Item Acesso aberto (Open Access) Determinação de mercúrio total em amostras de água, sedimento e sólidos em suspensão de corpos aquáticos por espectrofotometria de absorção atômica com gerador de vapor a frio(1996-06) VIEIRA, José Luiz Fernandes; PASSARELLI, Miriam MeyerThe use of metallic mercury in the extraction and concentration of gold causes the discarding of tons of this metal in the environment, leading to a considerable increase in the natural levels of the same and the contamination of the surrounding areas. Thus it is extremely important to monitor the presence of this metal in various sectors of the environment with a view aiming to previnting human exposure to excessive concentrations which can result in serious episodes of mercury poisoning. It is also important to estimate the possibility of river sediments becoming potential sources of contamination of human beings. The determination of total mercury was undertaken by using cold vapor atomic absorption spectrometry. River waters, as well as sediments and suspended solids were used as samples for the standardization of the analytical procedure. O presente artigo apresenta diferentes concepções sobre o processo saúde-doença; além de analisar as etapas de implantação de projetos hidrelétricos ressaltando alguns riscos para a saúde da população. SOURCES OF DATA: search of Medline and Lilacs databases; publications of scientific committees of institutions for children with special needs; and outpatient care reports of reference centers for the treatment of children and adolescents with mental and developmental disabilities. SUMMARY OF THE FINDINGS: this population presents health problems associated with the basic pathology and its consequences in addition to the usual problems of the specific age group. The etiology of developmental disorders and the main characteristics of each type of disability are discussed. Special attention is given to outpatient pediatric care. CONCLUSIONS: the most recent study of the Brazilian population, performed in 2000, showed that 14.5% of this population presented some type of developmental deficiency. Therefore, developmental problems are one of the most prevalent health problems among children and adolescents. Consequently, pediatricians need to be prepared to evaluate and identify factors that may influence normal children development. Pediatricians are responsible for prevention, early diagnosis and coordination of the multidisciplinary treatment of these patients in addition to basic medical assistance, which is essential to guarantee the patients' good quality of life.Item Acesso aberto (Open Access) Avaliação das práticas e conhecimentos de profissionais da atenção primária à saúde sobre vigilância do desenvolvimento infantil(2003-12) FIGUEIRAS, Amira Consuêlo de Melo; PUCCINI, Rosana Fiorini; SILVA, Edina Mariko Koga da; PEDROMÔNICO, Márcia Regina MarcondesIn the present study, the practices and knowledge of 40 physicians and 40 nurses from municipal health care units (UMS) and 40 physicians and 40 nurses from the Family Health Program (FHP) in Belém, Pará State, Brazil, all of whom working in primary health care, were evaluated in relation to child development surveillance. Measures of knowledge of child development showed an average of 63.7% correct answers for UMS physicians, 57.3% for FHP physicians, 62.1% for FHP nurses, and 54.3% for UMS nurses. Only 21.8% of mothers attending appointments mentioned that the health care professional had asked about their children s development, 27.6% of mothers reported that the health care professional had asked about or observed the child s development, and 14.4% mothers reported having received instructions on how to stimulate their children s development. According to this study, primary health care physicians and nurses in the municipality of Belém showed gaps in their knowledge of child development. Child development surveillance is not being conducted satisfactorily in primary health care in the municipality of Belém. It is thus necessary to raise the awareness of health care professionals concerning the problem and provide them with appropriate training.Item Acesso aberto (Open Access) Lipid profile and cardiovascular risk in two Amazonian populations(2003-12) FEIO, Claudine Maria Alves; FONSECA, Francisco Antonio Helfenstein; RÊGO, Simone Soares; FEIO, Max Nazareno Barra; ELIAS, Maria Cristina; COSTA, Eduardo Augusto da Silva; IZAR, Maria Cristina de Oliveira; PAOLA, Ângelo Amato Vincenzo de; CARVALHO, Antônio Carlos de CamargoOBJECTIVE: To compare the lipid profiles and coronary heart disease risks of 2 Brazilian Amazonian populations as follows: a riverside population (village of Vigia) and an urban population (city of Belém in the state of Pará). METHODS: Fifty individuals controlled for age and sex were assessed in each region, and the major risk factors for coronary heart disease were analyzed. RESULTS: According to the National Cholesterol Education Program (NCEP III) and using the Framingham score, both populations had the same absolute risk of events (Vigia = 5.4 ± 1 vs Belém = 5.7 ± 1), although the population of Vigia had a lower consumption of saturated fat (P<0.0001), a greater consumption of mono- and polyunsaturated fat (P<0.03), in addition to lower values for body mass index (25.4± 0.6 vs 27.6 ± 0.7 kg/m², P<0.02), of biceps skin fold (18.6 ± 1.1 vs 27.5 ± 1.3 mm, P<0.0001), of triceps skin fold (28.7 ± 1.2 vs 37.3 ± 1.7 mm, P<0.002), and of total cholesterol (205 ± 5 vs 223 ± 6 mg/dL, P< 0.03) and triglycerides (119 ± 9 vs 177 ± 18 mg/dL, P<0.005). Both populations did not differ in regard to HDL-C (46 ± 1 vs 46 ± 1 mg/dL), LDL-C (135 ± 4 vs 144 ± 5 mg/dL) and blood pressure (SBP 124 ± 3 vs 128 ± 3 mmHg; DBP 80 ± 2 vs 82 ± 2 mmHg). CONCLUSION: The riverside and urban populations of Amazonia had similar cardiovascular risks. However, the marked difference in the variables studied suggests that different strategies of prevention should be applied.Item Acesso aberto (Open Access) Influências ambientais na saúde mental da criança(2004-04) HALPERN, Ricardo; FIGUEIRAS, Amira Consuêlo de MeloObjective: To present an up-to-date review about environmental influences on child mental health, describing major risk factors and discussing recommendations for intervention by pediatricians. Sources of data: MEDLINE, PsycLIT and Lilacs, technical books and publications about child development and child and adolescent mental health problems and health promotion. Summary of the findings: Children are exposed to multiple risk factors, among them high prevalence of disease, premature birth, being born from a problematic pregnancy, and living in poverty. This negative chain of events increases the risk for emotional problems. The negative effects on development and behavior result from the complex interaction between genetic, biological, psychological and environmental risk factors. The main factors influencing the mental health of children are the social and psychological environment. The cumulative risk effect is more important in determining emotional problems in children than the presence of one single stressor, regardless of its magnitude. Conclusion: Environmental factors play an important role in the etiology of emotional problems in childhood. An adequate clinical investigation allows pediatricians to identify risk factors for the development of mental health problems and to ensure early intervention for children at risk.Item Acesso aberto (Open Access) Antropometria de escolares ao ingresso no ensino fundamental na cidade de Belém, Pará, 2001(Universidade Federal do Pará, 2006-03) NEVES, Olga Maria Domingues das; BRASIL, Anne Lise Dias; BRASIL, Laélia Maria Barra Feio; TADDEI, José Augusto de Aguiar CarrazedoObjectives: to perform anthropometric measurements in elementary school students of the public school system of the State of Belém, Pará, as a tool for nutritional surveillance and to study associations between nutritional status and biological, social and economic characteristics. Methods: 793 students were assessed for their nutritional status, by z scores of height/age and weight /height according to the World Health Organization criteria. Biological, social and economic variables were: gender, age, place of birth, age of school enrolment and maternal education level. The chi square test was used to relate nutritional status to variables. Results: of the 637 students assessed, 16.6% were of low stature or in risk of low stature, 4.5% malnourished or in risk of malnourishment, 7.4% were overweight or obese. There was no significant association between nutritional status and gender. The majority of the children was born in the capital, was enrolled late in school and had mothers with education level of >4 years. There was a significant correlation between obesity and low stature with low maternal education level; malnutrition risk and low stature with late school enrolment; and obesity and normal stature with regular enrollment. Conclusions: low maternal school level and late school enrollment are associated to nutritional problems. School nutritional surveillance is important to assess, treat and prevent children's health and nutrition risks.Item Acesso aberto (Open Access) Quinine levels in patients with uncomplicated falciparum malaria in the Amazon region of Brazil(2008-10) VIEIRA, José Luiz Fernandes; BORGES, Larissa Maria Guimarães; NASCIMENTO, Margareth Tavares Silva; GOMES, Andreza de Lourdes SouzaWe examined the plasmatic concentrations of quinine in patients with uncomplicated falciparum malaria in an endemic area of the Amazon region in Brazil in a prospective clinical trial, in which a standard three-day course of oral quinine plus doxycycline was used. We measured the quinine in the plasma samples on days 0 and 3by high performance liquid chromatography. The mean concentration of quinine was 6.04 ±2.21 µg/mL in male patients and 5.98 ±1.95 µg/mL in female patients. No significant differences in quinine concentration were observed between these two groups. All samples collected before starting treatment were negative for quinine. This information could help in the development of strategies for the rational use of antimalarial drugs in Brazil.Item Acesso aberto (Open Access) Principais queixas vocais de pacientes idosos pós-tratamento para hanseníase(Sociedade Brasileira de Geologia, 2010-04) PALHETA NETO, Francisco Xavier; SILVA FILHO, Manoel da; PANTOJA JUNIOR, José Mariano Soriano; TEIXEIRA, Larissa Lane Cardoso; MIRANDA, Rafaela Vale de; PALHETA, Angélica Cristina PezzinLeprosy is an infectious disease, with vocal involvement varying between hoarseness and difficult breathing. Aim: compare the main vocal complaints among elderly patients after treatment for leprosy and a control group. Study design: descriptive prospective. Materials and methods: We included 50 patients aged over 60 years, 32 had been treated for leprosy, and the others formed the control group. We used our own questionnaire to analyze the vocal symptoms presented by the two groups, as well as gender, age, life style and comorbidities. Results: among the treated group, the most frequent symptoms were hawk (34.4%) and hoarseness (28.1%), while in the control group the most prevalent symptoms were hoarseness (77.8%) and a foreign body sensation (55.6%). Conclusion: the most prevalent voice complaints in patients treated for leprosy are hawking and hoarseness, and that its development is influenced by life style and associated diseases.Item Acesso aberto (Open Access) Manifestações retinianas em pacientes portadores de anemia de células falciformes(Universidade Federal do Pará, 2011-10) ALMEIDA SOBRINHO, Edmundo Frota de; SARAIVA FILHO, João Carlos de Pina; SILVA, James Nunes da; SILVA, Ana Paula Siqueira da; LIMA, Rafaela Cavalcante deThe World Health Organization counts that more than 5% of the world's population carry some type of hemoglobinopathy. Among them we find the sickle cell aneamia that presents its main harmful effect on the peripheral vasculature. In the retina, the falciform lesions have a well defined physiopathology and classification. To identify the retinal manifestations caused by the falciform aneamia in patients directed to the Bettina Ferro de Souza Hospital from the Hemocenter of the State of Pará – HEMOPA, in the months of September through December of 2006. METHODS: Ophthalmologic examination was performed in the Department of Ophthalmology of the Bettina Ferro de Souza Hospital. and fifty patients with sickle cell aneamia (SS or SC) followed by the department of falciform aneamia of the Hematologia e Hemoterapia do Pará - HEMOPA have been randomly selected and submitted to interview in order to register their: sex; age; color; genotype; ocular alterations; medicines taken. Ophthalmologic examination was performed in the Department of Ophthalmology of the Bettina Ferro de Souza Hospital. It consisted of: evaluation of the acuity of vision with and without correction, indirect biomicroscopy, indirect binocular ophthalmoscopy, and, in case the latter presented alterations, a complementary study witangiofluoresceinography would be carried through. RESULTS: The outcomes have been registered in research protocol and later submitted to the statistic analysis using the Qui-square test, adopting, as level of significance, p<0,05. Eighty-eight percent of the patients did not present any falciform retinal sign, 3% presented peripheral vascular occlusion; in 2%, pigmented plate was shown, and 7% presented injuries which were not compatible with the falciform disease; there was a proportionality of 50% for males and females; there was a higher predominance (38%) of people aged from 11 and 15; 74% had SS genotype and 26% SC genotype; in relation to the use of medicines there was a higher prevalence of ocular alterations in patients who had made use of the folic acid isolated, with 5%, in contrast with those who had used the hidroxyurea association and folic acid, when all (27%) presented normal fundoscopic examination; all the patients (29%) that showed fetal hemoglobin rate above 10% had retina without alterations. CONCLUSION: Few cases of retinal signs have been observed in the studied group, however this research does not diminish the importance of early and periodic ophthalmologic examination, since falciform retinopathy is largely registered and its complications can lead to amaurosis.Item Acesso aberto (Open Access) Oral live attenuated human rotavirus vaccine (RotarixTM) offers sustained high protection against severe G9P[8] rotavirus gastroenteritis during the first two years of life in Brazilian children(2012-11) JUSTINO, Maria Cleonice Aguiar; ARAÚJO, Eliete da Cunha; VAN DOORN, Leen-Jan; OLIVEIRA, Consuelo Silva de; GABBAY, Yvone Benchimol; MASCARENHAS, Joana D'Arc Pereira; MIRANDA, Yllen Stefania Affonso; GUERRA, Sylvia de Fátima dos Santos; SILVA, Veronilce Borges da; LINHARES, Alexandre da CostaIn a large Phase III trial conducted in 10 Latin American countries, the safety and efficacy of the live attenuated monovalent rotavirus vaccine RIX4414 was evaluated in 15,183 healthy infants followed up during the first two years of life. Belém was the only site in Brazil included in this multicentre trial. The study in Belém included a subset of 653 infants who were followed up until 24 months of age for protection against severe rotavirus gastroenteritis. These subjects were randomly assigned in a 1:1 ratio to receive two doses of vaccine (n = 328) or two doses of placebo (n = 325) at approximately two and four months of age. Of the 653 enrolled infants, 23 dropped out during the study period. For the combined two-year period, the efficacy of RIX4414 was 72.3% [95% confidence interval (CI) 37.5-89.1%] against severe rotavirus-related gastroenteritis, reaching a protection rate of 81.8% (95% CI 36.4-96.6%) against circulating wild-type G9 rotavirus strains. It is concluded that two doses of RIX4414 are highly efficacious against severe rotavirus gastroenteritis in Belém during the first two years of life and provide high protection against the worldwide emergence and spread of G9P[8] strains.Item Acesso aberto (Open Access) Tratamento endovascular de angiomiolipoma renal por embolização arterial seletiva(2012-12) PALÁCIOS, Renato Menezes; GÓES, Amanda Silva de Oliveira; AGUIAR, Maurício Figueiredo Massulo; RIBEIRO, Flávio Roberto Cavalleiro de Macêdo; GÓES JUNIOR, Adenauer Marinho de OliveiraWe report a case of a patient with a major complaint of left lumbar pain, diagnosed with bilateral renal angiomyolipomas (AMLRs), with the most voluminous lesion of 6.2 cm in its largest diameter, underwent endovascular superselective arterial embolization with microspheres. The AMLRs are rare benign tumors. Most are sporadic, while a minority is associated with Tuberous Sclerosis Complex (ETC). The AMLRs larger than 4 cm must be treated due to higher risk of complications, especially hemorrhagic. A selective arterial embolization (EAS) is an effective and safe treatment for AMLRs.Item Acesso aberto (Open Access) Evaluation of the therapeutic response of hepatitis C in coinfected patients (HIV/HCV): a study of cases from a hospital for chronic liver diseases in the Eastern Brazilian Amazon(2013-02) AMARAL, Ivanete do Socorro Abraçado; MÓIA, Lizomar de Jesus Maués Pereira; BARBOSA, Maria Silvia de Brito; DEMACHKI, Samia; ARAÚJO, Marialva Tereza Ferreira de; SOARES, Manoel do Carmo PereiraINTRODUCTION: The aim of this study was to evaluate the therapeutic response of hepatitis C in patients coinfected with human immunodeficiency virus (HIV-1). METHODS: A retrospective study of 20 patients coinfected with HIV-1/HCV who were treated in the outpatient liver clinic at the Sacred House of Mercy Foundation Hospital of Pará (Fundação Santa Casa de Misericórdia do Pará - FSCMPA) from April 2004 to June 2009. Patients were treated with 180µg PEG interferon-α2a in combination with ribavirin (1,000 to 1,250mg/day) for 48 weeks. The end point was the sustained virological response (SVR) rate (HCV RNA negative 24 weeks after completing treatment). RESULTS: The mean age of the patients was 40±9.5 years, of which 89% (n=17) were male, and the HCV genotypes were genotype 1 (55%, n=11/20), genotype 2 (10%, n=2/20) and genotype 3 (35%, n=7/20). The mean CD4+ lymphocyte count was 507.8, and the liver fibrosis stages were (METAVIR) F1 (25%), F2 (55%), F3 (10%) and F4 (10%). The early virological response (EVR) was 60%, the end-of-treatment virological response (EOTVR) was 45% and the SVR was 45%. CONCLUSIONS: The median HCV viral load was high, and in 85% of cases in which highly active antiretroviral therapy (HAART) was used, none of the patients with F3-F4 fibrosis responded to treatment. Of the twenty patients treated, 45% achieved SVR and 45% achieved EOTVR. Studies that include cases from a wider region are needed to better evaluate these findings.Item Acesso aberto (Open Access) Perfil dos cuidadores e as dificuldades enfrentadas no cuidado ao idoso, em Ananindeua, PA(Universidade Federal do Pará, 2013-03) ARAUJO, Jeferson Santos; VIDAL, Glenda Marreira; BRITO, Felipe Nunes; GONÇALVES, Débora Cristina de Abreu; LEITE, Djeane Kathe Mascote; DUTRA, Claudia Daniele Tavares; PIRES, Carla Andréa AvelarObjective: To describe the profile of caregivers of elderly, as well as their importance and main difficulties in caring for the elderly. Methodolog y: This was a descriptive, exploratory cross-sectional study conducted by the PET Saúde staff in a Family Health Strategy (FHS) in the city of Ananindeua, state of Pará, Brazil. Data was collected through a semistructured questionnaire addressing sociodemographic and issues concerning the importance and difficulties encountered by caregivers of the elderly. Results: We interviewed 31 caregivers, predominantly female, married state, with complete secondary education, income between two and three minimum wages, time of exercise of the function over 12 months, most of them informal caregivers and with family relationship with elderly. Caregivers acknowledged the importance of a caregiver and found the elderly under their care as healthy. Conclusion: Most caregivers were informal type, characterized by women with family ties, which emphasized patience and lack of knowledge as the main difficulties of the caregiver. Data from this study are limited to a local reality, requiring further research to foster the particularities and difficulties faced by caregivers.Item Acesso aberto (Open Access) Effect of copaiba oil in hepatic damage induced by acetaminophen in rats(Sociedade Brasileira para o Desenvolvimento da Pesquisa em Cirurgia, 2013-07) TEIXEIRA, Renan Kleber Costa; YAMAKI, Vitor Nagai; YASOJIMA, Edson Yuzur; BRITO, Marcus Vinicius HenriquesPURPOSE: To investigate the effects of copaiba oil on the hepatic damage induced by paracetamol. METHODS: Thirty six rats were distributed into six study groups (N=6): control group, that didn't receive the acetaminophen; Acetaminophen Group, that only received the acetaminophen; Prophylactic Copaiba Group 1, that received copaiba oil two hours before the acetaminophen; Prophylactic Copaiba Group 7, that received copaiba oil seven days, once by day, before the acetaminophen; Therapy Copaiba Group, that received the copaiba oil two hours afther the acetaminophen; and N-Acetyl-Cysteine Group, , that received the N-Acetyl-Cysteine two hours afther the acetaminophen. Euthanasia was performed after 24 hours. The serum levels of AST, ALT, alkaline phosphatase, GT, total bilirubin, direct bilirubin and indirect bilirubin and histological analisis were analized. RESULTS: The prophylactic copaiba group 7, therapy copaiba group and N-Acetyl-Cysteine Group showed amounts of AST and ALT similar to the control group; and the prophylactic copaiba group 1 showed similar levels to the acetaminophen group. There was no significant difference between the groups regarding the amount of alkaline phosphatase and GT (p>0.05). The therapy copaiba group showed the highest levels of bilirubin and was statistically different from the other groups (p<0.01) and this increased the costs of direct bilirubin. Regarding histopathology, the oil of copaiba administered prophylactic or therapeutic form for 7 days could decrease the amount of necrosis and inflammatory infiltrate. CONCLUSION: Copaiba oil administered prophylactically for seven days, and therapeutic could reduce liver damage caused by paracetamol similarly N-Acetyl-Cysteine, however, when treated with copaiba therapeutically showed increases in bilirubin, costs increasing fraction indirect.Item Acesso aberto (Open Access) A reumatologia em um curso de medicina com aprendizagem baseada em problemas(2013-12) CALDAS, Cezar Augusto Muniz; PAZ, Otávio Augusto Gomes da; NEGRÃO, José Nazareno Cunha; CALDATO, Milena Coelho FernandesOBJECTIVE: To present the experience of a Brazilian institution in the learning of Rheumatology at medical school and whose teaching project is structured into active learning methodologies. METHODS: Derived from a descriptive study with a qualitative approach to content related to Rheumatology in the medicine course of CESUPA. RESULTS: The musculoskeletal system is discussed in the second and seventh terms as part of the modules related to the locomotor system and Internal Medicine II, respectively, and each stage and activity has specific minimum objectives. In addition to the theoretical content, in the seventh term the students conduct Rheumatology ambulatory care where the focus is on preparing proper anamnesis and physical examination. During internship the students resume Rheumatology ambulatory care and at this point their diagnostic, investigative and therapeutic skills are the most required. CONCLUSION: There still is much to evolve in terms of finding an ideal model for teaching Rheumatology. L.Inflammatory bowel diseases (Crohn’s disease and ulcerative rectocolitis) have extraintestinal manifestations 25% of the patients, with the most common one being the enteropathic arthritis. Methods: Prospective, observational, multicenter study with patients from 29 reference centers participating in the Brazilian Registry of Spondyloarthritis (RBE), which incorporates the RESPONDIA (Ibero-American Registry of Spondyloarthritis) group. Demographic and clinical data were collected from 1472 patients and standardized questionnaires for the assessment of axial mobility, quality of life, enthesitic involvement, disease activity and functional capacity were applied. Laboratory and radiographic examinations were performed. The aim of this study is to compare the clinical, epidemiological, genetic, imaging, treatment and prognosis characteristics of patients with enteropathic arthritis with other types of spondyloarthritis in a large Brazilian cohort. Results: A total of 3.2% of patients were classified as having enteroarthritis, 2.5% had spondylitis and 0.7%, arthritis (peripheral predominance). The subgroup of individuals with enteroarthritis had a higher prevalence in women (P < 0.001), lower incidence of inflammatory axial pain (P < 0.001) and enthesitis (P = 0.004). HLA-B27 was less frequent in the group with enteroarthritis (P = 0.001), even when considering only those with the pure axial form. There was a lower prevalence of radiographic sacroiliitis (P = 0.009) and lower radiographic score (BASRI) (P = 0.006) when compared to patients with other types of spondyloarthritis. They also used more corticosteroids (P < 0.001) and sulfasalazine (P < 0.001) and less nonsteroidal anti-inflammatory drugs (P < 0.001) and methotrexate (P = 0.001). METHODS: Twenty rats were distributed into two study groups (N=10): control group (CG), that was not exposed to tobacco smoke and used as normal standard for biochemical and histological analysis; Experimental Group (EG), that Animals were exposed to the passive smoking; Euthanasia was performed after 14 days of exposure. The serum level of nicotine and histological analysis were performed. RESULTS: There was a statistical difference on the nicotine serum levels between Experimental and Control group, with level of 286 ±23 nanograma/mL in the EG and undetectable on CG (p<0.01). The histological study suggested the model efficacy producing alveolar destruction and emphysema in the EG compared with the insignificant lesions in the CG's lung. CONCLUSION: The model of exposure to environmental tobacco smoke for rodents induced easily the changes related to secondhand smoke.
