dc.description.abstractThe central aim of this doctoral thesis is to analyze the relationship between ecological quality of environment and the neuropsychomotor development and language of children enrolled in elementary school in Belém, based on bioecological theory. Two studies with similar methods were elaborated. The first study analyzed the relationship between the environment’s quality (family, school and neighborhood) and the neuropsychomotor development (NPMD) of children enrolled in elementary school in Belém. The study design is longitudinal, descriptive, correlational and quantitative. The elementary school involved in this study is located in the Batista Campos, Administrative District of Belém (DABEL), central region. 70 children participated in this study. To evaluate the NPMD, the Denver II was used in three times. The personal and contextual data were collected by the Child Characterization Questionnaire. The family environment quality was measured by the Home Environment Resources Scale (HERS) and the Family Poverty Level (IPF). The neighborhood environment quality was measured by the Neighborhood Quality Instrument (IQV) and the school environment quality by the Early Childhood Environment Rating Scale (ECERS-R). The collection of data occurs in the year 2016, the results showed an increase in the percentage of children with normal performance during the three evaluations by Denver II. The IPF (p=0.005), HERS (p<0.001) and IQV (p<0.001) scores were statistically significant associated with suspected delay. ECERS-R pointed to the school environment quality as basic or minimum, however there was no statistically significant association with the Denver II. The logistic regression model indicated that the probability of a child with suspected delay in DNPM is 80% lower when female (OR=0.20); 57% lower when married parents (OR=0.27); and 98% lower, the more joint activities the parents perform with the child in the family environment (OR=0.01), and it is 5.9 times higher the poorer the family (OR=5.98). In the second study, verified the relationship between ecological quality of environment, family and neighborhood and the child's language development. This study had a cross-sectional design, and a total of 64 participants who were evaluated by Denver II, ASQ-BR and Naming Test (TIN) and Phonological Discrimination Test (TDF). The majority of the children had a suspected delay by Denver II (56.3%) and below the cutoff point (51.6%) by ASQ-BR. In contrast, the mean scores (48.4%) for TDF and high (45.3%) for TIN were obtained. The personal variables that showed a statistically significant association were sex (p<0.007), time in school (p<0.04) and breastfeeding (p<0.01). Overall HERS scores (p<0.001) and IPF (p=0.01) were associated with the score. The logistic regression showed that the probability of a child presenting low language performance is 62% higher the lower the professional qualification of the mother (OR=0.39); 2.05 times higher, lower maternal schooling (OR=2.05), 1.97 times higher, lower family monthly income (OR=1.97); 4.91 times higher the lower the phonological discrimination of the child (OR=4.91); and 4.30 times higher the smaller the joint activities between parents and children (4.31). The data were analyzed by the Bioecological Theory and were coherent to analyze the results of the children as a function of the proximal processes, the characteristics of the person, the three microsystems investigated and the time interval of the research. Thus, the high prevalence of suspected delays in the development of the participating children highlights the importance of the emergence of policies and actions to stimulate children's development, besides alerting to the presence of risk factors that interfere with the quality of the environments.
dc.description.resumoEsta tese de doutorado teve como objetivo geral analisar se há relação entre a qualidade dos ambientes ecológicos e o desenvolvimento neuropsicomotor e de linguagem de crianças de uma Unidade de Educação Infantil do município de Belém, a partir de uma compreensão bioecológica. Para tal, foram realizados dois estudos, com características metodológicas semelhantes. O primeiro analisou a relação entre a qualidade de três ambientes ecológicos (família, escola e vizinhança) e o desenvolvimento neuropsicomotor (DNPM) de crianças de uma UEI de Belém, através de um delineamento longitudinal, descritivo, correlacional e com abordagem quantitativa dos dados. A UEI selecionada está localizada no bairro da Batista Campos, Distrito de Belém (DABEL), região central do município. Participaram 70 crianças e um familiar de referência de cada uma delas. Para avaliação do DNPM foi utilizado o Denver II em três tempos, ao longo do ano letivo. Já a coleta dos dados pessoais e contextuais foi feita a partir da aplicação do Questionário de Caracterização da Criança, aplicado aos pais. A qualidade do ambiente familiar foi obtida pelo Inventário de Recursos do Ambiente (RAF) e pelo Índice de Pobreza da Família (IPF). A qualidade do ambiente de vizinhança foi mensurada pelo Instrumento de Qualidade da Vizinhança (IQV) e a qualidade do ambiente escolar pela Escala de Avaliação do Ambiente em Educação de Infância (ECERS-R). A coleta de dados foi realizada no ano letivo de 2016, os resultados mostraram que houve aumento no percentual de crianças com desempenho normal, ao longo das três avaliações pelo Denver II. Os escore do IPF (p=0,005), do RAF (p<0,001) e do IQV (p<0,001) apresentaram associação estatisticamente significativa com a suspeita de atraso no DNPM. A ECERS-R apontou a qualidade do ambiente escolar como básica ou mínima, no entanto não houve associação estatisticamente significativa com o desfecho do Denver II. O modelo de regressão logística indicou que a probabilidade de uma criança apresentar suspeita de atraso no DNPM é 80% menor quando for do sexo feminino (OR=0,20); 57% menor quando os pais são casados (OR=0,27); e 98% menor quanto mais atividades conjuntas os pais desenvolvem com a criança no ambiente familiar (OR=0,01), e ainda é 5,9 vezes maior quanto maior o nível de pobreza da família (OR=5,98). Já o segundo estudo verificou a relação entre a qualidade dos ambientes ecológicos, familiar e vizinhança, e o desenvolvimento da linguagem de crianças da mesma UEI. Este, teve um delineamento transversal, e o total de 64 participantes que foram avaliados pelo Denver II, ASQ-BR e pelos testes de nomeação infantil (TIN) e discriminação fonológica (TDF). A maioria das crianças teve o escore suspeita de atraso pelo Denver II (56,3%) e abaixo do ponto de corte (51,6%) pela ASQ-BR. Em contrapartida obtiveram as classificações média (48,4%) pelo TDF e alta (45,3%) pelo TIN. As variáveis pessoais que demonstraram associação estatisticamente significativas foram sexo (p<0,007), tempo na UEI (p<0,04) e aleitamento materno (p<0,01). As pontuações gerais do RAF (p<0,001) e do IPF (p=0,01) associaram-se ao desfecho. Os dados da regressão logística mostraram que a probabilidade de uma criança apresentar pior desempenho na linguagem é 62% maior quanto menor for a qualificação profissional da mãe (OR=0.39); 2,05 vezes maior quanto menor a escolaridade materna (OR=2.05), 1,97 vezes maior, quanto menor a renda mensal da família (OR=1.97); 4,91 vezes maior quanto menor a discriminação fonológica da criança (OR=4.91); e 4,30 vezes maior quanto menores forem as atividades conjuntas entre pais e filhos (4.31). Os dados foram analisados a partir da Teoria Bioecológica os quais mostram-se coerentes para compreender os resultados das crianças como uma função conjunta do processo, das características da pessoa, da natureza dos três microssistemas investigados e pelo intervalo de tempo da pesquisa. Portando, a alta prevalência de suspeita de atrasos no desenvolvimento das crianças participantes realça a importância da emergência de políticas e ações de estimulação ou monitoramento do desenvolvimento infantil, além de alertar para a presença de fatores de risco que podem interferir na qualidade dos ambientes.
