentin. Adhesive systems have been recently classifed according to their adhesion approaches in etch-and-rinse, self-etch and glass ionomer. The etch-and-rinse systems require a specifc acid-etch procedure and may be performed in two or three steps. Self-etch systems employ acidic monomers that demineralize and impregnate dental substrates almost at the same time. These systems are separated in one or two steps. Some advantages and defciencies were noted for etch-and-rinse and self-etch approaches, mainly for the simplifed ones due to some chemical associations and interactions. The SEM micrographs illustrate different relationships between adhesive systems and dental structures, particularly dentin. The knowledge of composition, characteristics and mechanisms of adhesion of each adhesive system is of fundamental importance to permit the adoption of ideal bonding strategies under clinical conditions.Artigo de Periódico Acesso aberto (Open Access) Air-powder polishing on self-ligating brackets after clinical use: effects on debris levels(Universidade Federal do Pará, 2016-10) ARAGÓN, Mônica Lídia Castro de; LIMA, Leandro Santiago; NORMANDO, Antonio David CorrêaIntroduction: Debris buildup on brackets and arch surfaces is one of the main factors that can influence the intensity of friction between bracket and orthodontic wire. Objective: This study sought to evaluate the effect of air-powder polishing cleaning on debris levels of self-ligating ceramic brackets at the end of orthodontic treatment, compared to the behavior of conventional brackets. Methods: Debris levels were evaluated in metal conventional orthodontic brackets (n = 42) and ceramic self-ligating brackets (n = 42) on canines and premolars, arranged in pairs. There were brackets with and without air-powder polishing. At the end of orthodontic treatment, a hemiarch served as control and the contralateral hemiarch underwent prophylaxis with air-powder polishing. Debris buildup in bracket slots was assessed through images, and Wilcoxon test was used to analyze the results. Results: The median debris levels were statistically lower in the conventional metal brackets compared to self-ligating ones (p = 0.02), regarding brackets not submitted to air-powder polishing. Polishing significantly reduced debris buildup to zero in both systems, without differences between groups. Conclusions: Ceramic self-ligating brackets have a higher debris buildup in comparison to conventional metal brackets in vivo, but prophylaxis with sodium bicarbonate jet was effective in reducing debris levels in self-ligating and also in conventional brackets.Artigo de Periódico Acesso aberto (Open Access) Alcalóides de Aspidosperma auriculatum Standl(2003) BARBOSA, Wagner Luiz Ramos; TAVARES, Isabel Cecília de Castro; SOARES, Daniela CristinaSpecies from genus Aspidosperma show the presence of indol alkaloids. From Aspidosperma quebracho-blanco Schlecht quebracacidine was obtained1, from A. olivaceum. Schmutz and Hunziker2 isolated olivacine, among others substances. This work reports the determination of the chromatographic profile of alkaloid fractions from Aspidosperma auriculatum, by Thin Layer Chromatography and High Performance Liquid Chromatography. At the Brazilian state Pará this plant species has the vernacular name carapanaúba and is traditionally indicated to heal fever and others affections and also malaria. The plant material - stem bark - was extracted with hydrochloridric acid 5%. Eight individuals were treated with conventional brackets installed on the lower dental arch and self-ligating brackets on the upper arch. Another eight individuals received self-ligating brackets in the lower arch and conventional brackets in the upper arch. The subjects received material and instructions for oral hygiene. Visible plaque index (VPI), gingival bleeding index (GBI) and clinical attachment level (CAL) were evaluated just after installation of orthodontic appliances, and 30, 60 and 180 days later. Mann-Whitney test was used to compare differences between groups (self-ligating and conventional), twoway ANOVA followed by Tukey’s test was used to assess CAL at each site of each tooth. Significance level was set at 5%. Results: No significant changes were found with regard to the assessed parameters (VPI, GBI and CAL) in either one of the systems.Conclusion: No significant changes were found with regard to the periodontal response to orthodontic treatment for the variables assessed and between subjects receiving passive self-ligating and conventional brackets. All individuals had received oral hygiene instructions and had their periodontal conditions monitored.Artigo de Periódico Acesso aberto (Open Access) Análise da correlação entre a angulação (mesiodistal) dos caninos e a inclinação (vestibulolingual) dos incisivos(2011-06) OHASHI, Amanda Sayuri Cardoso; NASCIMENTO, Karen Costa Guedes do; NORMANDO, Antonio David CorrêaOBJECTIVE: To assess the degree of correlation between canine angulation and incisor inclination. METHODS: Mesiodistal angulation of canines and labiolingual inclination of incisors were obtained by means of digital graphics software (ImageTool®) from standardized photographs of the casts of 60 patients. Incisor inclination was also assessed by lateral cephalometric radiographs. RESULTS: Random error showed a variation of around 2° in measurements made on the casts (1.8-2.5), while systematic error, measured by the intraclass correlation test, displayed excellent reproducibility for both methods used in this study (p<0.001, r=0.84-0.96). Linear correlation tests revealed a significant positive correlation between canine angulation and incisor inclination in the maxillary arch (r=0.3, p<0.05) and even more significantly in the mandibular arch (r=0.46 to 0.51, p<0.001), when both were measured on the casts. When incisor inclination was examined by cephalometrics, correlation level was statistically insignificant for maxillary incisors (r=0.06 to 0.21, p>0.05) and varied widely in the mandibular arch (r=0.14 to 0.50). CONCLUSIONS: The introduction of changes in the angulation of canines with the aim of monitoring compensations observed in incisor inclination is warranted, especially in the lower arch.Artigo de Periódico Acesso aberto (Open Access) Análise de percepções e ações de cuidados bucais realizados por equipes de enfermagem em unidades de tratamento intensivo(Universidade Federal do Pará, 2009-03) ARAÚJO, Rodolfo José Gomes de; OLIVEIRA, Layla Cristine Gomes de; HANNA, Leila Maués Oliveira; CORRÊA, Adriano Maia; CARVALHO, Liliane Helena Vilar; ALVARES, Nair Carolina do Socorro FerreiraObjectives: A study was carried out, by means of a questionnaire for guided interviews seeking to establish a profile of perceptions and oral care given by ICU nursing team to patients in intensive care units. Methods: The target population consisted of nursing practitioners divided in three education categories: nurses, nursing technicians and auxiliary nurses working at public and private hospital institutions providing for intensive care unit patients in Belem-PA. Dentistry experience has developed in this field reporting scientific findings and practical applications on prevention and reestablishment of the oral health in question. Results: This survey disclosed results suggesting that oral hygiene care given to intensive care units patients is insufficient and inadequate requiring changes be made in the care now provided in the nosocomial environment by the nursing team. Conclusion: Presence of a dentist, knowledge of preventive dentistry, dissemination and use of oral hygiene specific resources are means suggested in an attempt to resolve difficulties found in oral health maintenance and treatment of oral diseases that affect the general health of hospitalized individuals. B. Verlt. is a Bignoniaceae widely used in the popular medicine as anti-inflammatory and astringent agent, and for some illnesses as intestinal colic, diarrhoeas, anaemia and diseases of the skin. Its biological properties and dye biosynthesis lead the species to be used in the cosmetic industry. The use of natural products of vegetal origin implies in pharmacognostic quality control and purity assays that compose the technical specifications of the material. Therefore, the anatomical description of mature and young leaves of A. chica was carried through the use of optic microscopy on histological preparations. The leaves are hipostomatic and dorsiventral with heterogeneous mesophile. In petiole, the epidermis is unistratified, it contains trachoma and it is endowed with thin cuticle. The pharmacopoeia assays included the determination of the grain size distribution of the plant drug, the determination of humidity and the total ashes, beyond the phytochemistry approach of tincture, to establish parameters for its quality control.Artigo de Periódico Acesso aberto (Open Access) Analysis of imatinib adherence in chronic myeloid leukemia: a retrospective study in a referral hospital in the brazilian amazon(Associação Brasileira de Hematologia, Hemoterapia e Terapia Celular, 2019-06) ANDRADE, Alan Rodrigues; PAZ, Igor Penha; EVANGELISTA, Talitta Ribeiro; MELLO, Vanessa Joia de; HAMOY, Moisés; LEITÃO, Daniel da SilvaBackground: There has been a revolution in the treatment of Chronic Myeloid Leukemia since imatinib's introduction. However, patient adherence has a great impact on the response obtained with medical treatment. This study's objective was to analyze the drug adherence and the factors that influenced it in patients with Chronic Myeloid Leukemia in a referral hospital in the Brazilian Amazon. Method: This was a retrospective study including 120 patients with Chronic Myeloid Leukemia from January 2002 to December 2014. The adherence was estimated by the Proportion of Days Covered and the persistence by Kaplan-Meier analysis. The data was analyzed in Epi Info 7® software and the relationship between the variables was analyzed by Fisher's exact test. A p-value lower than 0.05 was considered significant. Results: Twenty-seven patients (22.5%) were considered non-adherent. There has been irregular medication use and disinterest in the treatment in 20.83% (n = 25), of which 13 were considered non-adherent (p < 0.001). A total of 26.67% (n = 32) abandoned the treatment for a period. Of those, 56.25% (n = 18) were non-adherent (p < 0.001). Distance to the hospital, lack of medication and side-effects were all non-significant to low adherence. At the end of a 360-day follow-up, 44.16% (n = 53) of patients presented a break in persistence, whose average was 255 days. Conclusion: The adherence found in this study was similar to that found in others of its kind. The only factors that negatively influenced the adherence were disinterest and abandonment of treatment, which can reflect the need to individually educate Chronic Myeloid Leukemia patients.Artigo de Periódico Acesso aberto (Open Access) Analysis of the mental foramen using cone beam computerized tomography(Universidade Federal do Pará, 2015-08) SAITO, Kunihiro; ARAÚJO, Ney Soares de; SAITO, Miki Taketomi; PINHEIRO, João de Jesus Viana; CARVALHO, Pedro Luiz deIntroduction: Knowledge of the anatomical structures located in the region between the mental foramina is of critical importance in pre-operative planning. Objective: To evaluate the position of the mental foramen relative to the apices of the teeth and the distance to the edges of the mandible, using cone beam computerized tomography. Material and method: One hundred cone beam computerized tomographs of the mandible were evaluated; the tomographs were taken using a single tomographic device. Each image chosen was evaluated repeatedly from both sides of the mandible, the position of the mental foramen, indicating the region in which the foramen was found and the measures of the mental foramen, the lingual cortex and the mandibular base. Initially, the data were analyzed descriptively. A value of p<0.05 was chosen as the level of significance. Result: Forty-two percent of the mental foramina were located in the apex of the second pre-molar. The lingual margin of the mental foramen was located, on average, 3.1mm from the lingual cortex. The lower margin of the mental foramen was located 7.25 mm above the lower edge of the mandible. Conclusion: The mental foramen was located more frequently at a level of the apices of the second pre-molars, with a distance to the lingual cortex, on average, of 3.1mm and to the base of the mandible, on average, of 7.25 mm.Artigo de Periódico Acesso aberto (Open Access) Anatomopathological aspects of acute Chagas myocarditis by oral transmission(Sociedade Brasileira de Cardiologia, 2016-07) SOUZA, Dilma do Socorro Moraes de; ARAUJO, Marialva Tereza Ferreira de; SANTOS, Paulo Roberto Silva Garcez dos; FURTADO JUNIOR, Julio Cesar Branco; FIGUEIREDO, Maria Tereza Sanches; PÓVOA, Rui Manuel dos SantosVector transmission of Trypanosoma cruzi has declined in Latinpt_B America, which has been attributed to better epidemiological control of this form of transmission, estimated at 8 to 10 million chronic cases, in addition to reducing the number of new cases.1,2 However, there has been an increase in the incidence of acute cases, predominantly by oral transmission due to the ingestion of food contaminated with feces of triatomids, both in isolated cases and in family micro-outbreaks.3 Necropsy studies that describe myocarditis in the acute phase of Chagas' disease are scarce and the existing reports in the literature are of studies carried out in the past decades and involve vector transmission in endemic areas. Cardiac involvement in the acute phase may have varied aspects, especially in relation to myocardial lesion, from an undetectable one, to the evolution to acute heart failure with severe myocarditis and death. METHODS: A repeated cross-sectional panel study of public health care centers of municipalities in the five Brazilian regions was conducted. Retrospective data were obtained from 12,119 medical records of pregnant women distributed in two groups: before fortifi cation (delivery prior to June 2004) and after fortifi cation (date of last period after June 2005). Anemia was defi ned as Hb<11.0 g/dl. Hb levels according to gestational age were assessed using two references from the literature. Statistical analysis was carried out using chi-squared tests, Student’s t tests, and logistic regression, with a signifi cance level of 5%. RESULTS: In the total sample, prevalence of anemia fell from 25% to 20% after fortifi cation (p<0.001). However, important regional differences were evident: while signifi cant reductions were seen in the Northeast (37% to 29%) and North (32% to 25%) regions, where pre-fortifi cation prevalence was high, smaller reductions were seen in the Southeast (18% to 15%) and South (7% to 6%) regions, where prevalence was low. Hb levels according to gestational age were slightly higher in the fi rst months of pregnancy and lower after the third or fourth months, depending on the reference used. Logistic regression analysis showed that group, geographic region, marital status, trimester of pregnancy, initial nutritional status, and prior pregnancy were associated with anemia (p<0.05). CONCLUSIONS: Prevalence of anemia decreased after fortifi cation, but remains high in the North and Northeast regions. Although fortifi cation may have played a role in this favorable outcome, the contribution of other public policies implemented during the studied period should also be considered.Artigo de Periódico Acesso aberto (Open Access) Angulação dos caninos em indivíduos portadores de má oclusão de Classe I e de Classe III: análise comparativa através de um novo método utilizando imagens digitalizadas(2010-10) AZEVEDO, Lucyana Ramos; TORRES, Tatiane Barbosa; NORMANDO, Antonio David CorrêaOBJECTIVES: This study aimed to determine the mesiodistal angulation of canine crowns in individuals with Class III malocclusion in comparison with Class I individuals. METHODS: Measurements were taken from digital photographs of plaster models and imported into an imaging program (Image Tool). These procedures were repeated to assess random method error (Dahlberg's formula), and analyze reproducibility by intraclass correlation. The sample consisted of 57 patients with complete permanent dentition, untreated orthodontically and divided into two groups according to their malocclusion: Group I consisted of 33 patients with Class I malocclusion, 16 males and 17 females, mean age 27 years; Group II comprised 24 patients with Class III malocclusion, 20 males and 4 females, mean age 22 years. RESULTS: Random error for canine angulation ranged from 1.54 to 1.96 degrees. Statistical analysis showed that the method presented an excellent reproducibility (p<0.01). Results for canine crown angulation showed no statistically significant difference between maxillary canines in the Class I and Class III groups, although canine angulation exhibited, on average, 2 degrees greater angulation in Class III individuals. Mandibular canines, however, displayed a statistically significant difference on both sides between Class I and Class III groups (p = 0.0009 and p = 0.0074). Compared with Class I patients, angulation in Class III patients was lower in mandibular canines and tended to follow the natural course of dentoalveolar compensation, routinely described in the literature. Male Wistar rats were treated with H. brasiliense extract (50, 250 and 500 mg/kg, p.o.) in 3% Tween 80 0.9% saline solution. The treatment of the edema induced by carrageenin with HBSE (500 mg/kg) showed significant inhibition when compared to the control group. At this dose, the edema decreased by 31.25% in the third hour after treatment (edema peak), but the dose of 50 mg/kg has inhibited the edema by 53.13% (p < 0.05). At the dose of 50 mg/kg, the decrease of the edema induced by dextran was similar to that caused by cyproheptadine. The decrease of the formation of granulomatous tissue (6.6%) was comparable to the control group. The HBSE inhibited the abdominal constrictions induced by acetic acid. At a dose of 50 mg/kg, the inhibition of the abdominal constrictions (46.4%) was comparable to that produced by indomethacin (42.9%). A dose of 250 mg/kg inhibited these constrictions by 70.66% when compared to control (p < 0.001). In the hot-plate test, an increase in the latency time was observed at a 50 mg/kg dose. These data suggest that HBSE has anti-inflammatory activity on acute process, developed principally by arachdonic acid derivates and analgesic effect due to its probable involvement in the Central Nervous System.
