    Clinical aspects in patients with pulmonary infection caused by mycobacteria of the Mycobacterium abscessus complex, in the Brazilian Amazon

    Objective: To describe the clinical manifestations of patients with pulmonary infection caused by mycobacteria of the Mycobacterium abscessus complex (MABSC), and to compare these manifestations with those of patients infected with other nontuberculous mycobacteria (NTM). Methods: This was a retrospective cohort study involving 43 patients divided into two groups: the MABSC group, consisting of patients with pulmonary infection caused by MABSC (n = 17); and the NTM group, consisting of patients with pulmonary infection caused by NTM other than MABSC (n = 26). Patients were previously treated with a regimen of rifampin, isoniazid, pyrazinamide, and ethambutol before the diagnosis of NTM was confirmed by two culture-positive sputum samples. The nucleotide sequences of the hsp65, 16S rRNA, and/or rpoB genes were analyzed to identify the mycobacteria. Data were collected on demographic, clinical, and radiological characteristics, as well as on treatment responses and outcomes. Results: Loss of appetite was the only clinical manifestation that was significantly more common in the MABSC group than in the NTM group (p = 0.0306). The chance of having to use a second treatment regimen was almost 12 times higher in the MABSC group than in the NTM group. Treatment success was significantly higher in the NTM group than in the MABSC group (83.2% vs. 17.6%; p < 0.0001). The chance of recurrence was approximately 37 times higher in the MABSC group than in the NTM group. Conclusions: In the study sample, treatment response of pulmonary disease caused by MABSC was less favorable than that of pulmonary disease caused by other NTM.

    Implicações da vivência de prisão preventiva por violência conjugal: narrativas masculinas

    This study explored the implications of the experience of preventive detention drawing on the oral histories of men being prosecuted for intimate partner violence. The oral history of 11 men living in Belém, Brazil being prosecuted for intimate partner violence and who had experienced preventive detention were collected using semi-structured interviews conducted between June and July 2015. The data was organized and categorized using thematic content analysis. The male narratives revealed physical and psychological implications and impaired social interaction, expressed by family exclusion, financial problems, and difficulty in finding employment. The analysis of the implications of experiences of preventive detention provided valuable insights into the importance of intersectorality, the creation of reflective spaces, and discussions in the field of gender inequality.

    Obstáculos para o encontro pedagógico entre professores e estudantes de enfermagem em diferentes contextos educacionais

    In teaching, the teacher rationalizes and acts pedagogically, with a view to transforming the disciplinary content into learning. However, in an educational context, rather than by the teacher's intention, learning is produced by the capacity to establish a dialogical relationship between teachers and students. With this in view, this collective study of cases with a qualitative approach has the objective of analyzing obstacles to the pedagogical encounter between teachers and students of nursing of public and private university of the South Region of Brazil. Two university nursing professors and the students of the disciplines taught by them participated in the study. Data collection took place between April 2014 and July 2015 and involved documents, interviews and observation, outlined in phases guided by Shulman's constructs. The data were analyzed using the Strauss and Corbin method, and the results are articulated in the metacategories: pedagogical action and reasoning as an obstacle; understanding and attitude of the student as an obstacle;and tactics of denial of obstacles. The phases of comprehension, transformation and teaching of the action and pedagogical reasoning of the teachers are pointed as obstacles; understanding and expectations of students; attitudes of both before contraries. It is suggested the sharing of expectations, intentions and more candid evaluations between teachers and students to strengthen the pedagogical encounter.

    Caracterização da estratégia saúde da familía no estado do Amazonas, Brasil: análise da implantação e impacto

    This article focuses on the implementation of the Family Health Program (FHP) in municipalities (counties) in the State of Amazonas, Brazil, analyzing potential population coverage, evidence of change in the health care model, and impact (2004 and 2008). Implementation was classified as fair versus unsatisfactory. The findings showed an increase in the number of municipalities with the FHP implemented, as well as in the program's coverage (which nevertheless was less than 50% of the population). Concerning change in the health care model, no municipalities were observed with a high rate of change. As for impact, there was a decrease in low impact and an increase in high impact. The final classification did not show a statistically significant change from one year to the next, and unsatisfactory implementation prevailed in the municipalities of the State of Amazonas. Evaluation of the FHP is not simple, since there are various factors involved in the issue. Further studies could reveal the factors that explain the program's suboptimal implementation in the State of Amazonas.

    Aumento das cargas de trabalho em técnicos de enfermagem na atenção primária à saúde no Brasil

    In this article we analyze the main aspects that contribute to increase the workloads of nurse technicians working in primary health care in Brazil, we identify the type of loads and rank the most prevalent aspects of this occupational group. It is based on a multicenter study with 24 professionals from the South, Center-west and North regions of Brazil, from 2011 to 2013, using data from semi-structured interviews. The data were systematized in Atlas.ti software and submitted to thematic analysis. The aspects that contribute to increasing workloads are low monthly remuneration, lack of material resources, work overload, wear and illness, relationship with the user, and lack of human resources, prevailing psychological and physiological loads. Adequacy of the conditions and organization of work are shown paramount to ease the workload on the primary health care setting, and to promote the health, wellbeing and quality of life in the nurse technicians working environment.

    Enfermeiras desafiando a violência no âmbito de atuação da estratégia de saúde da família

    This study aimed to describe and analyze how violence affects the working process of nurses in the Family Health Program. It consists of an exploratory-descriptive study developed in a district in the suburb of Belém, Pará, Brazil. Data were collected between August 2009 and February 2010, by means of interviews with 14 nurses, and then submitted to the method of content analysis, generating one of the themes of this study: 'Challenging violence', with derivative sub-topics such as territorial violence, institutional violence and intrafamilial violence. The result showed the impact of the violence phenomenon on the working process of the nurses of the Family Health Program units. This violence induces them to feel fear and professional frustration due to embarrassment and limitation of their roles, although they still challenge the obstacles encountered in their daily work.

    Ação e raciocínio pedagógico de professoras de enfermagem: expressões em diferentes contextos educacionais

    Objective: to report sources of knowledge base for teaching, categories of knowledge base for teaching and phases of pedagogical reasoning and action of professors of higher education in Nursing in public and private universities in the South region of Brazil. Method: t collective study of instrumental cases with a qualitative approach. The cases involve the pedagogical reasoning and action of two professors of Nursing. Data collection incorporated documents, interviews and observation analyzed using the constant comparative method. The analysis gave rise to two metacategories, termed public case and private case, and their transversal axes: relationships between sources and knowledge base for teaching, relationships between categories of knowledge base for teaching, and relationships between knowledge base for teaching and the Model of Pedagogical Reasoning and Action. Results: there are distinct relationships between sources, knowledge base for teaching and phases of the Model in the cases’ pedagogical reasoning and action. Conclusion: emphasis is placed on hierarchization and linking in the relationships between Shulman’s constructs and encouragement to reflection, and broadening of the sources of knowledge base for teaching as a source of lecturer training is suggested.

    Fatores relacionados ao aleitamento materno exclusivo em bebês nascidos em hospital amigo da criança em uma capital do Norte brasileiro

    The objective of this study was to know the factors related to Exclusive Breastfeeding in Infants born in a child-friendly hospital in the city of Belém, Pará State capital, and to verify whether the scores of the Self-Efficacy Scale in Breastfeeding were among those factors. We studied 414 mother-baby binomials which babies were born in 2016. Socioeconomic and obstetrical questionnaires applied the Self-Efficacy Scale in Breastfeeding to mothers at their hospital discharge. The breastfeeding score mean was 61 points; 32.45% of the mothers were between 19 and 23 years old; 36.56% of them had a high school diploma; 43.34% received up to a minimum wage; 62.95% lived an informal union with their partners; and 17.92% were employed. The factors related to exclusive breastfeeding involved the mother age and schooling, the fact that the mother is employed, her marital status, the partner incentive to breastfeeding, and the self-efficacy scale score.

    Nursing workloads in family health: implications for universal access

    Objective: to identify the workloads of nursing professionals of the Family Health Strategy, considering its implications for the effectiveness of universal access. Method: qualitative study with nursing professionals of the Family Health Strategy of the South, Central West and North regions of Brazil, using methodological triangulation. For the analysis, resources of the Atlas.ti software and Thematic Content Analysis were associated; and the data were interpreted based on the labor process and workloads as theorical approaches. Results: the way of working in the Family Health Strategy has predominantly resulted in an increase in the workloads of the nursing professionals, with emphasis on the work overload, excess of demand, problems in the physical infrastructure of the units and failures in the care network, which hinders its effectiveness as a preferred strategy to achieve universal access to health. On the other hand, teamwork, affinity for the work performed, bond with the user, and effectiveness of the assistance contributed to reduce their workloads. Conclusions: investments on elements that reduce the nursing workloads, such as changes in working conditions and management, can contribute to the effectiveness of the Family Health Strategy and achieving the goal of universal access to health.

    Healthy aging profile in octogenarians in Brazil

    Objective: to identify the healthy aging profile in octogenarians in Brazil. Method: this population-based epidemiological study was conducted using household interviews of 335 octogenarians in a Brazilian municipality. The decision-tree model was used to assess the healthy aging profile in relation to the socioeconomic characteristics evaluated at baseline. All of the tests used a p-value < 0.05. Results: the majority of the 335 participating older adults were women (62.1%), were aged between 80 and 84 years (50.4%), were widowed (53.4%), were illiterate (59.1%), had a monthly income of less than one minimum wage (59.1%), were retired (85.7%), lived with their spouse (63.8%), did not have a caregiver (60.3%), had two or more children (82.7%), and had two or more grandchildren (78.8%). The results indicate three age groups with a healthier aging profile: older adults aged 80 to 84 years (55.6%), older adults aged 85 years and older who are married (64.9%), and older adults aged 85 and older who do not have a partner or a caregiver (54.2%). Conclusion: the healthy aging profile of octogenarians can be explained by age group, marital status, and the presence of a caregiver.

    A Implicação do apoio social no viver de pessoas com hipertensão arterial

    This study aimed to know the types of support offered by the network of social support and implications on hypertensive life. Qualitative study whose methodology was based on Grounded Theory. 35 people were interviewed between October 2008 and August 2010, 22 hypertensive compulsorily enrolled in HIPERDIA at a Health Unit in Belém / PA, 5 family members, 5 health professionals and 3 representatives of community institutions, referenced by hypertensive patients, as members of the network support. Data were subjected to coding procedures: analysis, comparison and categorization. The category "Identifying the types of social support offered to people in their living with hypertension" was the Intervening Condition of the Theory. The types of emotional, informational and instrumental support originated mainly from family relationships and may mean alternative treatments, featuring a care focused on people with hypertension and their network of relationships which requires attention from healthcare professionals, including nurses.

    The starting point of this work consists on a revision of thesis and dissertations developed by nurses aiming to check how the social representation was used as a theoretical referencial on these researches. Through a sistematic reading of these works some points were dettached, as thematic, main resultas and reflections proposed in order to atain professional practice nursing. It was ascertained from the analysed works that this theoretical referencial is used by nurses, and that some  studies developed introduce significative results from these professionals.

    Vivências de mulheres cuidadoras de pessoas idosas dependentes: orientação de gênero para o cuidado

    Objective: To understand, in the memories of caregiving women, the meaning of the experiences of caregiving relationships in family units with dependent elderly people in the context of social construction of care - oriented gender identity. Methods: This is a study based on the oral history of life of six female caregivers of families with elderly dependent people, in Bahia countryside, Brazil. The memories, captured by recorded and transcribed interview, composed the corpus of analysis. Results: The social role of gender identity directs women to the responsibility of their family members who are dependent on care, with feelings and values of human obligation and strong religious influence. This care also favors intergenerational experiences in the space of family relationships with meaningful learning. Conclusion: A critical understanding of the female role of care is urgently needed, with an incentive for intergenerational and gender education that provides new perspectives for dependent elderly care

    Idosos residentes em instituições de longa permanência para idosos da região metropolitana de Belém-PA

    Descriptive-exploratory study conducted in four long-term institutions for the elderly (ILPI), being two philanthropic and two governmental, in the metropolitan region of Belém city, state of Pará, Brazil. Objective: To describe social characteristics of low income institutionalized elderly in ILPI’s of the metropolitan region of Belém, and also identify the living conditions, health and dependence level, in basic activities of daily living. Methodolog y: the study included 165 institutionalized elderly who were limited to describe data obtained by the Elderly Handbook of the Health Ministry / SUS, evaluating independence in activities of daily living, according to Katz Index. Results: The prevalence of 62.5% unmarried elderly residents, without familiy, reinforces the need and usefulness of the ILPI as an alternative geronto-geriatric program for society. For the age group of 80 years and more (49%), among residents, care strategies for elderly focusing on vulnerability and frailty must be reviewed. Physical inactivity within the ILPI (87%) and dependence in activities of daily life (58%) point to the need for increasing daily institutional practices focusing on active ageing. The high prevalence of comorbidities (90%) and polypharmacy (71%) among residents call for specialized care in diseases treatment, control and rehabilitation, maintenance of the possible balance of living and health of those elderly. Conclusion: The long-term institution for the elderly, although being of social nature, considering the findings of this study, requires living and health care actions.

    In this study we emphasise the personality caractheristics, tryingto amplify the mental functioning matter facing the richness of a work based on a tale. We aim to worth the mental functioning of each dwarf as a personality envolved on na emocional drama, exploded by the jelousy of the stepmother about the youth and beauty teeanage stepdaugther. On the other hand, we expect this theme can be used as didatic resourse about the mental functioning, serving as a practical, simple and dinamic guide for the professor in classroom, as for Psychiatric Nursing as for Medicine ones, specializing on Psychiatry, in their initial learnig phase.

    Dinâmica da família no contexto dos cuidados a adultos na quarta idade

    This study aimed to determine the pattern of family functioning on everyday care relationships of adults in the fourth age. This is a study of diagnostic-evaluative nature of adults with 80 or more years old who depend on care, and of their relatives as caregivers. The participants were selected among the registered patients of a Family Health Unit in a district in the suburbs of Belém-PA, Brazil. They were evaluated according to the dynamics of their family, and quality of life related health lifestyle. Most of the elderly rated their families with good functionality. However, data on the elderly and caregivers' quality of life and caregivers' life style only reached the median level, showing some difficulty in the family functioning system. It was concluded that the multiple results obtained through the assessments indicate some practical implications of care to the family unity and confirm the need for multidimensional assessment about the family intervention.
